Employee was loading sizing screen material, allowing screen to slip through his hands. A protruding metal burr caused a laceration to the palm of his hand. Injury resulted in stitches to the hand

Struck against stationary object

Employee was checking oil in haul truck, and put a flashlight in his mouth. He bit down on flashlight, which resulted in a small portion of tooth breaking off. Due to the existing decay present in the employee's teeth it was not immediately known whether this incident would require repair. After evaluation by dentist, a minor repair was made which made the incident reportable.

Struck against stationary object

While moving a water hose, employee bumped hand against a conveyor frame, causing a laceration to his right thumb requiring stitches.

Struck by falling object

While cleaning a plugged chute, a rock struck the employee in the back of his right hand. He was diagnosed with a hairline fracture in one of the bones in his hand. His hand was wrapped with an ace bandage and he returned to work on restricted duty.

Struck against a moving object

While operating a pit loader, the loader struck a large rock toe, jarring the loader. The loader operator suffered a muscle strain to his lower back and was placed on restricted duty.

Caught in, under or between a moving and a stationary object

While cleaning up, employee was moving a rock when he caught his right index finger between the rock and an I-beam. He suffered a laceration to the finger, requiring sutures.

Caught in, under or between running or meshing objects

Employee was assisting a mechanic to manually turn the drive on a Telsmith 68S crusher to prime the oil system when he accidently pinched his right little finger and ring finger in the v-belt against the sheave. The injury resulted in the amputation of the tip of his little finger and reconstructive surgery to the tip of his ring finger.

Struck by falling object

While changing a screen panel in the Deister wet screen, the employee lifted a panel over his face, banging it on the screen frame. Grit spilled into his face, past his safety glasses and into his left eye causing an abrasion of the cornea. His eye was flushed at the site. The company physician administered a prescription eye medication.

EMPLOYEE STATED THAT HE ATTEMPTED TO ROLL A ROCK FROM THE PATH OF HIS TRUCK. AS HE STOOD, HE FELT A ""POP"" SENSATION IN HIS RIGHT BICEP. HE WAS TREATED FOR A TORN BICEP MUSCLE.

WHILE PUSHING ON A RIP-RAP GATE, EMPLOYEE'S FINGER WAS CAUGHT BETWEEN THE GATE AND A PRY BAR USED BY A SECOND EMPLOYEE. EMPLOYEE'S FINGER WAS MASHED.

Fall from machine

CUSTOMER TRUCK DRIVER WAS EXITING HIS TRUCK CAB WHEN HE SLIPPED ON THE TOP STEP AND FELL APPROXIMATELY THREE FEET TO THE GROUND, RESULTING IN A BROKEN RIGHT ANKLE.

THE WORK AREA WAS WET FROM EARLY MORNING DEW WHILE TIGHTENING DOWN THE SCREENS SIDE RAIL BOLTS, THE EE WAS STANDING ON HIS MOTOR DRIVE WITH H IS LEFT FOOT AND THE NEARBY SECTION OF INSIDE HANDRAILING WITH HIS RIGHT FOOT. THE EE S RIGHT FOOT SLIPPED AND HE FELL AND HIT HIS RIGHT LOWER BACK ONT HE HANDRAILING. AS A RESULT, HE FRACTUR

EE WAS CLEANING CATWALK AROUND 68S TELSMITH CRUSHER. THE EE LAID HIS HAND ON TOP OF THE CRUSHERSHOUSING AND BENT OVER TO PICK UP SOME ROCK OF THE SURROUNDING CATWALK WHEN A ROCK OF UNKOWN SIZE BOUNCED DOWN OFF THE ABOVE 6X16 SCALPING SCREENAND HIT THE EE'S LEFT HAND.

EMPLOYEE HAD CUT OPEN AN OIL FILTER CANISTER ANDPLACED IT IN A VICE FOR INSPECTION. WHILE TURNING THE VICE HANDLE HIS RIGHT INDEX FINGER STRUCK THE OPENED EDGE OF THE CANISTER.

EE WAS HELPING CHANGE SCREENS WHEN HE STUCK HIS FINGER INSIDE THE SOCKET ON A 3/4""IMPACT WRENCH TO REMOVE A NUT STUCK IN THE SOCKET & ACCIDENTALLY HIT THE TRIGGER ON THE IMPACT WRENCH CAUSING THE NUT TO SCREW UP THE END OF HIS FINGER AND INTO THE FIRST LOWER FINGER JOINT. THE LACERATION REQUIRED STITCHES.

EE WAS RAISING THE BALL LATCH ON THE GREASE TRAILOR WHEN HE TOOK PIECE OF STEEL PIPE AND TRIED TO KNOCK THE LATCH UP. WHEN HE DID HE SMASHED HIS LEFT RING FINGER BETWEEN THE PIPE AND THE THREE WAY HITCH.
