Taking arm off of water truck and wrench slipped and hit underneath dash causing a cut to hand.

Fall to the walkway or working surface

Employee had arrived at work and parked in her usual parking space. As she exited her vehicle her foot slipped on loose gravel. She fell, resulting in a broken left ankle. She has been placed on restricted duty. She is scheduled to see an orthopedic physician on 3/5/07.

Struck against stationary object

Employee was operating a dump truck. He arrived at the dump site & got out to check conditions. When he stepped out of truck, he felt pain in his left knee. He continued to walk around truck. His foot slipped on a rock causing his left knee to twist. He suffered immediate pain & swelling. He was taken to the ER.

Absorption of radiations, caustics, toxic and noxious substances

EE was operating a R-50 Euclid off road dump truck when he was bitten by a spider in the upper right back. He thought he had only been stung but noticed considerable swelling and redness as time progressed. He reported the bite on 7/26/06. The physician treated the wound and returned the employee to work.

REMOVING A BUSHING FROM A SHOVEL BOOM WITH A HAMMER & PUNCH WHEN A SMALL PIECE BROKE OFF STRIKING HIM IN THE LEFT FOREARM.

USING A HAMMER TO HIT A WRENCH TO TIGHTEN A NUT.WRENCH SLIPPED AND FINGER WAS JAMMED AGAINST HAMMER MILL. PROPER TOOL WAS AVAILABLE, BUT WAS NOT USED.

Flash burns (electric)

A KEY HAD FELL THROUGH A SMALL HOLE IN THE TOP OF A SWITCH BOX. WHEN THE EE DISENGAGED THE SWITCH BOX IT ARCH. THE FLASH CAUSED HIS RIGHT HAND, ARM, AND FACE TO SUSTAIN FIRST DEGREE BURNS. EE WAS TRAINED IN PROPER LOCKOUT PROCEDURES. HE WASNOT FOLLOWING THEM AT THE TIME OF THE ACCIDENT.
