Fall to lower level, (Not Elsewhere Classified)

Employee is currently in the ICU unit. Crew was installing cross beams and noticed beams were to close together. Foreman went back to scaffolding and tied off to back rail, tried to pin and move beam over when structure clip slipped off of chime and employee fell with the beam. Employee was tied off to the structure.

EE and cohorts had lowered the counterweight following a repair from the previous day. EE went up in a manlift to observe operation when they became entangled in moving machine parts of the 48-inch conveyor. EE was degloved from the shoulder to the forearm, has multiple fractures including wrist, thumb, scapula and shoulder. This injury will require skin graphing as well.

Employee was greasing when EE stepped down from elevated platform (2 step) EE's left ankle rolled fracturing EE's fifth metatarsal. Employee was initially being treated with conservative care however went for X-rays on 06/11/2025 where it was determined to be fractured.

The employee was installing brackets for form scaffolding. While EE was trying to shift an alignment rail, EE had to loosen the alignment bolt. While hitting the bolt, the hammer ricocheted off the bolt and into EE's left hand EE had placed on a 4x4 form brace.

Struck against stationary object

EE ducked under the midrail just above the toe board to fit in a piece of material. On the way out the hardhat caught, EE flinched, EE's hardhat and glasses fell off. This resulted in EE contacting a piece of toe board with the bridge of nose. Four stitches were applied to 1/2 laceration.

EE WAS TRANFERRING FROM THE GANGWAY TO TRAILER WHEN LEFT HEEL OF BOOT CAUGHT ON THE DOME LID CAUSING THE EE TO STUMBLE. THIS RESULTED IN A STRAIN TO THE LEFT KNEE WHILE REGAINING THEIR BALANCE.

EE was replacing skirt material on CL- 303, decision was made to change size of material from a 3"" to 6"" skirt material. EE was utilizing a pair of needle nose vise grip pliers along with channel lock pliers to pull the material into position between the hold down bracket and chute when the vise grip pliers slip free striking the EE in the mouth fracturing the #9 tooth.

Absorption of radiations, caustics, toxic and noxious substances

EE was unplugging the 1651 DC, when material blew back striking EE in the left eye. EE was using air lance above the rotary valve slightly cracking the valve when fine material blew back striking EE's left eye. EE radioed for assistance to nearest eye wash. Following the initial flush EE was taken to the plumbed eye wash where additional material was removed.

EE had walked just outside the hydrate building North of blend loadout when EE collapsed falling to the ground. Medical opinion is that this occurred due to EE becoming overheated. However there is nothing to confirm the diagnosis. EE had a laceration requiring 5 sutures to close just over the left temple.

EE was engaged in the process of rolling out lay-flat hose when they caught the toe of boot on a small rock falling forward landing on right side. EE tried to catch themselves with right hand, however hand slid in wet material that was present. EE had multiple hair line fractures to ribs on right side and was restricted from normal work activity.

Employee was adjusting a wire rope sling, which was connected to a bar that assists in unrolling conveyor belt. The employee lost EE's balance and fell backwards. When EE fell backwards EE braced EE's fall on a pallet positioned behind self fracturing left wrist.

Struck by rolling or sliding object

Operator and rigger were replacing steel covers using a loader and chain rigging. The rigger removed the chain from one side of cover and signaled the operator to back up. Rigger did not signal the operator to stop and as they backed up the other rigging pulled across the riggers right foot. Fracture of the 5th metatarsal. Fracture boot for 6 weeks.

EE was going to check 1071, when EE bumped pipe which had been blanked off. Signage was in place, however the EE hit the pipe with their hard hat. EE had sharp pain in their neck area, however no medical treatment was sought. The EE was placed on restricted work duties and prescribed physical therapy.

Employee was closing lids on contractor trailer when thumb was caught between latch and dome lid. Employee had applied lubricant to free the latches when right thumb was caught in pinch point fracturing tip.

Struck by flying object

Maintenance employees were reinstalling discharge line on 6X6 pump when line blew off during start up. Employee was positioned to bleed air at the point when the line blew free striking the employee in the left forearm. Injury sustained was a laceration to the left forearm requiring twelve sutures to close.

Over-exertion in lifting objects

A Utility worker was lifting and setting plugs while participating in poke out at K3. Towards the end of the job, the employee felt discomfort in EE's groin. Medical attention was sought and the employee was placed on restricted work duties.

Employees began to lift up a block of refractory with a hoist & choker. The load was not centered below the hoist & it began to swing. One of the employees was in the path of the block. EE attempted to get out of the way. The block struck a crossbeam and continued to swing into lower left leg. EE was diagnosed with a hair line fracture in left leg.

Employee was attempting to remove the packaging from surveying tape. Employee went to cut zip ties with pocket knife. The employee's momentum from the upward force cutting the zip tie. The knife slipped and cut the employee on the left pointer finger. The cut was deep enough that medical treatment was required. The employee received 5 sutures to close the wound.

Hose ruptured on a Myers Unit sewer cleaning vehicle while being utilized to clean a railcar. The hose struck the employee on right leg below the knee resulting in a closed tibial plateau fracture.

Right angle grinder was used to dress-up the edge of a steel plate roof repair when the wheel wedged between the plate and the supporting channel iron kicking back into the workers face resulting in ten stitches in cheek and six damaged teeth.

Struck by falling object

Electrical motor failed on CL-1044m at the 1600 tower. Electricians were called in to replace the motor. Employees were engaged in the process of rigging a come-along from the elevation directly above the motor when material was dislodged entering employees right eye.

Employee shoved troughing idler through gate on aerial lift that was being utilized to transfer material and idlers from 1308 conveyor. Lift was lowered to with 6' to 8' of the ground and idler was shoved out by employee. Employee was struck on the right ankle by troughing idler. Ankle was significantly sprained and employed was placed on restricted duty with medication.

Two employees were removing material from beneath #4 storage, when employee was struck in the mouth with hose from vacuum truck. Hose connection was being taped and was rested against concrete wall, when hose connection slipped striking employee fracturing #9 tooth.

Struck against a moving object

Operator picked up the boulder, dropped it and the boulder did not break. During the second attempt the boulder was lifted closer to the edge/teeth. The rear wheels of the loader lifted off the ground and the sudden loss, or shift of weight caused the rear wheels to come down hard. The operator felt pain in his lower back. Diagnosis Compression fracture to a lower vertebrae.

Fall from scaffolds, walkways, platforms

Employee engaged in the process of demolition of #2 scrubber was injured when he fell while wearing fall protection harness attached to retractable lifeline. At some point employee fell through opening that was not protected by handrail. The employee fractured the humerus bone of his right arm near the elbow.

Fall from machine

Employee was on top of open top railcar observing a maintenance task, when employee lost balance and fell over the edge of the railcar. Employee fell to the ground and sustained significant injuries.

Employee slipped through bar grating while closing bottom hopper doors of rail car. Injury includes significant laceration to the left leg just below the knee. Seven sutures were required to close.

Employee was lifting hydraulic wrench on to pier when left pinky finger was pinched between timber and wrench when resting tool. Finger was lacerate at tip requiring three sutures to close wound.

Employee was assisting with the installation of connector plates in K2. Left pinky and index fingers were caught between connector plate and the stationary heat exchanger. As a result pinky nail was lacerated requiring sutures to close, and was fractured at the first joint. Index finger was bruised and was noted without further injury.

Employee was opening container lids when bar used in the process slipped striking employee in the lower lip causing laceration and fractured tooth.

Struck by falling object

Employee was replacing heater hose on Komatsu water truck when hood of engine compartment fell striking and compressing employee rib cage between hood and engine! Employee did not choose medical attention initially, however on 8/2/11 medical attention was requested and determined a fractured rib!

Contact with heat

Employee became dehydrated during course of workday and was relieved of duty the following day.

Over-exertion in lifting objects

Employee was utilizing a pry bar to lift dolly carrying 150hp electrical motor over uneven floor surface. Employee has a lower lumber sprain and sprain of the sacrum. He is significantly restricted regarding duties.

Struck against a moving object

Employee lost control of haulage unit, ran onto berm overturning unit. No significant damage to haulage unit, however employee sustained a contusion to the left shoulder region.

Employee was closing doors at primary screening facility, when swing arm used to secure doors swung striking left index finger requiring sutures to close the wound.

Fall onto or against objects

Employee was dismounting Komatsu Haul Trk in wet muddy conditions when foot slipped off access ladder. As a result a fall insued, and the right arm was lacerated on the toe plate of access platform. Injury required sutures to close the wound.

Employee slipped and fell on wet surface straining left shoulder. This was initially classified as a first aid treatment that has been reclassified as a medical treatment. The reclassification occurred on 09/02/2010 as a result of medical intervention.

Employee crushed and lacerated the fingertip of the RT index finger while storing ramp to the oil contaiment beneath pier 3 no. 2 kiln! Finger was crushed between ramp and 55 gal barrel of oil!

Employee has a RT ankle fracture due to rolling the ankle when he stepped down from the 1321 screen deck and onto a large stone along the walkway of the 1321 screen!

Employee was stepping down from haul truck to walking surface, and felt a twisting sensation in his right knee. Incident was filed as a precautionary report on 12/09/2008, and was changed to an RWI on 01/07/2009 as a result of the restrictions placed on the employee.

Employee was repositioning himself from one elevation to another. In doing so he contacted a piece of angle iron with his left knee causing a three centimeter long laceration that required sutures to close.

Employee stepped over a small vertical wall from one elevation to another, in doing so he stepped onto a 4 inch symmetrical hose laying on the ground causing his foot to roll out and in turn shift his weight onto his left knee. His leg is immobilized to a point that he is restricted and unable to return to his normal job function.

Over-exertion in lifting objects

Strained Thoracic muscle while opening container lids. Employee was restricted to no repetitive lifting, pushing, and or pulling of more than twenty pounds. Therfore employee could not return to his assigned task and work within these restrictions.

ACCIDENT OCCURRED DURING SHIFT CHANGE. EMPLOYEE HAD COME DOWN FROM SCAFFOLD & HAD TO CROSS UNDER SCAFFOLDING TO EXIT WORK AREA. EMPLOYEES CLIMBING ON THE SCAFFOLD FOR NEXT SHIFT KNOCKED A BRICK OFF SCAFFOLD AND IT STRUCK EMPLOYEE ON HIS UPPER BACK.

Employee was performing routine maintenance on the CL1667 screw, when lime entered his right eye. Employee received a burn to the cornea and conjunctival sac of the right eye.

Employee was welding on the CL- 300 system when he received cornea abrasions due to welding burns involving both eyes! Contributing factors include failure to inspect tools and equipment prior to performing task assigned!

Employee slipped and twisted right knee while crossing a warning barrier! Contributing factors include water flowing through area due to recent heavy rains! Injuries as a result of the activity include a sprain of a unspecified site of the right knee! Restrictions as a result include no squatting or kneeling, use of crutches 90% of the time and no climbing stairs or ladders!

EE WAS UTILIZING A RIGHT ANGLE GRINDER TO REMOVE ROUGH EDGES IN PREP. OF WELDING TASK ON THE CL-520 SCREW IN THE QUARRY/MAINTENANCE SHOP. EE INDICATED THAT SAFETY GLASSES WERE BEING USED AT TH E TIME OF THIS ACCIDENT HOWEVER A FOREIGN BODY ENTERED THE LEFT EYE REQUIRING MEDICAL TREATMENT TO BE REMOVED.

EE WAS WALKING BETWEEN THE NEW LOADOUT AND THE STORAGE TANKS WHEN A PIECE OF LIME BLEW OFF STRIKING THE EE IN THE LEFT EYE. SAFETEY GLASSES WERE BEING UTILIZED AT THE TIME OF INJURY.

EE WAS INSPECTING CRACKS IN THE SHELL OF THE #3 KILN PRIOR TO REPAIRS BEING PERFORMED. THE EE WAS STRUCK IN THE RIGHT EYE BY A PIECE OF FLYING DEBRIS DURING THE INSPECTION. THE FOREIGN BODY STRUCK THE RT. CORNEA CAUSING A SUPERFICIAL INJ.

EMPLOYEE WAS REMOVING MATERIAL BUILDUP FROM #2 CHUTE IN 1300'S TUNNEL WHEN THE CHUTE GUARD ACCIDENTLY FELL ONTO BAR BEING USED TO DISLODGE MATERIAL. THE GUARD FELL ONTO THE BAR CAUSING THE BAR TO JUMP UP PINNING EMPLOYEES FINGER BETWEEN THE BAR AND STRUCTURE. EMPLOYEE REFUSED MEDICAL ATTENTION AT TIME OF ACCIDENT. EMPLOYEE WENT TO THE DR ON 9/24/02. IT WAS DISCOVERED FINGER BROKEN.

EMPLOYEE WAS WALKING IN THE BAGGING BUILDING AND STEPPED ON A BROKEN PALLET. THE PALLET HAD A NAIL STICKING THROUGH ONE OF THE BOARD AND THE EMPLOYEE STEPPED ON IT PUNCTURING A HOLE IN HIS FOO T.

EE SAYS HE FIRST FELT HIS BACK HURTING WHILE OPERATING THE DRAGLINE CLEANING OUT PONDS. UNKNOWN WHEN IT STARTED HURTING OR WHY IT WAS HURTING. HE CAN'T PLACE ANY SPECIFIC INCIDENT AS TO THE ST ART OF THE PAIN. EE ISN'T SURE EVEN THAT IT HAPPENED HERE AT THE PLANT DURING WORK, BUT IT DID START HURTING HERE. EE DID NOT START LOSING TIME UNTIL THE DR. PLACED HIM OFF DUTY 7-17-00.
