Employee was crossing top of #0 tank and encountered a puddle of water and attempted to ""long step"" it when EE slipped and strained EE's lower back

Struck by flying object

Employee was sprayed with talc in the eyes. Another employee turned on the #11 bagger without a bag on the spout and didn't notice ee standing behind self.

Employee was attempting to exit the operators chair on #9 bagger to put up more bags when shoelace got caught on a bar causing the employee to trip. EE tried to catch self with hands resulting in the injury.

Struck by falling object

Employee was filling 2000# bulk bags with two fellow employees. The bag had been removed from the support and was being prepared to be lifted by straps when it started to lean. EE tried to hold it until the forklift could pick it up. EE was unable to hold it and it tipped over. EE was too slow in getting out of the way.

Inhalation of radiations, caustics, toxic and noxious substances

Employee was bagging on the #10 bagger and the bag busted and spewed talc into the air and employee inhaled a large amount of talc through nose and mouth. Employee was not wearing the issued respiratory gear.

Fall to lower level, (Not Elsewhere Classified)

The employee was closing a sliding door at the loading dock. The removable guard rail was not in place, leaving an open dock approximately 4' above ground level. As the employee pulled on the door to close it, it began to move causing him to loose his balance and fall to ground level. He landed on his feet twisting his right knee.

Employee was filling bulk bags. It was a long step down from the platform he was standing on. When he stepped down, he felt a pinch in his abdomen.

Over-exertion in lifting objects

Employee was cleaning up. He lifted and moved a 50# bag to empty it. As he did this he felt a pain in his groin area.

Fall from scaffolds, walkways, platforms

Routine maintenance and leaned on handrail, handrail gave way due to broken welds. Fell 6'6"" to the concrete floor. Due to fall he fractured spine, broke sternum, lacerated his head.

While employee was loading a flatbed trailer with a forklift, a strong gust of wind came through a picked up a sheet of R-panel from the bone yard and carried it across and landing on the right side of the forklift. Employee was able to see it coming that he rapidly turned his back to it. The sheet of R-panel did not hit him but in the rapid movement the strained his neck.

THE EMPLOYEE WAS STACKING BAGS NEAR THE #9 BAGGER WHEN HE SLIPPED ON THE WET FLOOR. HE HIT HIS RIGHT KNEE, RIGHT ELBOW, AND HIS HEAD ABOVE THE LEFT EYE WHEN HE FELL DOWN.

Left lower leg was caught under bagger roller stand at time of turn of body to set bag on pallet.

Maintenance mechanic was positioning a Euclid haul truck's gear on the truck's axle and strained his lower back.

Struck against a moving object

Employee was moving a railcar and applying the wheel brake. The railcar did not stop completely when connecting to another railcar causing the employee to strike the railcar's railing. Employee bruised the right side of his ribcage (no fractures). Doctor issued 2-days of lost time. Employee returned to work full-duty on Monday, May 22.

Unclear. Employee's statement is that as he was stacking 50-lb. bags at the end of the conveyor belt from No. 3 Bagger he felt a sharp pain in his right wrist. Employee's wrist was not immobilized (cast) with work restrictions until April 10.

Fall down stairs

After finishing maintenance work, employee slipped and fell off last step from Micro Mill access resulting in a diagnosed medial gluteal strain of his right hip and was issued work restrictions.

Injured employee was stacking 50-lb. bags on a wooden pallet. After laying a bag on a pallet he raised himself back upwards and felt discomfort in his back. The discomfort continued so he was taken for medical examination and diagnosed with a lower back strain. Treating doctor issued work restrictions.

Injured employee was stacking 50-lb. bags on a wooden pallet. He reached down to adjust a bag on the wooden pallet and alleges he felt a burning pain on his Right Side. Treating physician at the Hospital suspected a Hernia; however, Specialist confirmed as Groin Strain on April 26, 2005. Employee has work restrictions and has returned to work on Light Duty.

Employee was descending the Euclid E-25 Haul Truck's front access ladder and stepped off the ladder onto the ground and alleges he twisted his Left Knee causing a Strain. It was not clear until April 19, 2005 after the injured employee was seen by a Specialist whether the injury was work-related. Injured employee is having Left Knee Surgery April 27, 2005.

EMPLOYEE WAS BAGGING TALC, THE BAG SLIPPED AS HE REACHED FOR IT, TWISTING HIS LEFT WRIST. HE DIDN'T TELL US UNTIL 2-24-00. SENT HIM TO THE DOCTOR, THE DOCTOR PUT A WRIST BAND ON IT AND HE RETU RNED TO WORK.
