Fall to the walkway or working surface

Employee was dismounting the front end loader when he stumbled striking his right wrist on the ground resulting in a fracture

Caught in, under or between running or meshing objects

He was changing out drive shaft on vessel and was going back in with new shaft while still attached to chain hoist he stopped hoist to rest, placed his hand on shaft, weight shifted causing shaft to move and pinch his finger in between chain, shaft and vessel.

Caught in, under or between a moving and a stationary object

Employee was removing a cyclone to change out. When he started lifting the cyclone, it suddenly turned and pinched his hand against a brace in cyclone tub and mashed his fingers/ hand.

Over-exertion in lifting objects

Was riffling a coal sample and was turning his body while carrying a the sample, weighing approximately 70 pounds, and his left knee gave out on him. There was swelling and he could not put a lot of pressure on his leg.

Employee was working on a seperator in the plant when he got his left hand caught in a pinch point under the seperator resulting in one mashed finger and one fractured finger.

STEPPED BACK INTO A SMALL HOLE WHILE FUSING PIPES TOGETHER - SPRAINED ANKLE

Struck against stationary object

Working in trench drain, stepped on rock and twisted ankle. Did not find out ankle was broken until 10/30/09 and sought treatment.

Employee slipped on a metal walkway, resulting in a laceration on his lower leg, that required 5 stitches to close.

Contact with hot objects or substances

Employee was operating a 988 Caterpillar Front End Loader when it caught fire. The employee recieved a couple of second degree burns as he was exiting the loader. The loader was completely destroyed and the fire was not contained in a 30 minute period.

Pulling on slate bar to shut doors on railroad cars and felt a twinge in lower back.

Closing doors of railroad cars with a prybar and slipped and fell on some ice.

EMPLOYEE WAS INSTALLING A PIECE OF DUCTWORK THROUGH A BLOCK WALL. HE WAS STANDING WITH ONE FOOT ON THE 4TH STEP OF A STEP LADDER & ON A PIECE OF HANDRAIL WHEN HIS FOOT SLIPPED OFF. HE FELL 4' STRAINING HIS RT. KNEE.

Employee was carrying an outrigger pad around to set up on the boom truck. While carrying the pad, he stepped in a hole in the ground and felt a sharp pain in the groin area.

Employee was trying to pick up on a steel beam to work it into position for installation, when he felt a sharp pain in his groin area. Not reported to supervisor until 7/15/05.

Struck against a moving object

Driver had 2 previous verbal warnings concerning speeding. Turned over truck was found across tracks at bottom of ""A"" side lying on its driver's side. Transmission was in neutral, Jake turned off, engine still running, brakes still hot to touch, 1 hr 45 min later. Driver had attempted to shift gears going downhill & got the truck out of gear. Results, runaway truck.

Employee was in preparation dept. crushing coal samples. When he lifted the lid on crusher to clean, wt. approx. 70 lbs., it fell back down and hit hand

EE TRIPPED ON A SMALL MOUND OF DIRT. AS HE TRIED TO REGAIN HIS BALANCE HIS RIGHT FOOT TWISTED, CAUSING HIM TO FALL IN A SITTING POSITION WITH THE MAJORITY OF HIS BODY WEIGHT LANDING ON HIS RIGHT ANKLE.

DUE TO RATE OF SPEED, A LACK OF STOPPING POWER CAUSED EXCESSIVE SPEED BEYOND HANDLING CAPABILITIES THEREFORE RESULTING COAL TRUCK TURNING OVER INTO DITCHLINE & SLIDING ON ITS SIDE APPROX. 20 FT. ON ITS SIDE BEFORE COMING TO A STOP.
