The Injured Person (IP) was cleaning out the Roto-packer and bent over to cut a partially empty 50 lbs bag to discard remaining contents, and as the IP bent over, EE felt a pop in EE's lower back (left), and the IP had to take a minute before standing up straight. IP reported the incident to the Lead Operator. IP was taken to Urgent Care.

EE's description, 'While cleaning up from a job at 4:50pm in Alesa 1 building I was putting away my tools. While trying to remove a bit from my drill. My glove became caught on the drill and spun my glove with my fingers backwards. This resulted in spraining my ring finger on my left hand. Gloves where brand new from issue less than an hour old.'
We were dumping the grinding media balls (metal balls) out of the CF1 Ball Mill into barriers (constructed of 4x8 plywood, etc). One of the balls bounced out of the barriers, bounced up and hit a supervisor in the forehead. It caused a laceration that required 5 stitches.

Temporary Employee got dust in left eye. Rinsed at eyewash station, continued to work. Reported to work the next day with eye still irritated. The employee had shoveled up dust into a bucket and upon dumping the bucket into a larger hopper, got dust into the eye. EE was wearing safety glasses but not safety goggles.

An operator was standing on the tracks of a bulldozer, when the operator inside the cab, accidentally hit the travel lever. The operator had foot pinched between the tracks and the dozer frame. EE broke 3 toes on left foot. One was removed at the hospital Friday night. Expected to make full recovery.

Employee dropped roll of plastic (90 lbs) when team-lifting the roll. The roll needed to be replaced on our stretch-wrap system. Roll fell and struck top of right foot. Fracture to metatarsal bone.

Lab tech, moving 3 gallon buckets (weighing 7-10 lbs) from stacked on a pallet, to a pull cart. Some buckets that were stacked 5-6 high fell over onto him as he was kneeling. Buckets hit the employee in the head (wearing hardhat). Headache resulted. No other injuries. Became a Lost Work Incident on August 15th (Dr. gave 10 Days Off).

The mill had 2 scall man doors removed for maintenance. The repairman entered through the upper door via a ladder. He accidently put one foot into the lower door hole, lost his balance and fell onto his left side inside the mill. He was the first person to enter the mill and had not yet put a temporary light in place or covered the lower door hole. He has a torn rotator cuff.

The production utility man was disconnecting a 4"" vacuum hose from a manifold and when it came loose it hit him on the lower lip and caused about a 0.25"" split. He was taken to the hospital where a doctor glued the cut closed. He was immediately released to go back to work.

A repairman was hammering a pin punch with a 20 Oz hammer to drive out a pin. The hammer glanced off the punch and traveled down and hit his thumb. The thumb sustained an open fracture on the tip of the thumb and a laceration of the nail bed. He returned to modified work with the use of his right hand only.
Mechanic was hammering a wedge to hold a crusher liner in place. Hammer glanced off of the wedge and traveled down and hit his other hand. He was supporting his weight with the hand that was struck. The position was slightly awkward and left his supporting hand in the line of fire. Working surface was somewhat unstable.

The worker went to a mill bin to get the cement to flow. He attached high pressure to the internal air pads and tapped on the bottom to loosen the material. While checking for flow at the discharge, the seal came out from under the holding seal band and blew cement on him knocking off his safety glasses and getting into his eyes.
A contract laborer was assisting cleaning up a lime spill with a shovel and a big bag with several others. After he went home he noticed a burns on the upper inside of his legs. He went to the doctor the next day and the doctor treated the burns and told him to come back in 2 days.
A contract laborer was assisting a lime spill cleanup with a vacuum hose. He noticed a burn on his neck later that evening and went to a doctor on monday. It appears that some lime dust got on his neck and reacted with body moisture.

EE WAS CALIBRATING A CEMEMTN PACKING MACHINE. THE 2 CALIBRATION WEIGHT WERE 25KG EA. AND EE WAS LIFTING THEM OFF TOGETHER. AS HE WAS LIFTING THEM OFF THE CRADLE, HE TURNED TO HIS LEFT TO EXIT THE PACKER WHEN HE EXPERIENCED PAIN IN HIS LOWER LEFT BACK. HE PAUSED AND THEN CONTINUED TO WALK AROUND AND SET THE WEIGHTS DOWN. ANOTHER WORKER WAS WATCHING AND NOTICED EE WINCING IN PAIN.
EE USED A WELDING ROD TO HOLD THE LARGE SOCKET ON THE WRENCH AS DID EVERYONE ELSE. AS HE WAS RELEASING ENERGY BY RUNNING THE WRENCH OUT OF AIR, THE WRENCH HE WAS HOLDING SLIPPED AND HIT THE MI LL AND BOUNCED TOWARD HIM. SPINNING SOCKET HIT HIS RIGHT KNEE AND THE WELDING ROD PUNCTURED HIS JEANS AND SKIN.
A REPAIRMAN WAS TOSSING 75 LB LINERS OUT THE MILL DOOR THAT WAS POSITIONED ON THE SIDE. THE LAST LINER HE THREW HIT THE SIDE OF THE DOOR FRAME AND BOUNCED BACK IN AND SMASHED 2 LEFT HAND FINGE R TIPS BETWEEN THE DOOR FRAME AND THE LINER. SOME BONE WAS REMOVED FROM THE TIP OF ONE FINGER AND BOTH FINGERS REQUIRED STITCHES. BOTH FINGER NAILS WERE REMOVED, TRIMMED AND THEN PLACE BACK.

EE WAS CHECKING A PUMP SYSTEM OUT FOR OPERATION AND NOTICED PRODUCT LEAKING FROM GASKET ON MAIN AERATION NOZZLE. HE KNEELED AND TURNED SIDEWAYS AND WAS LEANING AGAINST THE AIR MANIFOLD WHILE TIGHTENING A BOLT WITH SOME STRAIN WHEN HE FELT A POP IN HIS LEFT SIDE WHICH TOOK HIS BREATH AWAY AND SENT A SPASM THROUGH HIS BODY.
EMPLOYEE WAS BREAKING UP LUMPS OF CLINKER THAT WERE CLOGGING A GRATE. THE HANDLE OF THE MAUL SHATTERED AND A WOODSPLINTER FLEW UNDER HIS SAFETY GLASSES AND LODGED IN HIS RIGHT EYE.
THE INCIDENT OCCURED ON 3RD FLOOR OF PACK HOUSE. EE WAS REMOVING METAL COVER FROM A SLIDE CONVEYOR THAT WAS FIXED BY SILICON. EE WAS PRYING OFF COVER W/HAMMER & SCREWDRIVER. HE WAS STRUCK ON T OP OF THE NOSE W/COVER WHEN IT WAS PRIED LOOSE. EE NEEDED 5 STITCHES BUT DID NOT HAVE ANY LOST WORK DAYS OR RESTRICTED DAYS AS A RESULT OF INJURY.

EE WAS UNLOADING A RAILCAR AT THE PLANT, HAMMERING ON A HOPPER TO IMPROVE PRODUCT FLOW. HE HEARDA SOUND CHANGE IN THE CAR'S AIRLINE THAT INDICATED LESS AIRFLOW AND HIGHER PRESSURE. AT THAT INS TANCE THE AIRLINE CONNECTOR SUPPLYING UNLOADING AIR BLEW OFF THE CAR & HIT HIS ARM. ONE OF THE 2METAL STRAPS THAT TIE THE HOSE TO THE CONNECTOR HAD 9"" OF LOOSE EXCESS.
EMPLOYEE WAS LOADING CLINKER INTO FEED HOPPER AND BREAKING UP LUMPS WITH MALLET WHEN A PIECE OF CLINKER FLEW UP TO HIS EYES AND CHATTERED THE RIGHT SAFETY LENS IN HIS GLASSES.
