Over-exertion (Not Elsewhere Classified)

Continuous miner lost power, was lifting miner cable out of mud to check cable splices, while lifting cable out of mud, right foot sumped up and went to pull it out and twisted and felt something pop in left knee.

Accident type, without injuries

Roof fall occurred in the number 5 entry between spad 78 and 88, in a area that was mined in excess of 10 years ago. The area showed no evidence of taking weight or roof cracking prior to the fall. The fall occurred 18 crosscuts from the surface in the intake entry with the cfm excess of 250,000 exposing the roof to atmospheric weathering and temperature changes.

Employee was dragging feeder cable with ride, when EE stood up and twisted left knee.

Struck by falling object

EE was helping load scoop bucket, piece of rib fell striking EE in left ankle, impact caused EE to fall.

Over-exertion in wielding or throwing objects

IE and another EE were throwing miner cable and waterline against rib in crosscut between #3 and #4. When slinging the cable and waterline IE felt a pain in lower back. IE worked on Jan. 3rd and 4th and did not seek medical attention until Jan. 6th.

Struck against stationary object

While operating mantrip, employee view was blocked by case of water. When they reached up to push it out of their way, they struck their head on the overcast of #3 belt. Employee did not seek medical attention until 02/14/2019 when they were taken off work pending MRI of their neck.

Over-exertion (Not Elsewhere Classified)

Employee stepped forward and slipped on miner cable causing them to fall and twist a knee. after seeking medical treatment, they went back to work. EE has continued to have issues with the knee that has lead to lost time during each follow up with doctors. On April 4th, they underwent an ACL surgery. EE was taken off work until reevaluation on May, 7 2019.

Struck by falling object

Employee was cutting left sump out of #7 head, cable pulled under the miner. Employee shut off miner and walked up to remove cable a piece of draw rock fell hitting EE on the outside of EE's left foot. Rock was approx. 3' long, 3' wide and 5"" thick. Employee was seen in the ED where x-rays revealed two broken metatarsal bones and EE received 7 stitches.

EMP was cutting the flapper from the take up on M5 Drive. As ee was cutting the belt, the knife slipped cutting left hand. EMP was treated and received 6 stitches.

Struck against stationary object

EE was resetting miner breaker turned to walk away & stepped on a nail approximately 3"" long, penetrating about 2"" into right foot. EE went to ER was on crutches, foot swollen, red, & tingling. EE followed up with doctor & returned to work full duty with no restrictions on the next scheduled workday (06/21/2017). EE missed two scheduled workdays.

Employee was cutting conduit off imer cable, holding cable in left hand, cutting away with right hand. When knife blade slipped cutting employees left palm. Employee received 3 stitches in the ER.

Struck by flying object

EE pulled shuttle car underneath the miner conveyor boom to begin loading. During loading, apparently a rock stopped the conveyor, resulting in the failure of the boom jack and sending several parts flying through the air, most landing on the shuttle car near the operator's deck. When the jack failed, the pin flew through the air, striking EE on right Knee Cap.

Struck by flying object

Employee was beating on a bolt with a hammer. A sliver of the hammer head came off and went into the index finger of his right hand.

Fall onto or against objects

Employee was walking up steps into the security office in the rain. She is uncertain if she slipped or tripped but she fell forward landing face down striking her nose on the steps resulting in a fractured nose. Doctor released her to return to work the same day with no restrictions. She reported she was using the hand rail at the time of the incident.

Employee was loading supplies onto roof bolter and his knee popped.

Pinner operator was putting up a roof bolt. His hand slipped from chuck and got caught between chuck & head of bolt. Hand was cut between thumb and first finger.

Over-exertion (Not Elsewhere Classified)

The ee stepped into a muddy spot, while removing his foot from the mud he experienced back pain. The ee finished his shift & worked the remainder of the week before seeking medical attention. This ee has had surgery to his back following a reportable incident in 2008. The doctors office informs SunCoke that ""he will likely continue to have problems at work or not"" (3/18/2013).

Over-exertion in pulling or pushing objects

EE was pulling cable on a roof bolter when he felt a ""pop"" in his left knee. He continued to work but the knee ""popped"" again. The ee later reported pain in his knee & went to the local hospital for exam. He was referred to a panel dr. for a follow-up.

Accident type, without injuries

A continuation of a previously reported roof fall occurred in the #1 entry just inby survey station 173 on the G-Section. The fall area measured 40' in length X 20' in width X 6' in height. The area had been bolted with 48"" resin bolts. The fall area has been dangered off to prevent access.

Fall onto or against objects

EE was walking up steps to office door and failed to raise foot high enough for step resulting in the toe of his shoe catching the step causing him to fall onto the floor and into the door jam. Suffered minor abrasions to right leg and left cheek of face. No first aid was required.

Accident type, without injuries

A roof fall was discovered during the second shift pre-shift exam at survey station 196 and continues one break outby. The roof in this area was shale with sandstone streaks and was bolted with 4 foot resin bolts and supplemented with 10 foot cable bolts. The fall measures 90 feet long x entry width x 7 feet in thickness. Timbers were installed to prevent access to the area.

Caught in, under or between a moving and a stationary object

Two employees were installing leaky feeder cable. After the cable was installed the two employees were attempting to remove the cable spool from the cart when the reel moved and caught the ee's middle finger on her right hand between the reel and the cart frame resulting in a laceration to the tip of her finger.

Struck by falling object

The employee was constructing a brattice between the #2 and #3 entries. While working in a restricted area between the brattice and the rib a section of the rib measuring 63 inches long, 21 inches wide and 4 inches to 11 inches thick fell striking him on his lower back and left leg.
