Struck against stationary object

During bulk bag packaging forklift driver was mounting forklift to operate, to remove a palletized bag of lime from the loading machine. As he was engaged in reaching for his third point of contact his right hand impacted the handle mounted on the side of the chair injuring his finger. Employee taken to clinic for further assessment. X-rays showed broken pinky finger.

Miner was cleaning coal mill grizzly --- fractured tip of left pinky with steel bar.

Hand was pinched between chain come-along and pipe elbow while moving a pipe elbow into place. Chain come-along slipped on metal, finger got in the way.

Under investigation. The individual involved in this incident is contracted by one of our customers to pickup product at our plant. We have been requesting follow-up information since the day of the incident, but have not received any information or documentation from our customer or from the individual involved in the incident. We do not know yet if this incident is reportable.

Accident is under investigation (EE slipped and hurt his back setting up a rail car).

Accident is under investigation: call to company obtained further information. EE had removed his hood to take a broken bag off of the packing machine. His shoulder accidentally hit the button that started the machine and he got lime dust in his eyes. He is a temporary worker, hired for the April thru October season and never returned to work.

A contractor entered the bag house hopper for maintenance on a deflector plate. He entered the hopper without a lifeline or locking out the systems. While inside a large amount of hot lime dust was knocked loose from above, engulfing him. He got out before completely engulfed with several injuries, including a fractured hand, twisted wrist, bruised knee, ankle and lung damage.

Employee was cleaning out chute. He ""hit the chute with a bar and smashed his right index finger between the bar and a beam"". Avulsion to right index finger.

EMPLOYEE GRABBED A 35 LB. BAG OF LIME THAT FELL OFF A CONVEYOR AND PULLED HIS LEFT SHOULDER. HE HAD PREVIOUSLY HURT HIS SHOULDER TWICE IN THE SAME LOCATION BEFORE WORKING HERE. HE'S UNDERGOING PHYSICAL THERAPY. HE MAY NEED SURGERY ON ROTATOR CUFF.

EE WAS MANIFULATING A LOAD OUT HOSE AND IT WAS NOT MOVING. HE WAS PUSHING AND PULLING AND STRAINED HIS LOWER BACK. CHIROPRACTOR SAYS A DISK SLIPPED AND PINCHED A NERVE. THE DISC WAS RE ALIGNTE D AND JUST WAITING FOR NERVE SWELLING TO GO DOWN AND EE SHOULD BE BACK TO WORK.

EMPLOYEE WAS STEPPING DOWN FROM A PIECE OF EQUIPMENT ONTO A WALKWAY AND TWISTED HIS LEFT KNEE. NO IMMEDIATE INJURY, LATER THE KNEE SWELLED. EMPLOYEE REPORTED IT TO MANAGEMENT ON 10/17/01.

A BAGHOUSE DISCHARGE PLUGGED & THE DUST WAS DUMPED OUT OF THE HOPPER ONTO THE GROUND. THE EEWAS SENT OVER TO VACUUM UP THE SPILL WITH A HIVAC INSTEAD OF STARTING AT EDGE OF PILE HE WALKED INTO IT GOING OVER TOP OF LOW TOP STEEL TOED SAFETY BOOTS DUST (LIME DUST) GOT INTO HIS BOOTS CHEMICAL BURN

EE WAS CUTTING UP SCRAP METAL AND PARTS TO PUT TINTO A RECYCLE BIN. HE WAS PUTTING METAL IN THE BIN AND GOT A PAIN IN HIS GROIN. HE KEPT WORKING ALL DAY AND REPORTED IT 2 DAYS LATER. ON THE 20 TH OR 21ST HE WENT TO SEE HIS DR WHO SAID HE HADA HERNIA. UPON OPERATION THEY FIXED 4 HERNIA'S OCT 5TH.

WAS BAGGING HYDRATED LIME WITH A PROTECTIVE HOOD ON. HE SAYS HE BENT OVER AND THE HOOD WAS CAUGHT ON SOEMTHING AN DIT PULLED OFF HIS HEAD AND AT THE SAME TIME THE START BUTTOM WAS PUSHED BLOW ING HYDRATE INTO HIS EYES. TEMPORARY LOSS OF VISION DUE TO HYDRATE IN EYES. 4 DAYS WITH BLURRED VISION IN 1 EYE.

EE STEPPED INTO A PILE OF HYDRATED LIME & GOT SOME INSIDE HIS RIGHT BOOT. HE DID NOT NOTICE UNTIL HE GOT HOME THAT IT IRRITATED HIS SKIN AROUND THE TOP OF BOOT & CAUSED A BLISTER AROUND HIS ANKLE. HE WAS GIVEN BANDAGE & PAIN MEDS HE WAS OFFERED LIGHT DUTY & REFUSED. WHEN TOLD WE HAD AN OFFICE JOB FOR HIM HE QUIT AFTER 8 DAYS OFF. ""BLISTER""

Struck by falling object

EMPLOYEE WAS LIFTING A VIBRATOR HEAD UP TO A PLATFORM AND DROPPED IT ON HIS RIGHT INDEX FINGER TIP. REQUIRED 2-3 STITCHES AND HIS FINGER NAIL WAS REMOVED BY THE CLINIC, LIMITED MOBILITY TO HI S INDEX FINGER FOR 1-2 WEEKS.
