Employee was using a chain hoist at which time the chain hoist slipped out of the clevis causing the handle of the chain ratchet to strike employees LEFT pinky finger causing a laceration.
Employee stated was setting up the bolter to install a resin bolt close to the left rib when a portion of draw rock measuring 24"" x 21"" and approximately 4"" in thickness fell hitting EE's lower right side of EE's back.
A Roof Fall was discovered around 9:45 a.m. on No. 4 Belt, 33 crosscut in No. 4 entry. No employees were injured, no equipment involved. Ventilation controls were not affected. The fall measured 20' in width, 20' in length and approximately 10' in height. This fall will be cleaned up.

A ROOF FALL WAS DISCOVERED IN AN INTERSECTION OF #7 ENTRY AT SPAD #4147 IN THE PRIMARY ESCAPEWAY ON THE 4 BELT MAINS PANEL.THE FALL WAS APPROXIMATELY 7' IN HEIGHT, 19' IN WIDTH AND 20' IN LENGTH. ALL APPROACHES WERE DANGERED OFF,TIMBERS SET, CRIBS BUILT AND STEEL JACKS SET. NO EMPLOYEES WERE INJURED, NO EQUIPMENT INVOLVED. VENTILATION WAS NOT AFFECTED. THE LIFELINE WAS RE-ROUTED.
Employee stated employee was carrying an acetylene bottle when tripped over the corner of a top structure causing employee to fall and twist back and hip.
A roof fall was discovered in the intersection of No. 5 intake track entry at spad #3519. The fall was 20' in length, 20' in width and approximately 7' to 8' in height. No employees were injured. No equipment or ventilation controls were affected. This area was timbered off at all approaches. The lifeline was re-routed and management reviewed the changes with all employees.
A continuation fall was discovered by the fireboss. The falls encountered were connected by previous falls in the Old 3 West Mains at the following spad locations, 2059 (#1 entry), (#7 entry-2060) and (#5 entry 2043 to 2061). Timbers were set, barriers erected and danger signs hung to prevent employees from entering that area.
A roof fall has occurred at #6 break on #1 belt line (partial intersection and between block 20' in length x 19' in width and 5' in height). No employees were injured, no equipment was involved. Danger tags were hung and timbers were set at all approaches. No ventilation controls were affected by this incident. This fall will be cleaned up.
A roof fall was found near spad #2552. No employees were injured, no equipment involved, ventilation was not affected.Additional roof supports in the form of timbers were set to prevent reoccurrence. The fall was 50' in length, 10' in height, 19' in width. This fall will not be cleaned up. The lifeline and primary escapeway was re-routed.
Employee stated was walking by a rail supply car when a portion of rib rock measuring 24"" in width, 18"" in thickness and 20"" in length fell hitting employee's lower left leg around the calf area.

An Ignition of methane has occurred in the No. 5 face on the 5 left panel. A methane bleeder had been cut into by the continuous mining machine. A spark created by a miner bit against the sandstone roof caused the ignition. The flash of methane was quickly extinguished by the water sprays which were on at the time of the incident.
EE has been diagnosed with Occupational Pneumoconiosis
Employee has been diagnosed with disorders (repeated trauma) involving employee's back. The operator is reporting this instance as a code 26.
A certified electrician was load testing a 6 volt battery on a 3-wheeler with a volt meter when a cap blew off from the center cell, exposing the electrician to liquid from the battery, toward their face and eyes.
Employee has been diagnosed with Occupational Pneumoconiosis.
Several roof falls have occurred on the old worked out panel from spad #'s 6719, 6717, 6696, 6701, 6716 2nd crosscuts outby the old faces. Also at spads 6680 in #3 entry and spad #6699 in #2 entry. Supplemental support in the form of timbers and cribs were installed. All approaches have been dangered off.
The mine roof collapsed on top and outby the MRS as it was being moved. The fall measured approximately 8 feet in height, 25 feet in length and 19 feet in width. A clean up and retrieval plan was submitted to recover the MRS. No employees were involved and no ventilation controls were affected by this incident.

A roof fall was discovered in the intersection of spad #6718 on the old 10 belt panel. The fall was approximately 7 to 8' in height and the entire intersection. Timbers were set and danger signs posted all approaches. No ventilation controls were affected. No employees or equipment were involved. A map of the area was sent to the local MSHA office.
Contractor employee was pulling a rubber tire supply car with a scoop. To avoid hitting a dewatering pump ee decided to set the scoop over against the right rib. As ee proceeded to tram the scoop, the back of the scoop deck undercut the rib causing some rib rock 30"" x 14"" and approximately 5"" in thickness to dislodge striking ee on the back of ee's hard hat and shoulders.
2 roof falls discovered. 1st fall near spad 6577. 2nd fall near spads 6553, 6560 and toward spad 6573. No equipment or employees were involved. No ventilation controls affected. These areas have been timbered off and danger tags hung. Each fall was 19.5 feet in width and approximately 14 feet in height. A map of the falls were sent to the district office.
2 roof falls were discovered by the weekly examiner between spad locations 5918 and 5916, also from spad 6254 to 6256 on the old No. 10 belt panel. All approaches to the falls have been timbered off and danger signs hung. No employees or equipment were involved. No ventilation controls affected.
Employee stated EE was installing a 6' resin bolt when a section of draw rock fell from the mine roof striking the tip end of EE's left hand ring finger. The rock measured 18"" x 18"" and approximately 3 inches in thickness.
Employee has been diagnosed with Occupational Pneumoconiosis.
A roof fall was discovered near spad 600 and measured 35' in length 18.5' in width and approximately 7' in height. The fall was between #5 and 6 entry and did not fall into the adjacent intersection. No ventilation controls or escapeways were affected. No employees or equipment were involved. The area has been timbered off and all approaches flagged.
EE was helping 2 other employees lift a shuttle car speed reducer on to a 4-wheeler when EE felt something pop in right knee. EE works a 4 on 4 off schedule and was scheduled to return to work on 6/26/2017 in which EE's doctor stated was unable to return to work.

A roof fall was discovered by the fireboss in the right return of B-Panel of the old southwest mains. The length of the fall was approximately 110' by 18'5"" in width and 8' in height. No ventilation controls were affected, no employees or equipment were involved. This area has been dangered off and timbers set to prevent access.
A roof fall was discovered by weekly examiner along the old No. 3 belt panel between spads 1034 & 1043. No equip or ee's were involved. No ventilation controls were affected. Area was mined in July 2010. 6' torque tension bolts were used. The fall measured 7' in height, 18.5' in width & 45' in length. Timbers were set at approaches & dandered off. Fall will not be cleaned up.

A roof fall occurred in No. 1 entry outby the face area. The fall measured approximately 18' wide, 8' in height and 60' in length. No ventilation controls were affected, no employees or equipment were involved. Cribs and timbers were set and the affected areas of approach were dangered off. There are no plans at this time to clean up the fall.
Employee stated was removing a rock off of the left side continuous miner measuring 12"" in length 13"" in width and 10"" in thickness and as EE was going to roll it into the conveyor chain the rock shifted and rolled back onto left hand.
IE was operating Roof Bolter in 5 Right location in face. In Process of installing second row of bolts. When piece of draw rock fell from the roof between ATRS and drill canopy. The rock struck employee's hard hat and cut employee between hard hat and safety glasses. Rock measured 21 inches wide x 19 inches long x 6 inches thick.

An unplanned fall was reported on 12/18/2015 near spad No.4796. A continuation of this fall was discovered by the weekly examiner and a MSHA inspector traveling with him. The fall is 7' in height, 19' in width and approximately 120' in distance. No employees were injured, no equipment involved and ventilation was not affected. Cribs, timbers were set and the area dangered off.
An unplanned roof fall has occurred on the No. 1 Old North panel in No. 4 entry at spad location 1164 to 1172. This fall is approximately 60 feet long and 7 feet in height and 18 feet in width. 6 foot torque tension bolts were used when this area of the mine was developed in September 2010. No employees were injured or equipment was involved. This fall does not effect ventilation.
Employee was lapping up approximately 50 feet of line curtain when they felt a sharp pain in their lower back. The curtain was covered with mud and water at the time of the incident.
EE (contractor) was assisting 3 co-workers to hang a section of belt and structure to a belt chain. As ee was lifting ee felt a sharp pain and something pop in lower back.
Employee has been diagnosed with Occupational Hearing Loss. Brooks Run South safety department was notified April 15th, 2016 of the diagnosis. Employee retired on 12-18-15.
EE was moving the miner cable out of the roadway when the bolt machine was backing up the bolter cable caught the employee's left leg and caused it to bend backwards injuring EE's left knee. This employee did not seek medical attention at the time of the incident. EE went to the doctor on 3/14/2016 in which it was determined EE could not return to work pending on an MRI.
The injured employee was being assisted by a co-worker loading top structure into a scoop bucket and as EE turned to load the last piece of structure EE felt some pain in upper back between shoulder blades.
A roof fall was discovered by our weekly examiner in the intersection of spad #4091 in the 2 Left B Panel near MP 15. Ventilation controls were not affected. No employees or equipment were involved. The fall was approximately 7' in height, 25' in length and 20' in width. This area has been dangered off, props set and cribs built. This area will not be cleaned up.
Contractor employee was installing resin bolts in the face of No. 8 entry. Employee was pushing the bolt over with the palm of employee's right hand and as employee was aligning the pin wrench to finish installing the bolt employee engaged the rotation causing the bolt to spring striking employee's right hand.

Roof fall was discovered in the inactive working of the old 3 main panel at spad location 1934 extending over toward 1940 between No. 1 and 2 entry. Fall was approximately 40' in length, 20' in width and 6' 1/2 in height. Area was developed 11/15/2011. Cribs set and timbers. Area dangered off. Ventilation controls were not affected. No injuries, no equipment involved.
Employee was positioned underneath the offside of the Fairchild scoop when it inadvertently moved causing an injury to the employees left ankle and shoulder.
Employee was positioned next to the center section of the scoop near the operators compartment when the scoop inadvertently moved causing an injury to his right lower leg.
Employee stated he was unlapping belt for a section move when he felt some pain in his right knee. Employee continued to work and sought medical attention on 6/19/2015 where an x-ray was negative. He was referred to a specialist and an MRI results were discussed with the employee on 7/23/2015.
An unplanned roof fall in an inactive area has occurred in the Wyoming No. 2 Mine inby spad 3673 extending to spad 3781 between the 2 blocks. The fall is approximately 70 feet long by 19 feet wide and 7 1/2 in height. This area of the mine was developed approximately 11/201/2013. No injuries, no equipment involved. No ventilation controls were effected.
Employee was operating a roof ranger 2 bolter in the 2 left crosscut. While he was installing the outside bolt of the second row on the operator side a piece of draw rock measuring 24"" long 17"" wide 1"" 1/2 in thickness came loose from the roof and slid down a pie pan striking the employee's left hand and fingers between the rock and the side of the bolter.
Employee was operating a continuous miner in the No.1 entry face during a clean-up of the left side of the entry when a piece of draw rock measuring 67"" long 27"" wide and 0 to 6"" thick fell from between the bolts and the right rib line striking him on the back side of his right ankle. Employee was taken to the hospital where X-Rays revealed a broken bone.

Spad location 1660 and extends to spad 1653. Does not affect ventilation. Cribs were built and the approaches to the fall were dangered off.

Employee was on his knees at his tool box searching for some fittings for a grease hose. When he started to get up to return to the section, he felt something cut his knee. He took his knee pad off and saw that he had been cut, but he did not look for what had cut his knee. He returned to the section for assistance.
Employer was notified by the compensation provider on 2/24/15 that the employee had a diagnosis of pneumoconiosis from an examination on 11/26/14. His employment terminated at this mine on 12/11/14.
Employee set steel against the top and was moving hand away to start the rotation. A rock that measured 2.5' long X 6"" thick X 1"" wide fell catching hand between rock and the drill pot.
An unplanned roof fall occurred in Wyoming #2 mine near spad #625, that measured approximately 15' long x 10' wide x 4'6' high. No injuries were reported. The area was developed on 2/10/2010.
EE was knelt down behind the miner picking up pull ropes that had fell off. The rib rolled out and struck him on the back of the head and his back. The rock measured approximately 80"" long X 17"" wide X 11"" thick. He was transported to Raleigh General Hospital where x-rays were negative. He was released to return to work. Due to continuing pain unable to RTW on 10/8/14 & 10/9/14
An unplanned roof fall occurred in the Wyoming #2 mine near spad #127090 in the #4 entry of the Old 2 right panel. The fall measured approximately 20' long X 20' wide X 6-8 high. No injuries were reported. the area was developed on 3-4-14.
EE was assisting 3 others remove a shaft from a Mac 12 mantrip. They had one side of the shaft suspended in the air approx 26 off the mantrip with a pair of come-alongs. The IE grabbed the bearing end of the shaft to lift it over the rail of mantrip. Another EE continued to crank the come-along causing the shaft to shift, catching his middle/index fingers between trip and shaft.
Employee was rolling a rock from in front of the high voltage sled to rib. He was approximately 2' from the rib when it rolled out and onto his right leg. He suffered a fracture to the medial malleolus, which is the small upper bone of the inside ankle. he was place on restricted duty for 6-8 weeks.
An unplanned roof fall occurred in the Wyoming #2 mine in the #5 entry, approximately 40' outby spad #4009 that measured approximately 50' long X 19' wide X 6.5' high. No injuries were reported. The area was developed on 6/2/14.
Employee received a pneumoconiosis diagnosis on 9/4/13. His employment was terminated at the Brooks Run Mining Co. Wyoming No. 2 Mine on 5/20/10 and he worked at another mining company since then. However, the responsibility for his claim was changed to Brooks Run Mining on 4/3/14. The company was notified of the 9/4/13 diagnosis on 5/12/14.
An unplanned roof fall occurred in the Wyoming #2 mine, 1 brk. inby spad #870 in the #6 entry, that measured approximately 20' wide X 70' long at or above anchorage. No injuries were reported. The area was developed on 5/13/10.
EE was carrying the top part of an overcast when he slipped on loose rock and fell causing a fracture to the left hand. He was placed on restricted duty as of 1/19/14 and has a follow-up with Ortho. 1/30/14. EE is employed by MC Ent. Brroks Run os providing this report in accordance to MSHA PPL P-11-V-05
Unplanned fire occurred at the Wyoming #2 mine in one of the office trailers. The fire was extinguished by the dispatcher within about 20 minutes of discovery. Brenton & Pineville fire dept were also dispatched to the seen. It is believed the fire started in the heater of the maintenance chiefs office. The trailer has been removed from property. no injuries were reported.

The employee had been provided a Part 90 letter in 1983 while with another employer. He was hired at this company on 1/15/07. He recently produced his Part 90 letter and completed his option to exercise form on 12/16/13.
An unplanned roof fall occurred in the Wyoming #2 mine at spad #8915 that measured 20' long x 20' wide x 6 to 7' in height. No injuries were reported. The area was developed on 1/16/2011.
Employee was pulling cable when he slipped in a mud hole and began to fall. He caught himself with his left hand and felt a burning sensation in his left shoulder. He has been placed on restricted duty as of 11/7/13 pending the results of an MRI.
While turning a curve around #6 head, the mantrip was pulled toward the rib due to the front tire being caught in a rut. The mantrip struck a timber causing it to become dislodged. It fell and struck employee in the left ear, causing a laceration to the ear and a bruise to the head just behind the left ear.

Measurements were unobtainable due to this crosscut previously being cribbed off on both sides for bad roof conditions. No injuries were reported. The area was developed on 8/10.
EE was lifting a piece of belt structure and felt pain in the left bicep. EE was diagnosed with a torn bicep ligament.
Employee was striking a knocker with a hammer while making a splice. A piece of metal broke off the knocker and embedded in the employees left thumb.

While bolting top, he put his (L) hand on pot of bolting machine and crushed it with drill pot and top.
Employee was changing seats in the shuttle car after his car had been loaded.The continuous miner was backed up striking the car and scooting it into the employees feet. His feet were caught between the shuttle car frame and rib.
A roof fall was discovered in No. 2 entry of the left return aircourse near spad 626. The fall measured approximately 20'wide by 80'long x 7' thick. There were no injuries and no damage to equipment.

Employee had surgery to repair a hernia on 6/18/10. This condition is related to lifting and pulling in low coal. He sought medical attention after moving a piece of coal in the face and experiencing pain.
Employee was operating a small battery-powered transportation vehicle when it ran over a rut or obstruction in the roadway causing the steering wheel to slip from his grip and spin. The steering knob struck him on the hand.
EE was plastering stopping and did not use gloves and burned his right wrist.
A piece of rock fell on EE and pined him to side of bolter.

The continuous miner operator reported that he saw a flash near the left side of the ripperhead while cutting in bottom at the face. There were no injuries and no damage to equipment.
The continuous miner operator alleged that he thought he saw a flash while cutting down coal between shuttle cars in the 8 right crosscut. There were no injuries and there was no damage to equipment. The immediate investigation after this allged event failed to uncover any direct evidence that an ignition of gas or dust occurred at all.
While drilling a roof bolt hole, a piece of rock fell from the roof striking the drill head and hitting the employee on top of the hard hat.
While bolting top, a piece of roof fell on his ankle. #3 left break, spad #341.
Employee was adding pusher steel. After coupling the two pieces of steel together, he started the rotation. He had rested his left hand on the drill chuck and his glove caught on a burr. This wrenched his hand resulting in injury to his little finger.
Employee was operating the roof bolter. He got up from his knees and was moving towards the back of the machine when he hit his right knee on the frame of the machine. This caused pain in his knee and left hip.(As this investigation proceeded, it was determined that the injured was moving toward the back of the machine due to the continuous miner thru unexpectedly-MM 3/16/09 #1)
The employee was waiting on the scoop operator to clean the No. 2 face. The scoop ran over an aerosol paint can causing a popping noise. The employee twisted and felt discomfort in his groin area.
