NO INJUYR OR DAMGE TO REPORT. LENGTH OF BOTLS 4'-5',6',00 SIZE FACE BOTLED WTIH 4" 75 GRADE CONVETNIONAL BOTLS ON 2'-3' CENTER. ADDITIONAL SUPPORT WAS CROSS STRAPED AND TIMBERED ALSO SPOT PI NNED SLICK TOP PRIOR TO FALL 2/ 1 SURFACE CRACK IN THE 24 SURVEY AREA. A SLICK ABOVE ANCHOR ZONE MAY HAVE SLIPPEC CREATING PRESSURE TO FALL.

INJURED WAS GETTING OUT OF END LOADER. WHEN HE STEPPED DOWN OFF STEP. HE WAS WEARING RUBBER BOOTS. WHEN HIS FOOT SLIPPED AND TWISTED HIS LEFT KNEE. NO SIMPTONS ACCURED AT THIS, BUT LATER D ID OCCUR ON 9-30-93. AT THE TIMEHE STEPPED OFF LOADER NO PROBLEMS SHOWED UP.
