EMPLOYEE WAS CHANGING THE CUTTING EDGE OF A FRONTEND LOADER BUCKET. USING AN IMPACT WRENCH WITH LEFT HAND TO TIGHTEN THE NUT. PUSHED DOWN ON WRENCH, FORCED HEAD OF BOLT OUT OF HOLE AND THE HEAD OF THE BOLT SPUN AND CUT EMPLOYEES RIGHT INDEX FINGER.

REMOVING CUTTING EDGE OF LOADER BUCKET, EMPLOYEE USED HIS HAND TO APPLY UPWARD PRESSURE TO BOLT.THE BOLT SPUN QUICKLY, CUTTING HIS MIDDLE FINGER. EMPLOYEE RECIEVED 8 SUTURES ON HIS MIDDLE FING ER, LEFT HAND.
