EMPLOYEE STATED EE WAS EXITING THE CONNEX BOX STORAGE TRAILER WHEN EE SLIPPED ON SNOW/ICE CAUSING EE TO CATCH SELF IN A SPLIT.

EMPLOYEE REPORTED HE WAS WORKING ON THE CRUSHER WHEN HE STRUCK HIS BACK ON PART OF THE CRUSHER. HE ALSO REPORTED BENDING AND LIFTING CONTRIBUTED TO THE INJURY. HE HAS A BACK STRAIN.
EMPLOYEE STATED HE WAS WORKING ON CRUSHER JAW WHEN HE CUT HIS RIGHT INDEX FINGER.
Employee stated he was removing a bolt from the cone, located on the ground, when the bolt broke off causing him to step backward rolling his ankle.

THE EMPLOYEE REPORTED HE IS NOT SURE HOW HE INJURED HIS SHOULDER, BUT CAN FEEL A SNAP ON OCCASION AND IT PAINFUL WHEN IT HAPPENS.
Employee reported he was exiting the loader when he lost his grip on the right side of the ladder. He swung around backwards holding on with his left hand. His lower back hit the frame of the loader. Injury is a lumbar strain.

EE stated he was shoveling dirt under the conveyor belt,twisted and felt a pop in his shoulder.
Employee dismounted ladder, stepped on rock, twisted ankle.
