Fall to the walkway or working surface

The employee was walking when he slipped causing him to fall. The employee caught himself with his right elbow causing a fracture.

Over-exertion (Not Elsewhere Classified)

On 2/18/14, a temporary contract employee complained of shoulder pain. He could not specify a particular date or time the pain began, but said he thought it was related to his activities at work. The temporary employment agency was contacted and the employee was removed from the worksite.

Fall to the walkway or working surface

The employee slipped and fell causing a contusion to his leg and his right elbow.

Fall onto or against objects

The employee was exiting the loader when his right foot slipped off the top step. When his right foot slipped off the step his right knee struck the top step.

Over-exertion in wielding or throwing objects

To move product to the poly bagger, the operators have to occasionally hit the sides of the bin with a dead blow hammer. While the injured was striking the bin, he said he heard something pop in his shoulder. He was still able to move it freely and refused on-site first aid. He was later diagnosed with a shoulder strain, and his lifting ability was restricted.
