A ROOF FALL OCCURRED IN THE INTAKE AIR COURSE IN #1 ENTRY TWO BREAKS OUTBY SPAD 869. THE FALL MEASURED 20' WIDE, 20' LONG AND 4' THICK.
A ROOF FALL OCCURRED OUTBY SPAD 6482, IT WAS 6 TO 8 FEET THICK, 180 FEET LONG AND 18 FEET WIDE.
EE WAS HANGING CONTINUOUS MINER CABLE AND TWISTED HIS KNEE. HE STARTED MISSING WORK 4/13/04.
HE WAS PULLING ON THE SHUTTLE CAR CABLE, WHEN HIS BACK WENT OUT.
PRYING ROCK OUT OF FLUME WITH BAR, HE TWISTED HIS BACK. HE DID NOT START MISSING WORK UNTIL 2/7.
Employee was guiding a cutting edge that was being moved with a magnet when the magnet unexpectedly released the cutting edge and caught his left thumb. This caused a laceration that required stitches.
DRIVING A TRACK PIN INTO THE MINER WITH A SLEDGE HAMMER, A CHIP FLEW OFF STRIKING HIS RIGHT LEG. HE HAS NOT MISSED ANY WORK BUT HAD TO HAVE STITCHES.
