Fall to the walkway or working surface

Employee was moving a heater in the basement area of the plant. He lost his footing and fell on his left side straining his back. The area was dry and the heater had wheels for ease of movement. Employee worked until 12/5/14 at which time he became a Lost Time Accident.

Employee was replacing a return roller when chain hook came loose and struck the top of his head requiring staples to close the wound.

Employee was installing new tires when the tire bar slipped and struck him on the right knee. Employee's first day missed was on 1/19/09.

Employee tightened new traction tires with ratchet on 1/15/09 and he states that his back started to bother him on the evening of 1/16/09. Employee reported this to the employer on 1/19/09 and sought treatment on that date. His first missed day of work was 1/19/09.

Caught in, under or between a moving and a stationary object

Employee was working to insert auger screw back into barrel using a backhoe to insert screw when the key steel that lines the screw with the planetary gear started to come out. He used his finger to push the key steel back in and it got pinned between the collar of the screw and the top of the mast requiring a surgical procedure.

Was throwing a 4x6 rubber screen with metal edges out of a ras coal screen and when he flipped it over, it caught his finger requiring stitches.

Employee was helping install a 10"" piece of pipe. The pipe slipped and when he reached for it he felt a pulling sensation in his left arm. He worked until 7/7/08. At this time he sought medical attention believing it to be a re-occurrence of a previous injury to the same arm. He was sent to specialists by workman's comp. and it was determined to be a new injury.

EE stated that he felt a sharp pain in his groin when lifting a sample and twisting to load into a truck. He did not report the inury until 1/8/08. The company and the workers compensation carrier investigated and determined that the injury was compensible on 2/27/08.

Employee was carrying 25 lb rock when he slipped and rock fell on hand injuring his right middle finger.

Over-exertion in lifting objects

Employee was lifting pump out of sump hole to reposition, pump slipped off of chain hook causing him to pitch forward, he used his left arm to catch himself straining his left arm, his neck and his back. He is currently off work effective 8/27/07.

Miner was lifting starter in place for 773D Cat Hauler when he felt pain in groin area. Injury required surgery for hernia and he did not miss work until 3/16/07.

Miner was walking to silo feeder to clean up spillage, tripped and got arm caught in top roller. It pulled his arm in and slammed his chest into an angle brace injuring his left arm, left elbow, left chest and left thumb. Employee was not taken off work by his physician until 12/12/06.

Strained back and neck assisting fellow employee lift rock out of Stamler. He did not miss any work until 11/20/06.

He was using a hammer to knock a bolt out and a very small piece of the hammer came off and lodged in his hand. Had to have surgery later on to have the small piece of metal removed.

Control panel door came down and struck him on the head, when the door latch did not hold the door open.

EMPLOYEE WAS USING A SLEDGE HAMMER TO DRIVE A SPROCKET UP ON THE SHAFT OF THE SPEED REDUCER OF THE FEEDER, HE HIT IT SEVERAL TIMES, ABOUT THE 5TH TIME A PIECE OF METAL CAME OFF THE SPROCKET STRIKING EE IN THE CHEST, HE WAS OVER 20'AWAY FROM THE SPROCKET.
