Over-exertion (Not Elsewhere Classified)

EE stepped out of pickup onto uneven ground, twisting ankle.

Fall from piled material

Employee was working on top of a bench placing a marker stake near a highwall when the ground under him collapsed, dropping him to a muck pile.

Climbing down off of catwalk employee's leg hung up causing strain/sprain to knee.

While employee was operating shatter box, he was distracted and inadvertently placed his hand in a pinch point, resulting in a laceration with finger tip fracture. Employee had not followed procedure and did not close the lid to the box while it was in operation.

EE had finger pinched between hydraulic ram and stairway tread while moving ram down stairway resulting in sutures.

Struck against stationary object

While handling a scrap piece of wire rope, strand punctured leather glove and finger of EE causing a puncture/tear that required sutures.

While installing a nut on the end of a hydraulic ram, EE cut the tip of his finger on the frame resulting in 6 sutures. Machine was locked and tagged out. EE was wearing leather gloves.

A Welder complained of eye irritation on 8/29/13. An eye doctor found a small piece of debris in the eye and prescribed antibiotic drops. It is believed the debris entered the eye sometime the previous shift. The cause is not known as the Welder wore all required PPE during welding/grinding activities.

Fall onto or against objects

A mechanic was carrying two breathers in his hand when he slipped and contacted his elbow. The contact resulted in a two inch laceration on his elbow that required stitches.

Employee turned on the wash down water hose before he could pick up the hose it started spraying and the end of the hose struck him in the safety glasses

Employee struck head on cyclinder head requiring four staples behind the ear.
