Flash burns (welding)

Grinding on welds, standing next to welder, his eyes were burned due to exposure to ultraviolet rays.

Struck against a moving object

Driving haul truck uphill, truck stalled and rolled backwards up onto berm. Rear wheels dropped into berm. Facial lacerations, hairline hip fracture.

Welding metal in a chute with torch, spark went in his glove and burned his left hand.

Employee was working in steel plate. flipped the plate over, fell on his right hand, pinching finger between steel plate-bench.
