Struck by powered moving object

Truck Driver in TDW060 pulled over and stopped at a curve in the road, partially off the road, which created a blind spot. Loader Operator in 3LV025 was driving in the same direction of the truck. As the Loader Operator passed the truck the Truck Driver was struck and killed.

Caught in, under or between a moving and a stationary object

While disconnecting a 6"" suction hose from a water pump, employees left hand got caught between the hose and a rock.

Fall to the walkway or working surface

Employee was pulling a 6""x10'L suction hose and tripped over a rock. As employee was falling EE put EE's left hand out to catch self. Employees left hand hit the ground.
