AN UNPLANNED ROOF FALL OCCURRED 20' OUTBY SPAD #1750 IN THE RETURN AIR COURSE. IT MEASURED 20' X50' X 5'-6'. NO RIDER SEAM PRESENT. FALL 4000' FROM ACTIVE FACE. VENT. & PASSAGES NOT AFFECTED.
EE WAS INSTALLING 6"" WATERLINE AND TRIED TO BUMP A PIECE OF LINE WITH ANOTHER AND HIS FINGER WAS CAUGHT BETWEEN THE TWO JOINTS OF WATERLINE.
EE ATTEMPTED TO PULL IN SHIELD #173. WHEN IT DID NOT COME IN ALL THE WAY HE KNELT DOWN WITH LEFT KNEE ON PONTOON OF SHIELD #174 & RIGHT FOOT BETWEEN #174 & TAIL DRIVE. HE ADVANCED #173 & IT DR AGGED ALONG THE ROOF PULLING THE TAIL DRIVE BACK ABOUT 4"" PINNING HIS RIGHT FOOT BETWEEN #174 & TAIL DRIVE. HE WAS TAKEN TO E.R. WERE X-RAYS WERE NEGATIVE & DIAGNOSED WITH DEEP CONTUSIONS.
EE AND CO-WORKER HAD RELEASED THE 48"" BELT CLAMP. AFTER COMPLETING THE BELT SPLICE FOR THE BELT MOVE. AS EE WAS REMOVING THE BELT CLAMPS FROM THE WALKWAY SIDE OF THE BELT HE LOST HIS BALANCE. HE STUMBLED BACKWARDS AND TWISTED HIS RIGHT KNEE.
EE WAS PULLING DRAW ROCK IN LAST OPEN X-CUT OF LONGWALL FACE W/ 5' SLATE BAR, WHEN A PIECE OF DRAW ROCK APPROX. 2' X 1' X 1""-2"" STRUCK HIM ON SIDE OF THE HEAD. EE CONTINUED TO WORK UNTIL TUESD AY, OCTOBER 28, 2003 WHEN HE WENT TO THOMAS MEMORIAL EMERGENCY ROOM & WAS DIAGNOSED WITH A CERVICAL SPRAIN. EE STARTED MISSING WORK ON 10-28-03, THAT IS WHY REPORT IS LATE.
EE WAS ASSISTING THE LEFT SIDE CM OPERATOR WITH HIS CABLE & WATER LINE WHEN THE EDGE OF A ROCK RIB MEASURING APPROX 7'X 8' X 2 1/2' FELL STRIKING HIS RIGHT FOOT. EE WAS TRANSPORTED TO LOGAN RE GIONAL MEDICAL CENTER WHERE X-RAYS REVEALED FRACTURES TO THE THREE MIDDLE TOES. RECOVERY TIME UNKNOWN.
TWO EMPLOYEES WERE MOVING A FLETCHER ROOFBOLTER THROUGH A SET OF 5' DOUBLE DOORS WHEN THE HEAD OF THE BOLTER CAUGHT ONE OF THE DOORS. THEY WERE TRYING TO FREE THE DOOR WHEN VICTIM FELT A SHARP PAIN IN HIS LOWER BACK. HE FINISHED THE SHIFT & CONTINUED TO WORK UNTIL 6/24/03 WHEN HE SAW A DOCTOR & WAS DIAGNOSED WITH A MUSCLE STRAIN IN THE BACK.
EE AND COWORKERS WERE MOVING A RR II THAT HAD A BROKEN OPERATOR FRONT SIDE TRAM CHAIN 1 BREAK OUT BY THE LOB. THEY HOOKED A SCOOP UP TO THE BACK OF THE RR II CHAIN TO MOVE. EE WAS STANDING ON THE INSIDE OF THE RIB WATCHING CABLE WHEN THE ATRS STRUCK HIS RIGHT KNEE. HE STATED HE FLAGGED THE OPERATORS, BUT NEITHER SEEN HIM.
EE WAS LOADING TOP ROLLER IN A SCOOP BUCKET. THE RAM WAS PUSHED 6"" OUT AND EE STEPPED IN BEHIND THE RAM WITH HIS RIGHT FOOT. HIS LEG BECAME TRAPED OR WEDGE BEHIND THE RAM AND HIS BODY FEEL BAC KWARDS OFF THE SCOOP BUCKET.
EMPLOYEE WAS RIDING A MATRIP TRAVELING OUTBY ON THE #5HG. A MOTOR LEFT THE BOXCUT CALLING FOR THE ROAD AT DESIGNATED LOCATIONS HEADING TOWARDS #5HG. THE MOTOR RECEIVED THE ROAD TO TRAVEL INBY ON #5HG WHEN IT STRUCK THE MANTRIP TRAVELING OUTBY.
EMPLOYEE WAS OPERATING A 10 SC SHUTTLE CAR WHEN HIS ARM WAS CAUGHT BETWEEN THE COAL RIB AND THE SHUTTLE CAR. HE WAS TREATED BY EMT AND TRANSPORTED TO LOGAN GENERAL. HE WAS DIAGNOSED WITH A DIS LOCATED LEFT ELBOW AND TISSUE DAMAGE. RECOVERY TIME IS UNKNOWN.
EE & COWORKERS WERE PUTTING A WRECKED SHIELD CAR & LONGWALLS SHIELD BACK ON THE TRACK WITH AIR BAGS. AS THEY WERE INFLATING THE AIRBAGS WITH THE CRIBS UNDER THE CAR THE SHIELD CAR SHIFTED CAUSING THE AIRBAGS TO SHIFT & A CRIB BLOCK SHOTFROM UNDER THE FRONT OF THE CAR. EE WAS KNEELINGAPPROX 5' AWAY & WAS STRUCK IN THE FACE. REC'D SUTURES TO THE CUT & SURGERY TO THE FRACTURE OF
MEMBER WAS RIDING ON THE INBY END OF THE MANTRIP WHILE COMING OFF THE SECTION. THE BUS PROCEEDED THROUGH THE FIRST SET OF AIRLOCK DOORS. THEY THEN OPENED THE NEXT SET OF DOORS & PROCEEDED THRO UGH. WHILE THE MANTRIP WAS GOING THROUGH, THE DOORS GRADUALLY CLOSED & CAUGHT THE INBY END CANOPY POST CAUSING THE DOOR TO BREAK DOWN. THE CANOPY LANDED ON INJURED'S RIGHT ARM AND SHOULDER.
EMPLOYEE WAS RIDING A MANTRIP TRAVELING OUTBY ON THE #5HG. A MOTOR LEFT THE BOXCUT CALLING FOR THE ROAD AT DESIGNATED LOCATIONS HEADING TOWARDS #5HG. THE MOTOR RECEIVED THE ROAD TO TRAVEL INBY ON #5HG WHEN IT STRUCK THE MANTRIP TRAVELING OUTBY.
EE ALLEDGES THAT HE STRAINED HIS BACK LOADING ROOF BOLTS FROM A SCOOP TO THE BOLTER. NOTE: THIS ACCIDENT IS UNDER PROTEST AND THIS REPORT IS FOR COMPLIANCE REASONS ONLY. THIS WAS EMPLOYEE'S LA ST SHIFT.
PICKING UP METAL TO REPAIR A LOADER JUST RECEIVED REPORT ON 7-8-03
EE WAS OPERATING THE LW SHEARER FROM THE TAIL TO HEAD WHEN THE DRUM THREW OUT A SMALL PIECE OF SANDSTONE ROCK STRIKING HIM ON THE UPPER LIP. EE FINISHED THE SHIFT & THEN WENT TO LOGAN REGIONAL HOSPITAL WHERE 3 SUTURES WERE APPLIED TO THE LACERATION.
EE WAS INSTALLING A DRAW HEAD ONTO A FLAT CAR WHEN THE ACCIDENT OCCURRED. AS HE WAS INSTALLING THE PIN, BETWEEN THE DRAWHEAD & FLATCAR, THE PIN SMASHED HIS LEFT MIDDLE FINGER. EE WENT TO THE HOSPITAL WHERE HE RECEIVED STITCHES.
EMPLOYEE WAS SETTING A HEINTZ MAN JACK. THE JACKFELL STRIKING THE TOP OF HIS LEFT HAND. HE RECEIVED SUTURES AT LOGAN GENERAL.
EE WAS BLOCKING UP THE PAN OF A MINER WHILE LIFTING IT WITH A SCOOP. THE PAN SLIPPED OFF THE SCOOP BLADE AND LANDED ON THE BLOCKING. A HALF-HEADER FLEW UP AND HIT EE IN THE MOUTH ON IMPACT. EE FINISHED HIS SHIFT & DROVE HIMSELF TO THE HOSPITAL & RECEIVED 8 STITCHES IN HIS UPPER LIP.
EE WAS AWARDED 5% FOR OCCUPATIONAL PNEUMOCONIOSIS BY THE WV WORKERS COMPENSATION DIVISION, OFFICE OF OCCUPATIONAL PNEUMOCONIOSIS.
EE WAS AWARDED 5% OCCUPATIONAL PNEUMOCONIOSIS BY WEST VIRGINIA WORKERS COMPENSATION. EE DID NOT HAVE ANY TIME AWAYFROM WORK. THISIS CLASSIFIED AS OCCUPATION ILLNESS.
