A ROOF FALL OCCURRED ON THE INTAKE ENTRY 1 BK OUTBY SPAD #1141. IT MEASURED 70' LONG 15' WIDE AND 4' HIGH. THIS MINE IS IN THE PROCESS OF BEING REOPENED.

A ROOF FALL OCCURED ON THE #9 HEADGATE PANEL INBY SPAD 3003 IN THE #2 ENTRY THE FULL MEASURED 20' WIDE AND 6' HIGH.

AN UNPLANNED ROOF FALL OCCURRED IN THE #4 ENTRY AT BRK 12 & 14. 4'-10' HIGH, 20' WIDE, 20' LONG. LOCATION SPAD #1597.

AN UNPLANNED WATER INUNDATION WAS ENCOUNTERED AT THE #2 TRACK ENTRY AT X-CUT 17 BY THE #3 SECTION MIDNIGHT CREW. THE WATER WAS APPROX. 36"" DEEP IN #2, 3 & 4 ENTRIES.

AN UNPLANNED ROOF FALL OCCURRED AT SPAD #2333 IN THE #3 ENTRY. THE FALL WAS APPROX. 15'H 60'LONG 19'WIDE. THE AREA WILL NOT BE REHABILITATED.

Struck by falling object

Pushing line onto as it was feeding out of equipment. rock struck him on ankle.

Fall to the walkway or working surface

EE WAS WALKING UP THE HILL ON THE #1 MAINS BELTING AT BREAK #18 WHEN HE SLIPPED & FELL. EE CONTINUED TO WORK UNTIL THE PAIN IN HIS LOWER BACK WORSENED. HE SAW SEVERAL SPECIALISTS & WAS DIAGNOSED WITH A HERNIATED L4 & L5. RECOVERY TIME IS UNKNOWN. NOTE: HIS FIRST DAY MISSED WAS 8-10-04.

A shield operator pushed the wrong direction on the shield pulling the shield on the shearer operator pinching his right foot between the pontoon and relay bar, the shearer was being operated by embilical cord.

EE was raising his clothes basket, which is raised and lowered by chain, in the bathouse when the chain hit him in the mouth causing cornea damage.

Fire bossing belts, while walking along belt line, he hit his right ear on a roof bolt.

EE WAS ATTEMPTING TO INSTALL A ROOF BOLT WHEN A PIECE OF ROCK 12"" X 6"" X 1"" THICK FELL OUT OF THE ROOF STRIKING HIM IN HIS FOREHEAD. HE RECEIVED 5 STITCHES.

Struck by falling object

EMPLOYEE WAS TRAMMING HIS MINER OUT OF THE #2 HEADING WHEN A PIECE OF ROCK 3' X 3' X 4"" THICK FELL OUT BETWEEN BOLTS STRIKING HIM IN HIS BACK.

Struck against stationary object

A PIECE OF ROCK HAD FALLEN ON THE WATER LINE AND BUSTED THE COUPLING. EE WAS TRYING TO FLIP ROCK OFF OF WATER LINE. THE ROCK HAD A SHARP EDGE THAT CUT EE'S MIDDLE AND RING FINGERS. THIS REQUIRED STITCHES. NO LOST TIME.

Flash burns (welding)

Burned both eyes, ee went to CMAC emergency room.

Struck by falling object

EE WAS PLACING A WRECKED CAR ON TRACK. WHEN A PIECE OF DRAW ROCK 7"" LONG 5"" WIDE 2"" THICK HIT HIM IN THE BACK OF THE HEAD. THIS CUT REQUIRED STITCHES. (NO LOST TIME.)

EE WAS GRINDING ON EQUIPMENT. HE REPOSITIONED HIMSELF AND HIS KNEE CAME INTO CONTACT WITH THE GRINDING WHEEL.

Fall onto or against objects

EE HAD FINISHED BOLTING A VALVE CHEST ON A LOCOMOTIVE IN THE SHOP AREA. HE WAS WALKING BY THE LOCOMOTIVE AND TRIPPED, FALLING INTO AN OXYGEN CYLINDER HOLDER IN THE SHOP AREA. HE WAS TAKEN TO L OGAN REGIONAL MEDICAL CENTER WHERE SUTURES WERE APPLIED TO THE LEFT PALM. NO DAYS AWAY FROM WORK.
