Employee was cutting the strut on miner with a torch - Flame caught pan jack hose on fire, burning his nose and right ear - Drove himself to hospital

Welding on a Joy 14-9CM when a piece of hot material went into boot causing a burn on the inside of the left calf area causing a burn. Was not reported until 8/18/13.

Struck against a moving object

EE was running #3 shuttle car ran over ledge in bottom turning right handed toward feeder. His head jerked back and rammed head into canopy. Has pain in neck and down right side of body.

Over-exertion in pulling or pushing objects

Employee & co-worker were pulling the cable of a s-car when employee felt a tightening in his back. He finished the shift and didn't stop work until 3-18-13.

Over-exertion in pulling or pushing objects

EE was pulling on miner cable and strained/pulled muscle in his back. Did not go to the hospital until 1/14/2013

Caught in, under or between a moving and a stationary object

Miner operator sitting inby red zone on #4 CM when miner swung toward him pinching him between machine & rib.

Fall onto or against objects

Traveling the face area, ice between 3 & 4 entry slight grade covered with ice. Employee went through fly pad slipped on ice landing on his back on his SCSR.

Struck by falling object

A piece of draw rock fell out of the top and struck operator on top of the left hand
