Struck by falling object

Employee was pulling a rib in the number 5 to 6 crosscut (near spad 6756). Tripped/slipped while pulling rib and pulled rib on him.

EE was at section feeder running rock through feeder when he attempted to hit steel bar to activate e-stop. The steel bar slid out by and did not hit e-stop. EE hit e-stop cable which caused a rock to flip over and pinned his hand and cut it.

Fall from ladders

Hanging face curtain in #2 entry on 4ft ladder. Ladder leg buckled and employee fell from the 3rd rung on the ladder landing on elbow forcing shoulder upward.

Fall onto or against objects

Bolt man went to walk past the bolter after hanging his curtain and tripped and fell into bolt machine.

Over-exertion (Not Elsewhere Classified)

Employee was re-entering a miner cable into the machine when he felt a sharp pain in his back.
