An unintentional roof fall occurred at teh 86+80 crosscut, entry #2 in the #2 return. The fall was 40' long, 20' wide & 7' thick.
An unintentional roof fall occurred in the old 3D travelway at #30 xcut, 18+70' entry #5. Fall was 30' long by 20' wide by 8' high.
An unintention fall occurred on 20 travelway at the 122 x/c 78+35' fall was 20' long by 20' wide by 10' high-travelway was reouted and didn't affect air.

AN UNINTENTIONAL ROOF FALL OCCURRED ON THE 3RD EAST MAIN, ENTRY #6, AT THE 31 + 50 INTERSECTION. THE FALL MEASURED 20' LONG, 20' WIDE AND 7' HIGH. NO ONE WAS INJURED AND THE PASSAGE OF MEN WAS NOT AFFECTED.
FALL OCCURRED IN THE 4 B ENTRY'S 11 + 05' IN #2 ENTRY. FALL WAS 22' LONG, 20'WIDE & 9' HIGH.

UNINTENTIONAL FALL OCCURRED AT #44 CROSSCUT IN 3RD EAST ENTRY BETWEEN TRAVELWAY & BELT ENTRIES. FALL WAS 30 LONG, 20' WIDE, 6' HIGH. BOTH ENDS OF CROSSCUT HAD BEEN CRIBBED OFF PREVIOUSLY.
An unintentional roof fall occurred at the #2 entry of 3c at the 9+10. The area was bolted with 48"" fully grouted bolts. The fall was 30'L x 20'W x 6'.

AN UNINTENTIONAL ROOF FALL OCCURRED ON THE 3A INTAKE AT 16 + 20 ENTRY #2. THE FALL MEASURED 35' X 20' X 8' HIGH. THE FALL OCCURED DUE TO DEFRACTION ABOVE THE ANCHOR POINT
A FALL OCCURRED IN THE #3 ROOM OFF 3E ENTRY'S ON 003 UNIT AT THE 0+65'. FALL WAS 25' LONG X 18' WIDE X 8' LONG.
An unintentional fall occurred at #3 xcut, 30 travelway 1 and 95' fall was 25' long, 20' wide and 10' high. Fall due to deterioration of mine strata above bolt anchorage.

An unintentional fall occurred on 004 mining unit at the 17+90' intersection on entry #2. Fall was 8' high x 20' long x 20' wide. 4' fully grouted bolts were used. Fall was due to adverse roof conditions above bolt anchorage.

An unintentional entries 8 & 9. no one was injured and ventiletion a passage of mess was not affected. the fall was caused by a hidden slip.
Employees were using a come-along to pull a new impact slide pan into a belt tail piece. They were aligning the bolt holes up and as the pan was pulled foward, his finger was caught between two pieces of metal/ Avulsion/ pinched off finger tip.
Employee was driving a personel carrier down the MN travelway, when a small piece of rock fell from the roof and struck him in the face. This caused a laceration to the employees nose. He required 4 stitches to the bridge of the nose and 2 black eyes.
Employee stopped to help the miner operator pull miner cable out of the roadway and strained his arm.
Employee was driving down the main north travelroad, when a rock fell from the roof and struck the victim on the forehead, between the eyes, causing a laceration and stitches.
Employee had been operating a underground tractor watering travelways. He had backed the water car into an area off of a intersection to begin filling it. When he went to exit the cab he started having muscle spasms in his lower back. He stated there wasn't any certain incident that caused this.
Employee was working over time shoveling on the conveyor belt. While shoveling a rock fell from the roof and struck him on the wrist and forearm.
TRAMMING BOLTER, STANDING BESIDE IT WHEN ACCIDENT HAPPENED. EE WAS INJURED WHEN HE STEPPED ON A SMALL LEDGE ON THE RIB AND SLIPPED, SPRAINING HIS RIGHT ANKLE. STARTED MISSING WORK ON 8/25/05.
Employee was helping the repairman take the electrical cable entrance junction box lid off. He was stepping backward to go get another wrench, when he stepped in a hole in the floor, injuring his ankle. Employee stated he was in a hurry.
Employee was working in the Battery Barn shop and was walking in a hurry toward a Ramcar Battery Charging. He stepped on a baseball size rock and bent his toes backwards.
Employee was bending/leaning over the side of the Roofbolter lifting a bundle of plates to move them up to his boom for use. As he was pulling on the bundle a wire which holds them together was hung on another bundle and he jerked on it to get it loose, straining his back.
Employee was lifting a Ramcar articulating hyd. cylinder upon the side of the car when he felt pain in his shoulder.
Employee was lifting and loading belt structure. Made a turn and felt pain in his left shoulder. He agravated it again hanging miner cable since. He has continued to work until 03/14/2006. Having surgery 03/15/2006.
Employee was crossing over a conveyor belt and when he stepped down off of the belt he stepped on a rock and twisted his ankle.
Employee was helping change out the Coal Hauler Batteries. He was handling the Jumper cable used in this process when he felt pain in his right shoulder. Employee did not miss any work until 07/12/2005.
Employee was a scoop operator and had just supplied the roof bolter. He had walked back to enter the operators compartment, bent down and had just stuck his head inside the cab when a rock fell from the roof (between the bolts) and struck the victim in the middle and lower back.
Employee was standing on a 8"" water line greasing the drive roller and tail rollers when he slipped off the water line and injured his knee.
EE had just got on section and was walking around the feeder to shovel the conveyor tail piece. he stepped on a piece of coal and twisted his knee.
Employee was helping pull a piece of belting upon the conveyor Tail for adding Skirt Rubber. As he was pulling he felt pain in his Lower/Upper Back between his shoulder blades. Didn't miss work until 4/7/2005.
EE WAS TRAMMING THE CONT MINER WHEN A SMALL SULFUR BALL FELL OUT OF THE ROOF AND STRUCK EE ON THE HEAD AND TOP OF LEFT HAND. DID NOT MISS WORK UNTIL 02/14/05
EE was using a cutting torch, laying on his side, cutting on a beam at the #1 conveyor Belt Tail. A piece of hot metal flew into EE's ear. He did not fill out an acc. report at that time. He started having trouble with the ear, so he went to the doctor & did not fill out a report until 3/29/05. He also did not miss any work until 4/21/05. Surgery, hole in eardrum.
Employee was walking up #6 entry going to the roof bolter when he stepped over a pile of loose material and twisted his ankle.
EE WAS RECLAIMING BELT STRUCTURE. AS HE WAS CARRYING A RAIL TO LOAD ON THE TRAILER, A RIB ROLLED OFF HITTING HIM IN THE RIGHT ANGLE. THE PIECE MEASURED ABOUT 1 FT THICK, 2 FT LONG, AND 2 FT HIGH.
Employee was pulling rocks off of the Continious Miner. When he pulled one off, the rock hit the ground and landed on another rock. A piece of rock flew up from the ground and struck the victim just above the eye, just above his safety glasses. This caused a small laceration that required 4 stitches.
Employee was helping a repairman repair on a Ramcar. He was using a Track Jack and as he was raising the Ramcar Stinger up, the Jack slipped out and the handle of the jack pinned the victim's hand against the roof.
Employee had finished his shift and was walking to his truck to go home. As he walked around his truck to get in it , he slipped/tripped and fell to the ground striking his hand on the truck. The accident happened on 7/26/05 at 1:15 AM, but this system would not let me put it in that way.
Employee was in the process of unloading some belt rollers off of a pallet. He had cut the metal bands and tucked it under the pallet. As he pulled the second roller off the pallet the band came loose and sprang up and struck victim on the upper forearm. Laceration(Stitches)
Employee was pushing a piece of rock off of the top of the cont. miner. As he pushed the rock another thin piece spun around and struck him on the arm.
Employee was replacing some bolts in a coal transfer chute when he slipped and grabbed the side of the chute with his hand, when he grabbed his thumb was cut or lacerated on a piece of jagged metal.
As a result of doing repetitive motions using hand tools, such as wrenches, channel locks, screwdriver, sidecuts, come-alongs in association with Belt Maint., both hands are going numb, loosing his grip in both hands and having a tendency to drop tools. Employee did not start missing work until 10/17/05; he had surgery on the 19th.
