Accident type, without injuries

An unintentional roof fall occurred on the 10th West panel at crosscut 52+60 entry #2. The fall did not affect ventilation, passage of men or cause any personal injury. The fall measured 36 feet long x 20 feet wide x 8 feet thick. The area had been roofbolted with 6 foot fully grouted roof bolts.

Accident type, without injuries

An unintentional roof fall occurred on the 3rd East intake at the 57 crosscut. The fall did not affect ventilation, passage of men or cause personal injury. The fall measured 40 feet long x 30 feet wide x 9 feet high. The area had been roofbolted with 6 foot fully grouted roof bolts.

Rock fall was in #3 Entry, 3rd Main East Intake. 15' x 15' x 6' high. 6' fully grouted bolts. The fall was in part of intersection & part in pillar line.

Rock fall was in 3rd East Intake, #2 Entry, approx. 6' high by 20' x 22'. All approaches were timbered. Fall did not affect ventilation or block escapeway.

An unintentional roof fall occurred on the 3rd east panel back belt line at 57+40 entry #4. The roof fall measured 30' long x 20' wide x 6' to 8' high. The fall did not affect ventilation or passage of men.

The fall is in 4-B intake, #2 entry, intersection, 2+60. Developed approx. 8/25/05, the fall is 25'long, 20'wide, 6'high. Bolted with 6' fully grouted bolts.

THE FALL IS IN 4-A INTAKE, #3 ENTRY, 10 + 30, APPROX. 25'LONG X 20'WIDE X 6'HIGH. DEVELOPED APPROX. 7/25/05. 6' FULLY GROUTED BOLTS.

Rock fall in 4-A intake #1 Entry at intersection 6 + 65. The fall was approx 6' high 25' long 20' wide development was approx 7/5/05. 6' fully grouted bolts.

An unintentional roof fall occurred at 11 & 70 entry #2 in the 4/5 intake. The fall did not affect ventilation. It measured 25' x 25' x 6'.

The intersection at 8+00 fell approx. 20' x 25' & 6' in height. This area was supported with 6' fully grouted glue bolts. Developed 8/1/05.

The intersection fell at 10 + 55 in #3 enty in the # 4 unit intake the fall was approx. 25' long 20' wide and 6' high. This area was mined 8/6/05. The unit is on retreat

An unintentional roof fall occurred at #16 crosscut, in the 4b intake entry #3, the fall measured 25' long by 20 wide by 6' thick.

AN UNINTENTIONAL ROOF FALL OCCURRED BETWEEN G1+00-G1+55 (2a) INTAKE ROOMS. 1+30 STATION ENTRY #3 THE FALL MEASURED 65'LONG X 20' WIDE AND 6' THICK

AN UNINTENTIONAL ROOF FALL OCCURRED IN THE 3C RETURN AT 17 + 20 ENTRIES 7 + 5. THE FALL MEASURED 100' LONG X 20' WIDE X 6' THICK. THE AREA HAD BEEN SUPPORTED WITH 48"" FULLY GROUTED ROOF BOLTS. THIS PANEL IS A RECOVERY AREA.

AN UNINTENTIONAL ROOF FALL OCCURRED IN THE 5 A RETURN BETWEEN 7 + 70 ABD 8 + 40 ENTRY #6. THE FALL MEASURED 35' LONG X 10' WIDE X 7' THICK. THE AREA HAD BEEN BOLTED WITH 72"" FULLY GROUTED ROOF BOLTS.

AN UNINTENTIONAL ROOF FALL OCCURRED BETWEEN 15 + 20 AND 15 + 90 ENTRY #6 ON THE 3C TRAVELWAY. THE FALL MEASURED 30' LONG X 20' WIDE X 6' THICK. THE AREA HAD BEEN BOLTED WITH 48"" FULLY GROUTED ROOF BOLTS.

Fall to the walkway or working surface

Employee stated that he and another laborer were carrying a joint of 6"" aluminum water line into a belt drive area when he tripped on a crib tie and fell to the ground injuring his back.

Over-exertion in lifting objects

Employee stated he was lifting a cutter motor cover plate from a box located on a wooden pallet. After he had placed the cover on the shelf he felt pain in his groin area. Employee continued to work and his fist day missed was 12/03/2006. He has undergone surgery.

Over-exertion in lifting objects

Employee bent over to get a rock from behind stinger on scoop and strained his back. This accident occurred on the mmu 002 working section.

Struck by falling object

Employee was roofbolting in the 5L entry. Employee had set the foot jack of the roofbolter on a piece of draw rock (unknowingly). When he started to drill the rock broke causing the atrs and the canopy to come down striking the employee on top of the head.

Contact with hot objects or substances

Employee stated that he was using a torch cutting an Axle Pin out from underneath a Shuttle car. A piece of hot slag popped off the pin and went into his ear. He has not missed any work. Started missing 11/27/2006 with surgery to repair a hole. This was not turned in to me until 9/27th.

Employee is an examiner and was walking a conveyor belt. He was walking down a grade at a underpass when he slipped on some debris and strained his knee.

Struck by falling object

Employee was sitting on the ground, helping the repairman fix a bad spot in the roofbolter cable. A Sulfur ball or kettle bottom fell from the roof and struck the victim on the thumb as he held the cable on the ground. It fell from about 6"" high.

Fall onto or against objects

Employee was walking and stated he tripped over a metal band and fell on his arm. Only first aid was given and he continued to work without complaint. After visiting a doctor he reported he had Olecranon Bursitis with spur. We contend that he already had this condition and being he is a mechanic it is Occup. Illness. He is scheduled for minor surgery on 10/28/2006.

Employee was roof bolting using 6' bolts. While bending the bolt for installation he felt pain in his shoulder.

Employee is a repairman and was helping work on a conveyor motor. He and two others were prying on the motor with a 6' pry bar. The bar slipped and the victim felt pain in his shoulder.

Over-exertion in lifting objects

Employee was attempting to hang the battery jumper cables up to the roof after changing the ram car batteries and felt pain in his lower back.

Fall onto or against objects

Employee was working on a new conveyor belt drive and was stepping over some concrete forms. As he stepped down over the forms his foot went off the concrete pad and he slipped and fell, striking his back and shoulder on the tire of a scoop. Employee did not miss any work until 09/08/2006 Shoulder surgery.

Employee was cleaning (shoveling) under the raw coal belt head at the divider to the raw coal shakers. Employee first missed day was 05/09/2006

EE and 4 other employees were hanging continous miner cable when EE's back got hurt. Witness said they heard his lower back pop. EE's first day missed was 05/07/2006

Fall from machine

Employee was in the prep. plant shop and was dismounting a 553 Case Bobcat. He was doing repair on this piece of equipment, hung his toe of his boot and fell to the concrete floor. He tried to catch himself and he landed on his hand and shoulder. (Left). Employee has worked every day but underwent shoulder surgery 06/26/2006.

Contact with electrical current

Employee is a Roof Bolter and had just finished bolting a place. He backed the bolter up out of the way and him and the other bolter started hanging their cable up to the roof. The cable was laying in a small mud hole and when the injured picked up on the cable he recieved an electrical shock.

Caught in, under or between a moving and a stationary object

Employee was turning a large metal cable spool as helpers were loading high voltage cable onto a trailer. The shaft that the spool was turning on was not secured and it came off the A frame holder. The spool struck the victim, knocking him into the A frame and then onto the ground. Suffered a fractured rib.

Employee was binding a load of supplies down, using a 3 in. strap with a ratchet. As he was ratcheting the strap tight he felt pain in his lower back. Did not miss any work until 02/27/2006. Going for MRI.

Employee was removing a rock that was hung in the Grizzly, using a pry bar and sledge hammer. He stated that when he got done removing the rock that he felt pain in his lower back. Employee did not miss work until 03/21/2006. He has a history of degenerative disc disease. Back surgery in 1975,1987 and 1997. MRI showed considerable degenerative problems.

EE was working at the breaker station. He was stepping down to a platform to clean the Grizzly when he slipped on a step. He caught himself before he fell down and had a strain to his abdomen. He continued to work and reported this to his supervisor on 01/11/06. He did not miss any time until 1/27/06. On 1/27/06 he had surgery for small hernia.

Over-exertion in lifting objects

EE was a Ramcar operator that was assigned to help build a concrete stopping along with a couple other ee's. As he was bending down grabbing the block, twisting & putting them in place he felt a pain in his lower back & leg. He has been sent to a neurosergeon. The MRI shows considerable degenerative disc disease with prior surgery a few years ago. Started missing 9/16/06.

Caught in, under or between a moving and a stationary object

Employee stated he was installing a Tail lift jack on a cont. miner. Two other people were holding the new jack up with a bar. The bar was ran thru the wire that holds the jack in the collapsed position. The injured had placed his hand in the jack holder trying to get some excess gob out of it. Wire broke, jack expanded and caught his hand.

Employee stated he was bolting an area that had a slicky side slip in it . He went to put his drill steel in the chuck and a huge rock fell hitting the canopy and fell toward the face. When he heard it fall he lunged and dove backwards away from it and was not hit by the rock. When he got up off the floor he had a lacerated ear lobe.

Employee was using a belt knife to cut a piece of curtain. The knife slipped causing employee to stab himself in the right thigh requiring 3 stitches.

Employee was installing belt blocks on a conveyor belt to make a splice. A clamp came off the othe side of the block causing them to come together and caught his thumb between the metal blocks. required 6 stitches.

Employeewas inside a cyclone overflow box using a straight arm chipping hammer. He was in a congested area with restrictions of movement and was having to apply pressure on the hammerwhile chipping at the ceramic. The chisel slipped and his right index finger struch a piece of sharp ceramic causing a laceration.

Struck by flying object

Employee was operating a ramcar and was going around a corner. He turned the car too sharp and hit the coal rib with his cab/canopy. The rib dislodged and some pieces of coal flew off the rib and struck the victim in the mouth. This caused a laceration to his chin that required some stitches.

Struck against stationary object

Employee was reaching into his tool box to get a pair of channel locks out. A utility knife had been left open and as he grabbed the channel locks his thumb struck the blade. Laceration / stitches

Employee was recovering equipment doors and frames. He had knocked the blocks from around the sides and the capping material from the top. As he pushed the top frame over, a straight leg support for the header sprang or kicked up, striking the employee in the eye. Recieved 3 stitches above the right eye.

Employee was operating a ram car, driving it up onto a lowboy trailer. Bridge boards had been placed in brackets on the side of the trailer. When the ram car tire hit the joint of the boards, the board sprang up on the other end. The employee had leaned out of the cab to see if he was clearing the ramps & was struck by the board. Required stitches around his right eye.

Struck by falling object

Employee had removed his gloves and was getting ready to eat lunch. A rock fell from the roof and struck him on his right hand & forearm, causing a laceration. Employee received stitches in his right hand.

Employee was helping square up a conveyor belt on the ground, for a splice. He was using a utility knife and the knife slipped and the blade struck him in the leg.

Struck by falling object

Employee had just finished loading material on the roof bolter and had walked back up to the boom controls. He had removed his gloves to eat an orange slice and noticed a couple of roof bolts that were out of place on his tray. When he reached out to straighten them up a small thin rock fell from the roof and struck his hand causing a laceration to the knuckle area.
