Struck by flying object

The subject was watching the cable on the miner. A rib blew out and struck him in the leg and ankle

Over-exertion (Not Elsewhere Classified)

Hurt his back using a pry bar on a tire. (He did not report it to anybody, we thought he had quit until he showed up to work later)

Over-exertion in pulling or pushing objects

Was pulling a pump out of the mud, and hurt his shoulder, (He did not report to his foreman, or have any witnesses) May not have been hurt. He was given light duty at doctors office, and did not come to work.

Caught in, under or between a moving and a stationary object

Workers tramming feeder on tailpiece when feeder jumped to the right pinning employee against the rib.

He was changing a bit inset on the head of the miner. While he was hitting the tool to remove the bit insert the tool bounced back and hit him in the face above the right upper lip.

Subject was holding screen in place as the bolter operator lowered the ATRS. The clamp sprang up and smacked his hand against the machine.

Struck by rolling or sliding object

He was moving dust line that was covered with coal slouphage, rib rolled and hit him in the left hand.
