Struck by falling object

The Continuous Miner had started into a left hand turn on the second lift off the pillar. A roof fall approx. 22'x25' ranging in thickness from 12"" to 22"" occurred, striking the employee, causing fatal crushing injuries.

Employee hurt knee on 11/12/2018 and had surgery. While working on a muddy section employee's knee started hurting. Employee seen the doctor on 5/21/2020 and was advised to rest the knee for two weeks.

A bundle of roof mesh was leaning against the rib, the miner positioned self between the mesh and the Roof Bolter. When the miner cut the bands on the mesh, the bundle of mesh rolled over pinning the miner against the Roof Bolter.

Operating Continuous Miner. A piece of rock, 2'x 4'x 6"" fell from the roof and landed on EE's right foot. Fractured EE's little toe.

Was prying a loose rib. When the rib came down it fell towards the employee striking employee on the right knee.

Was leveling a roll of belt that was suspended from the roof with a 3/8 in. chain. The belt shifted and smashed EE's hand between the belt and chain.

Employee was changing a tire on a buggy. EE was breaking lug nuts loose using a breaker bar, when EE felt a sharp pain in back.

Employee was loading belt structure. Employee threw a top cluster and felt left elbow pop and felt a sharp pain.

Employee was loading stopping panels onto a trailer. Felt a sharp pain in EE's right shoulder. EE continued working until 4/13/2016, when the Doctor put EE on restricted duty.

Employee was holding timber, another employee was using an axe to wedge timber in, the other EE hit the cap piece and the axe veered off and hit injured EE in the mouth.

Employee was roof bolting, the drill steel got bound up and while he was trying to get it out it broke loose and struck him on his right arm.

Employee was removing belt clusters from belt structure. He felt a sharp pain in his left shoulder. He was taken to the emergency facility and released to full duty. On 10/8/2015 he was examined by a different doctor and was put on restricted duty.

Employee was cutting a Joy nip off a pump cable, under excessive force went through cable and leather glove cutting left thumb, requiring 3 stitches.

On 6/11/2015 employee was packing 48"" belt rollers, and twisted his lower back. He continued performing his regular duties, until 7/16/2015, when he was examined by a Physician and put on light duty.

Employee was using a razor knife to cut belt skirting rubber. His knife slipped and cut the side of his left pinky finger. Required 4 stitches.

Employee was changing a cutter head motor on a continues miner. He was prying the new motor into place with a 3' pry bar. He felt a sharp pain in his abdomen. He continued working until 5/8/2015. He seen the doctor on 5/8/2015 and had surgery on 5/9/2015.

Over-exertion in lifting objects

Employee reached across the roof bolter to get a bundle of roof bolt plates. When he lifted the plates he felt a sharp pain in his lower back. He continued working for 9 days, then went to the doctor on 4/20/2015. The doctor put him on restricted work.

A rock outburst from rib struck an employee's right ear leaving a laceration.

Employee was removing a water hose from the main line. When the hose was disconnected it slipped out of his channel locks and hit him in the mouth.

Employee was loading roof matts on the roof bolter. When he stepped up on the walkway he slipped and caught his finger between the matt and the roof bolter, smashing/cutting his middle finger on his left hand.

Fall from machine

The employee had been working on/cleaning off the miner. As he stepped off the miner he missed the ""rub rail"" he intended to use as a step, and went all the way to the ground. As he came in contact with the muddy ground his left knee twisted, causing a strain on his knee.

Changing a tire on a battery scoop. He was guiding the tire onto the hub while a employee was lifting the tire in to place with a fork lift. The victim got his hand caught between the tire and the fender of the scoop, fracturing one finger and cut one finger.

Employee was assisting in changing a hydraulic pump on a face scoop. One employee was using a hammer and chisel to break the pump loose. When he struck the chisel with the hammer the chisel shot out hitting injured employee in the left eye and side of head.

Employee was installing a roof bolt, as he fed the bolt into the hole with the machine, the bolt head popped out pinching his left ring finger between the bolt and boom.

On 8/17/2013 this employee jumped off a roof bolter and twisted his left knee. He continued to work with no restriction until 10/24 2013 at which time he had surgery on his knee.

At approximately 11:15 pm 10/7/2012 a roof fall was discovered on the 4th left a panel at cross cut 3 in #3 entry (belt entry). The fall is appx:20'x 56'x 4' to 5'high.

Ran over a rock while operating a battery scoop. The jolt bounced him into the top of the cab,jaring his back. ( Complaining of upper and middle back pain)

EE was dropping a hose when a rib fell. He put his arm up to block it and it struck his right wrist.

EE was reading voltage in a shuttle car controller with a Precision volt meter when the probes on the meter blew up and coursed flash burns on his hands and face.

He was putting a cap piece under a scoop battery when the battery slipped off the scoop bucket and landed on his foot. Broke three toes and cut one toe.

The subject was building cribs. He was on a step ladder putting the last block on. The ladder slipped and he fell.

We had the cap rock come down. It was in the tailgate of the longwall. It should be noted that the fall involved the skin only and nothing above the anchorage point.

Subject was walking down beltline going towards the LW face and a rock fell out from the side of the roof screen and hit subject in the right shoulder.

Subject was carrying a yippi post on his shoulder. When he tried to drop it to the floor it caught on his clothing and it dropped straight down and hit his right foot.

The subject was moving a shield in manually. He knelled on the toes of the shield with his feet hanging off. He moved the shield towards the pan line. His right foot and left toes were crushed against the pan line and the toe of the shield.

Subject was holding screen in place as the bolter operator lowered the ATRS. The clamp sprang up and smacked his hand against the machine.

The subject was watching the cable on the miner. A rib blew out and struck him in the leg and ankle

He was moving dust line that was covered with coal slouphage, rib rolled and hit him in the left hand.

Was pulling a pump out of the mud, and hurt his shoulder, (He did not report to his foreman, or have any witnesses) May not have been hurt. He was given light duty at doctors office, and did not come to work.
