Over-exertion in pulling or pushing objects

Employee states that he was boarding a haul truck and as he pushed off with his right foot he felt popping in his right knee.

Fall to the walkway or working surface

Employee was leaning up against the loading bay door way frame and fell backwards, landing on his right side. Employee had a hand held radio fastened to his right side pant waist band. Employee was given first aid and refused transport to a medical facility. He later sought medical treatment and was diagnosed with a fracture.

Fall to the walkway or working surface

While walking to the shot he stepped into a borehole, that had been frozen over with mud, and fell twisting his left knee. As a result he has a mild sprain to his left knee.

Inhalation of radiations, caustics, toxic and noxious substances

Received notification that employee was diagnosed with simple pneumoconiosis category s/t, 1/0
