Over-exertion in lifting objects

Employee was lifting an electric motor when he strained his left shoulder.

Employee was getting down from a screw housing and twisted his right knee when stepping onto the floor.

High winds - cleaning area of debris/barrels/equipment on North side of Vehicle Shop. Basket to skid loader was blown and tumbled into employee.

EE caught the tip of his left ring finger between the tailgate and grain lock on dump trailer. He was pulling tailgate away from trailer when his ring finger got caught on grain lock severing the tip of his finger.

Employee was climbing into 777D truck, when he slipped, grabbed onto the handrails and strained his left shoulder.

Employee was exiting the forklift when his right foot slipped off. He continued hanging on with his left hand and felt a ""pop"" in his left shoulder. Prescribed Naproxen and placed on restrictions of no use of his left arm.

Employee was lifting scaffolding when he felt a pain in his back. He reported the incident but wanted to wait and see if it improved. It did not, he went to the doctor on 9/14/12. Prescribed Naproxen and Felxeril but not placed on any restrictions. On a follow up appointment on 9/21/12 the dr. ordered physical therapy and placed him on lifting/bending restrictions.

While vacuuming 952 tunnel, employee was lifting pipe in order to clear material, when he strained right shoulder.

An employee was injured while operating a guzzler truck. He re-injured an existing wound (received while off duty) that resulted in modified duty.

Employee got right thumb pinched between flange on barge and cement line.

While employee was reaching through a handrail to tighten a u bolt, the ratchet slipped from the nut causing his right wrist to strike against a sharp edge on uni strut causing a laceration. Employee received four sutures.

Employee accessed the manlift, high heat and humidity, his left hand slipped from top rail of manlift, when his hand caught on the second rail, he had pain in his left thumb. Employee reported to company however did not want medical attention. On 10/23/12 he was seen by Dr whom after an Xray determined his left thumb was dislocated.

Struck by falling object

Employee was replacing bolts on a hopper when dust fell out of the hopper and on to the employee. The force knocked his goggles off and dust entered his eyes. He was taken to the emergency room where he was prescribed Hydrocodone, Nevanac and Zymaxid and given a contact lens.

Employee was on ladder changing light bulb when dust fell from above and got into his eye.

Employee was removing a return roller, when it struck his middle finger on his right hand causing a laceration.

While draining oil from 1003 mill, a cover plate struck employee causing him to contact his lower lip and tooth with the framework. The result was a chipped tooth.
