FLAMMABLE VAPORS FROM A HYDRAULIC RESERVIOR ACCUMULATED IN THE TRUNNION GUARD. IGNITION WAS CAUSED BY CUTTING/WELDING IN THE AREA. HYDRAULIC OIL WAS CONTAINED BY A FLAMMABLE SOLVENT.

Operating skid loader, the machine started jerking violently. He strained his lower back. Controls on skid loader are very touchy.
Employee stepped out of crane onto a pile of rocks causing him to slip, twisting his left knee. Taken off work starting 1/19/05 pending surgery to repair miniscal tear.
EMPLOYEE WAS CLEANING SPILLAGE ON WALKWAY. AS HE WAS WORKING, AT THE TOE OF THE PILE THE TOP OF THE PILE STARTED SLIDING DOWN & PINNED EMPLOYEE'S RT. LEG.
EMPLOYEE WAS CLEANING #59 SCREW, TRYING TO UNPLUG IT ON THE SOUTH SIDE. HE WAS TAKING SCREW COVER BOLTS OFF AND HOT MATERIAL GUSHED OUT OF THE SEAL, FALLING ON TOP OF HIM. HE HAD PROPER GEAR ON BUT DID NOT HAVE HIS GLOVES INSIDE HIS JACKET SLEEVE. HOT MATERIAL GOT INTO HIS RIGHT GLOVE CAUSING SECOND AND THIRD DEGREE BURNS TO HIS RIGHT HAND AND WRIST.
EE was working in GBF modules. The modules were full of raw kiln feed. He was using a vac hose and grinding screens. His skin was moist from sweating. It came in contact with akaline dust, resulting in alkali burns to both shins, both forearms and both hands. The burns to his shins required skin grafts.
EE was cleaning under a slag feeder with a skid loader. There was 3 inches of clearance between the top of the loader and the bottom framework of the feeder. He went under the feeder and filled the loader bucket. When he reveresed the loader to come out, it lurched upwards pinching the 3rd and 4th fingers of his left hand between the feeder and loader controls.
PLANT LUBEMAN WAS RETURNING TO MAINTENANCE SHOP. AS HE ENTERED THE SHOP AREA, THERE WAS ANOTHER HYSTER AND TRASH DUMPSTER IN THE TRAVEL WAY. AS THE HYSTER WAS MOVING FORWARD EE LEANED TO HIS RIGHT TO MAKE SURE HYSTER WOULD CLEAR. HIS FOOT SLIPPED OFF THE FOOTREST & DANGLED OUTSIDE HYSTER AND BECAME WEDGED BETWEEN PARKED HYSTER & MOVING HYSTER.
Contract employee mashed end of both feet while operating cat 236 bobcat
The EE was working to clean rock out from the belly pan of a dozer. This task required the EE to lie on a piece of cardboard and move around underneath the equipment. While working on the dozer EE began to feel pain in his right leg. The EE reported he had originally injured his leg on 3-7-04 when he struck his leg on the fender of the 990 loader.
EE WAS WORKING IN #2 FINISH MILL BALL REMOVING SHELL LINERS FROM INSIDE OF MILL. DEVELOPED SORENESS AND STIFFNESS IN BACK FROM LIFTING LINERS, WAS SENT FOR MEDICAL EVALUATION. WAS GIVEN A PRESCRIPTION FOR MUSCLE RELAXERS AND ALSO WAS GIVEN PHYSICAL THERAPY. WAS UNABLE TO RETURN TO WORK HIS NEXT SCHEDULED SHIFT (LTA).
2 EE'S RIDING IN GATOR DROVE UP TO 3 EE PEDESTRIANS, THE PASSENGER IN THE GATOR WENT TO GET OUT AND SLIPPED. HIS FOOT HOT THE ACCELERATOR AND THE GATOR DROVE INTO THE PEDESTRIANS. ONE EE SUSTAINED A FRACTURE OF HIS LEFT ANKLE.
EMPLOYEE GOT OUT OF HIS VEHICLE IN THE EMPLOYEE PARKING LOT. THE WATER IN THE LOT FROZE OVER BECAUSE OF THE WEATHER CONDITIONS. WHILE WALKING AWAY FROM HIS VEHICLE HE SLIPPED AND FELL ON THE ICE STRIKING HIS HIP AND ELBOW. ON 2/24/2004 HE WAS SENT FOR MEDICAL EVALUATION FOR SORENESS IN ELBOW. ON MARCH 5, 2004 HE BEGAN THERAPY FOR PAIN IN HIS ELBOW.
EE WAS ATTEMPTING TO REMOVE A PUMP SCREW THAT WAS STUCK IN THE PUMP'S HOUSING. HE WAS HITTING THE SCREW SHAFT WITH A 10 LB SLEDGE HAMMER TO DRIVE IT OUT OF THE HOUSING. WHEN HE STRUCK THE SCREW SHAFT WITH THE SLEDGE HE TORE HIS RIGHT BICEP MUSCLE AND LIGAMENT WHERE IT ATTACHES TO HIS SHOULDER.
EE WAS INSTALLING A DRIVE BELT ON A SHEAVE. FINGER WAS CAUGHT BETWEEN THE DRIVE BELT AND THE SHEAVE. SHEAVE GROOVES WERE WORN, CAUSING THE EDGES TO BE SHARP. FINGER WAS SEVERELY LACERATED WHEN PRESSED INTO THE GROOVE OF THE SHEAVE.
EE went to clean #58 screw. He stood on top of screw (approx 4-5' off ground) and opened lid, inserted the air lance. He had not yet turned on the air and hot material puffed out and hit his face. He jumped down and landed in a pile of hot material located next to screw. he quikly got out of pile and ran to get help.
The employee was in the Maintenance Shop working on a gear box. He was using a hammer and a block of brass to remove the shaft from the housing. While striking the brass block the employee missed the block and struck the middle finger of his left hand. The finger was struck between the hammer and the sharp edge of the gear box.
Employee was lifting a heavy lid/door on a dust collector. he was lifting with help. he felt sudden pain, dropped the door and doubled over.
On 6-5-04 the employee was squatting down to check a railcar when his left knee popped. No medical treatment was required. On 6-21-04 the employee squatted down to pick up trash when the same knee popped again. Ice was applied to the knee and he was sent to the company doctor.
EMPLOYEES WERE CHANGING THE ""DOWN THE HOLE"" HAMMER ON THE 1R DRILL. THIS REQUIRES SOME LIFTING OF A HEAVY OBJECT AND MANEUVERING IT INTO POSITION SO THE STEEL THREADS WILL GRAB IT. WHILE EE WAS MANEUVERING THE HAMMER HE FELT A STRAIN IN THE GROIN AREA.
Lifting 50 gallon drum with some finish mill balls. Partner helping lost his grip causing injury to right arm.
EE HAD BEEN SQUATTING DOWN USING OX-ACETYLENE TORCH. WHEN HE STOOD UP HE FELT SHARP PAIN IN RIGHT KNEE. KNEE BEGAN TO SWELL AND EE FELT A LOT OF PAIN. EE WAS PUT ON LIGHT DUTY BY DOCTOR.
EE WAS BENDING OVER (DOWN) TO CHECK STARTER ON MCC. FELT PAIN IN LEFT KNEE. EE HAVING KNEE SURGERY 4/19/04.
WHILE CLEANING BRICK OUT OF KILN BY HAND, EMPLOYEE PULLED SOMETHING IN LOWER BACK.
Coal was frozen in coal silo. Employees had to use pokebar to get material to flow. Felt sudden pain in right wrist.
Employee was operating a D6 Dozer on the rock pile. Employee stated that the equipment was riding very rough and injured his back
Pins in Cat 980 G Loader broke causing the seat to slip back, causing the employee to strike his head against the rear glass.
Employees were replacing air direction vanes on the preheater coal mill. EE was turning mill with a pipe wrench on the drive shaft. The vanes were rubbing on the liner. The diver hung-up and while he was tying to turn it back the pipe wrench slipped causing him to loose his balance. He fell to the lower level, spraining his left ankle when he landed.
The power went off in the tunnel feeder. He went down to investigate and all power was off including the lights. I started to head back up to the control room and stepped in the sump pump hole and fell down. The cover was removed to clean the hole out but never replaced.
A NEARBY EMPLOYEE WAS USING AN 8 LB SLEDGEHAMMER TO REMOVE A SHAFT. THE INJURED EMPLOYEE WALKED BEHIND THE EMPLOYEE USING THE SLEDGEHAMMER AT THE MOMENT HE PUT THE SLEDGEHAMMER TO REST ON HIS SHOULDER. THE INJURED EMPLOYEE WAS HIT ON TOP OF SHOULDER WITH THE HAMMER. THE BLOW WAS WITH MINIMAL IMPACT, BUT CAUSED SLIGHT BRUISING.
EE in a manbasket working on blast pipe trolley system, using a hydraulic jack to hold shaft up. Partner went to adjust jack while ee was aliging shaft, bumped knob and jack released, employee's right thumb was smashed.
This is a repetative strain injury. The employee originally reported it to his sprvisr on 11/20/04. He requested a Dr appt on 12/03/2004 and went to the Dr on 12/04/04. There were no restrictions at that time. He returned to the Dr on 12/11/04 at which time the Dr restricted him from using a shovel.
while walking on the catwalk, the catwalk was overhung and came up on one end, causing employee to fall into hopper and screw. He twisted his left ankle and scraped his right shin.
Employee was cutting a piece of conveyor belt to use on a repair job. the location of the belt prevented the injured to use the correct procedure to position himself. as the employee was cutting the belt with a razor knife the razor knife slipped and contacted left knee.
Employee was moving clinker hammers into a metal dumpster, and strained his lower back
Employee was unchoking conditioning tower, material fell against pry bar causing employee to smash his finger between bar and housing
While standing on the second step of a 6 ft step ladder, the employee fell backwards and sprained his left wrist while trying to catch himself. The employee did not hit the ground.
Employee was operating a bobcat cleaning in the clay area. When he stopped the equipment to get off, he stepped onto a rock and rolled his left ankle. Resulting injury was diagnosed as a sprain.
Employee was trying to get a jack hammer bit disloged from the Primary Crusher. While doing this he strained his lower lumbar.
While using an air-lance to clean Swril Chamber (on pre-heater tower), a sudden blow back of hot material occured. All PPE (personal protective equipment) was used ""except"" for a nomex hood, causing burns to the face and neck.
While working on G cone crusher conveyor a wire screen guard which was wedged between conveyor and spilled material became loose as material was removed and fell approx. 2 feet and hit his left knee leaving a small puncture and bruise.
Employee was removing timbers from clinker cooler throat. he was standing on 2 timbers when he lost his balance and fell approx. 2 feet
While working in area of clay silo #1, employee lost his footing and twisted his lower back.
Drilling hole and from above hot preheater dust blew out on upper neck and lower head burning occipital scalp light duty began 8-27-04.
He was replacing screens on the wash plant and was tightening screen bolts. He then later started complaining about shoulder soreness.
EE was repairing overhead vibrator from its bracket. He felt a pull/ strain. The weight of the vibrator was not an issue, but the angle of getting the vibrator out of the bracket. The vibrator cone goes up in a 20-30 degree angle.
EMPLOYEE WAS WORKING INSIDE A FINISH MILL REPLACING LINERS. A SCAFFOLD BOARD HAD BEEN INSTALLED APPROX. 3'HIGH TO INCREASE THE REACH TO THE TOP OF THE MILL. THE SCAFFOLD BROKE, CAUSING EE TO FALL. THE EE LANDED ON HIS FEET & BRUISED RT. HEEL.
EMPLOYEE WAS PLACING A LADDER BELOW THE OPENING OF A FINISH MILL IN PREPARATIONS TO ENTER THE MILL. EE STEPPED ON A MILL BALL, SLIPPED & STRUCK HIS SHOULDER ON THE GROUND. RECEIVED FIRST AID, ICE COMPRESS. (2)DAYS LATER, WAS REFERRED TO DR. FOR FURTHER TREATMENT.
EE WAS WALKING DOWN STAIRS ON SOUTH SIDE OF GBC6. HIS RIGHT FOOT SLIPPED ON BUILDUP, CAUSING KNEE TO TWIST. THOUGHT TO BE A STRAIN UNTIL RESULTS OF MRI REVEALED FRACTURE OF TIBIA. MUST USE CRUTCHES. EE IS WORKING RESTRICTED DUTY AT THIS TIME.
Employee was moving boxes around weighing approximately 35 to 40 pounds. While moving them, he felt a sharp pain in his right should, resulting in a muscle sprain.
The employee was working in the clinker elevator pit. In order to access the north elevator slide gates he had to pass under a concrete beam. The clearance from the ground to the beam is approximately 4 feet. The employee squatted and while passing under the beam felt pain in his lower back.
Employee was climbing onto the ladder of Cat 980-G loader. With one foot on the ladder, he dropped his radio to the ground. Still with the one foot on the ladder, he bent to the ground to pick up his radio, straining his lower back
EMPLOYEE WAS ASSISTING 2 OTHER REPAIRMAN INSTALLING A BALL MILL ACCESS DOOR. A CRANE WAS BEING OPERATED BY ONE OF THE REPAIRMAN TO LIFT THE DOOR INTO POSITION. EE WAS HELPING TO GUIDE THE DOOR INTO POSITION WHEN HE INADVERTANTLY PLACED HIS HAND ON THE CRANE CABLE BLOCK. HIS LT. FINGER GOT CAUGHT BETWEEN THE CABLE & SHEAVE ON THE CRANE BLOCK. CUT & FRACTURED LT. PINKY FINGER.
The employee had positioned the vacuum truck and had stretched out the hose in preparing to vacuum out the area. The hose became disconnected from the truck and fell to the ground. The employee bent down and pulled the 6 inch hose with both hands. He moved the hose 5 feet from ground level to the truck. The employee complained of a sharp pain in his right shoulder.
Strained right wrist raising bed on gator in shop. EE put on light duty 7-13-04.
THE EMPLOYEE WAS DESCENDING STAIRS INTO A BUCKET ELEVATOR HOLE TO PERFORM REPAIRS. HE SLIPPED ON THE STAIRS & TWISTED HIS RT.KNEE. HE RECEIVED FIRST AID (ICE PACK), BUT NO RELIEF FROM THE PAIN AFTER 2 DAYS. SOUGHT MEDICAL ATTN. ON 6/18/04 WAS GIVEN RESTRICTED WORK DUTIES.
DRIVER AND PASSENGER WERE TRAVELING EAST ON THE PLANT ROAD SOUTH OF GBH8 IN A COMPANY PICKUP. DUE TO DIRTY WINDSHIELD AND MORNING SUN, DRIVER VEERED OFF ROADWAY AND STRUCK A LARGE ROCK. DRIVER SUSTAINED BRUISES THAT DID NOT REQUIRE MEDICAL ATTENTION. PASSENGER STRUCK WINDSHIELD AND DASHBOARD CAUSING BRUISES TO FACE AND A CUT ABOVE HIS RIGHT EYE REQUIRING STITCHES.
THE EMPLOYEE WAS EXITING THE OPERATOR'S CAB OF A BOBCAT SKID-STEER LOADER. WHEN HE STEPPED FROM THE LOADER HE STEPPED ON A ROCK & TWISTED HIS ANKLE.
Ee was lifting an empty 55 gal drum to stack on top of another drum. He lifted the drum and felt a snap & pain in right upper forearm.
EE STEPPED DOWN OUT OF THE DUMP TRUCK. BOTH FEET SLIPPED OUT AND HE FELL TO THE GROUND, LANDING ON HIS RIGHT HAND.
EMPLOYEE WAS ASSISTING MAINTENANCE DURING KILN OUTAAG, REPLACING BELTS ON #1 COOLER FAN. EMPLOYEE CAUGHT MIDDLE, LEFT FINGER BETWEEN BELT & SHEAVE. HE WAS SEEN BY A DR. WHO CLEANED & BANDAGED THE WOUND. AN X-RAY REVELASED A FRACTURE BETWEEN THE TIP OF HIS FINGER & HIS 1ST KNUCKLE. NO STITCHES WERE REQUIRED. EMPLOYEE WAS GIVEN A TETANUS SHOT & RETURNED TO RESTRICTED DUTY.
Employee was working on ES4 separator, while putting board into seperator his left knee buckled under. He was seen by a doctor on April 30 and placed on restrictions. Injury type is unknown at this time, pending further medical tests.
Employee stepped off a step onto a clinker ball, twisting left knee.
Employee was attempting to clear discharge chute with a pry bar. a large chunk hit the end of take bar, causing the bar to slam against side of chute cutting hand and requiring stitches.
Employee slipped on muddy steps exiting a loader and twisten his left knee. light duty.
EE GETTING IN POSITION TO CUT BELT, LANDED ON A ROCK CAUSING HIM TO JERK KNIFE BACK AND AWAY FROM BELT, CATCHING HIS LEFT INDEX FINGER. REQUIRED NINE STITCHES.
EE WAS POKING A LARGE CHUNK WITH A TOWER BAR IN THE SPRAY TOWER. A LARGE CHUNK FELL FROM ABOVE, HITTING HIS TOWER BAR AND CAUSING THE TOWER BAR TO HIT HIM ON RIGHT SIDE OF FACE NEAR RIGHT EYE.
EMPLOYEE WAS REMOVING WHITE IRON BRG. BLOCK FROM TRACTION SHAFT. EE CUT BOTH SIDES OF BEARING WITH TORCH. EE STRUCK BEARING WITH SLEDGEHAMMER & BEARING BROKE & SLIVER OF METAL BECAME EMBEDDED IN AREA ABOVE EE'S RIGHT KNEE.
EE was dropping airslide into place, as it was being lowered, ee rolled his left thumb, resulting in a small break of bone.
The employee was squatting under the back of the D-9 Dozer. He was unhooking a clevis and cable. While he was squatted the employee felt a pop in his right knee.
EE was stepping from clay drag, when he slipped and fell, jamming right thumb against conveyer housing.
Employee was working with another to remove the cover of the #6 Syngyp screw conveyor. The sections of the cover have handles on each end to assist in removal. Injured & co-worker had removed one section and were in the process of removing the second when injured began to feel pain in his left shoulder.
EE WAS WORKING WITH 2 OTHER EMPLOYEES TO REMOVE THE CRUSHER/DRYER ELEVATOR MOTOR FROM THE MOTOR HOUSING. A CHAIN FALL WAS BEING USED TO MOVE THE MOTOR APPROXIMATELY 2 FEET. THEY EACH TOOK TURNS PULLING ON THE CHAIN. DUE TO THE AREA WHERE THE MOTOR WAS LOCATED, FREE MOVEMENT WAS RESTRICTED SOMEWHAT.
THE EMPLOYEE WAS WORKING ON THE MAIN BAG HOUSE - WEST COLLECTING SCREW. THE SCREW WAS CHOCKED UP AND HE WAS DRIVING THE SLIDE GATES OUT TO DUMP THE SCREW. AS HE WAS STRIKING THE GATE HANDLE WI TH A HAMMER, HE MISSED AND HIT HIS RIGHT HAND ON THE HANDLE OF THE SLIDE.
EE BECAME DIZZY, LOST HIS BALANCE AND FELL DOWN THE STAIRS IN THE SHOP SUSTAINING A LACERATION TO HIS LEFT KNEE. REQUIRED LIGHT STITCHES.
EE WAS SENT TO SODIUM FEEDER DUE TO DEVIATION ONFLOW. EE GOT DOWN ON HIS KNEES TO HIT THE CHUTE BELOW THE FEEDER WITH HIS HAMMER. HE PLACED HIS HAND ON THE SOUTH SIDE OF THE DRIVE CHAIN GUARD. HIS RIGHT FINGER SLIPPED INTO THE GAP BETWEEN THE GUARD & BACK PLATE & ENTERED THE PINCH POINTBETWEEN THE SPROCKET & CHAIN. THE TIP OF HIS RIGHT MIDDLE FINGER WAS LACERATED AT THE TIP PULLIN
LIFTING CEMENT PUMP BEARING ON #3 MILL (WEIGHT ABOUT 200-250 LB), HE FELT A PULL IN RIGHT BICEP. ON THE 29TH, WALKING UP STAIRS, HE REACHED OUT AND FELT A SHARP PAIN IN HIS RIGHT SHOULDER. RES TRICTED DUTY AND PHYSICAL THERAPY ON 2/29/04.
EMPLOYEE LIFTED AN AIRSLIDE (PARTIALLY EMPTY) WITH ANOTHER EMPLOYEE. HE FELT IMMEDIATE PAIN IN THE GROIN AREA. STRAIN OF GROIN - NO HERNIA - RESTRICTED DUTY.
JACKHAMMERING MATERIAL UNDER 629-2 BELT IN DUST HOPPER, JACKHAMMER GOT STUCK IN A HOLE. WHEN EMPLOYEE TRIED TO PULL IT OUT OF THE HOLE HE STRAINED HIS LOWER BACK.
FATIGUE HAMMER SLIPPED & STRUCK HAND BELOW LEFT THUMB. OTHER FACTOR HITTING RUBBER, HELPED CONTRIBUTE TO FATIGUE.
WHILE BRICKING KILN (HANDLING BRICK) HE STRUCK HIS RIGHT SHOULDER AND NECK ON THE BRICK PLATFORM.
The employee spent the morning cleaning around the tail pulley at the rock reclaim. After lunch he began shoveling at the Crusher/Dryer around the guards and air-lancing the rotors. At 2:00 P.M. he reported to Joe Roark that he had some soreness around his right elbow.
EE WAS USING A CUTTING TORCH WHEN A LEAK IN THE LINE CAUSED A FLAME THAT BURNED EE'S LEFT HAND.
EE WAS CLOSING THE INSPECTION DOOR OF THE DISCHARGE END OF A RAW MILL. THE EE'S THUMB WAS CAUGHT IN THE DOOR AND WAS BRUISED AND SCRAPED.
Employee was attempting to get sand flowing out of bottom of hopper with a pry bar. He did not stop the belt or lock it out. The moving belt caught the pry bar causing it to jerk and smashing his left hand between pry bar and hopper. Cut on knuckle of middle finger required stitches.
The employee was packing T-S masonry on the masonry packer. He raised his shield to look around and check the pallets on the palletizer when a bag blew up, blowing material into his left eye. He immediately began flushing the eye with solution.
Co-worker was attempting to align a metal liner in a Finish Mill, and while striking the casting with a metal sledgehammer, a small piece of the metal casting struck the injured miner and became embedded in the miners neck. Due to the position of the metal minor surgery was required to remove the embedded metal.
EMPLOYEE WAS CLEANING UNDER CONVEYOR BELT FEEDING ALPHA BLENDING PLANT. THE CONVEYOR FEEDING THE PLANT RAN EMPTY WHEN FEED WAS STOPPED, RESULTING IN ONE ROCK TUMBLING BACKWARDS DOWN THE BELT AND STRIKING EMPLOYEE ON THE LOWER BACKSIDE OF THE RIGHT SIDE OF HIS HEAD. REQUIRED FIVE STAPLES.
EMPLOYEE OPENED HIS POCKET KNIFE AND TRIED TO OPEN A PACKAGE. KNIFE SLIPPED AND PUNCTURED HIS HAND. HE REQUIRED 2 STITCHES.
An employee was using a grinder with a wire brush to get silicone off the edge of airslide in the Maintenance Shop and the injured ee was helping him. As injured ee turned away, a foreign object was thrown from the grinder and struck him in the eye. He wears prescription safety glasses.
EE went to retrieve a mid kiln sample. He found the port plugged so he inserted a rod to loosen material. The rod became stuck, so he had to pull hard to remove. The rod became free and hot feed spilled from the port and entered the gap between the cuff of glove and coat sleeve. 2nd degree burns to lower arm and 1st degree on upper arm.
Employee was using a pry bar to remove chunks from preheater tower. When chunk broke free, bar pinched employees little finger against access door. Injury resulted in stitches, though he was returned to full duty.
The employee struck his head, directly above his eyebrow on a pipe.
EE WAS WORKING ON CEMENT FINISHED PRODUCT COOLING TOWER. SOME CEMENT DUST FELL FROM ABOVE & WENT INTO EYES.
EMPLOYEE WAS ASSISTING IN OPENING KILN BAGHOUSE COMPARTMENT DOOR. EMPLOYEE'S HAND BECAME TRAPPED WHEN DOOR CLOSED, CATCHING 3 FINGERS ON LEFT HAND. HOT METAL DOOR AND AIR IN COMPARTMENT CAUSED SECOND DEGREE BURNS ON MIDDLE, RING, & LITTLE FINGER OF LEFT HAND.
The employee was rebagging a dust collector when dust blew into his eyes. He attempted to wash out the particles but was not successful. The employee was wearing safety glasses and a dust mask.
EE WAS PERFORMING NORMAL REPAIR DUTIES. WINDY CONDITIONS CAUSED DUST TO BECOME AIRBORNE IN THE WORK AREA AND GET INTO HIS EYES. EE WAS WEARING SAFETY GLASSES.
EE WAS THROWING A DIVERTER PADDLE ON THE STONE CHUTE FROM SILO 1&2 TO SILO 8. TO DO THIS, THE EE HAD TO PULL THE PADDLE ARM TOWARDS HIMSELF. THE PADDLE WAS HARD TO THROW BECAUSE OF WEAR AND BUILDUP. THE PADDLE ARM IS SLIGHTLY HIGHER THAN SHOULDER HEIGHT. IN THE PROCESS OF PULLING ON THE PADDLE ARM, HE STRAINED THE MUSCLES ON BOTH SIDES OF HIS NECK. THERAPY 3/29/04.
Employee was welding, torching & grinding on 3/13. The next day he thought he might have burned his right eye welding. Employee returned to TN from FL on 3/17, right eye had gotten worse, went to doctor on 3/19 and it is believed that he got debris in right eye while grinding, which caused infection.
EE was changing out a damaged screw auger. the old screw was lying on the floor adjacent to the immediate work area. the ee stepped back onto the screw auger and tripped, falling onto the auger. the screw auger scraped his lower back slightly. the ee felt o.k. at the time but began experiencing lower back pain by the 25th of march.
2 EMPLOYEE WERE TRYING TO ATTACH QUICK CONNECT COUPLING WHEN THE PRESSURE RELEASED, SPRAYING EE IN THE FACE. EE WAS CUT IN 4 TO 5 AREAS AROUND HIS RT. EYE.
EE WAS USING A JACKHAMMER TO REMOVE HARDENED MATERIAL. WHEN HE PULLED THE JACKHAMMER TOWARD HIS BODY, THE JACKHAMMER BIT CAME OUT AND FELL ON HIS LEFT INDEX FINGER.
Employee was changing batteries on the road grader. When he lifted the new battery over the wheel and down into the battery case, he felt something pop in his right wrist. He began physical therapy on 3-19-04 to strengthen his wrist.
MINER INJURED HIS LEG WHEN A TEMPORARILY WELDED SUPPORT ANGLE IRON GAVE WAY AND THE TEMPORARY PLATFORM SHIFTED SLIGHTLY. WHEN THE MINER JUMPED OFF THE SUPPORT, HE LACERATED HIS LEG ON THE COR NER OF THE ANGLE IRON.
EE STRAINED RIGHT SHOULDER AND LOWER BACK PUSHING A 4 WHEEL CART AND USING A COME-A-LONG TO MOVE A 250 F.K. COMPRESSOR. THE COMPRESSOR WEIGHS APPROX 400 LBS. HE WAS SENT FOR MEDICAL EVALUATION, AND SHORTLY PHYSICAL THERAPY 3-1-04.
EE WAS REMOVING SLUDGE/ WATER PUMP FROM BASEMENTOF UNLOADING STATION. HE PICKED THE PUMP UP TO PLACE IT ON A 2 WHEEL CART. THE PUMP WEIGHED APPROX 200 LBS. IN THE PROCESS OF LIFTING THE PUMP, HE STRAINED HIS UPPER LEFT ARM AND SHOULDER. HE WAS SENT FOR A MEDICAL EVALUATION, AND STARTED PHYSICAL THERAPY TO REHABILITATE HIS SHOULDER.
WHILE USING CUTTING TOCH HOT MATERIAL LENDED ON TABLE WHERE HE WAS RESTING HIS ARM THE STEADY THE TORCH. IT BURNED THRU HIS CLOTHING CAUSING 2ND DEGREE BURNS TO ELBOW. WENT TO URGENT CARE WHER E THEY CUT AWAY SKIN AND APPLIED OINTMENT AND BAANDAGES.
EMPLOYEE WAS INSTALLING A SCREW HOUSING TO A SCREW CONVEYOR. HE WAS WORKING OUTDOORS. THERE WAS SNOW AND ICE IN THE WORK AREA. WHEN HE WENT TO POSTION THE HOUSING INTO PLACE, HE SLIPPED ON THE ICE AND STRAINED HIS LOWER BACK. HE WAS SENT FOR MEDICAL EVALUATION ON 2/27/04. ON 3/1/04, HE BAGAN PHYSICAL THERAPY TO STRENGTHEN HIS BACK.
EE WAS WORKING ON A SCREW CONVEYOR, USING A 3' PRY BAR, WHEN HE PULLED BACK ON THE PRY BAR IT CAME LOOSE AND STRUCK HIM IN THE MOUTH, RESULTING IN SUTURED LIP.
EE WAS CUTTING CONVEYER BELT, KNIFE SLIPPED LACERATING HIS RIGHT THIGH.
Twisted right knee on material after stepping from cab of dump truck.
THIS IS AN ONGOING STRAIN PROBLEM RESULTING IN CARPAL TUNNEL SURGERY ON 11/12/04. EE WILL BE OFF WORK FOR 2 1/2 WEEKS.
The employee was weed-eating when he was exposed to poison ivy.
APPROX. 8 HOURS AFTER LEAVING WORK, EE WENT TO THE E.R. COMPLAINING OF BREATHING DIFFICULTY. DURING THE WORK SHIFT, EE DID NOT COMPLAIN OF WORK CONDITIONS OR BREATHING DIFFICULTIES.
Approx. 8 hours after leaving work, EE went to the E.R. complaining of breathing difficulty. During the work shift, EE did not complain of work conditions or breathing problems.
EMPLOYEE WAS CLIMBING LADDER ON THE PRE-HEATER, REACHED UP AND FELT SOMETHING PULL IN HIS LEFT SHOULDER. DOCTOR'S FINDINGS: IMPINGEMENT SYNDROME AND PARESTHESIA. EMPLOYEE ORDERED PHYSICAL THERAPY ON 8/5/04.
TWO MECHANICS WERE WORKING ON A ROTARY VALVE IN THE RETURN DUST SILO. THEY HAD CLOSED THE MANUAL GATE, PULLED THE END CAP THEY CLEANED OUT THE ROTARY VALVE, MATERIAL FELL FROM ABOVE AND ENTERE D HIS EYE.
