Caught in, under or between a moving and a stationary object

The deceased was fatally injured when a P&H Omega 20 ton mobile crane tipped on top of him while he attempted to exit the crane. The crane was attempting to postion a de-watering pipe when the accident occured. This accident is still under investigation by MSHA and Hazmat and United Steelworkers.

Fall from ladders

Employee was inside transition and went to get on ladder to get below to recoup duct. He swung leg over to get on ladder rung but missed it. Having thought his foot was close to rung he let his weight go to make contact with it. Since nothing was there he skidded down ladder a way and then fell off of it.

Injured sprained his left ankle when he stepped on uneven ground after dismounting his haul truck.

Employee was securing equipment with chain and chain binder. When trying to lock chain binder, the pipe binder kicked back and hit the injured in the chin.

Injured pinched his middle right finger between rod mill rod and the rod rack causing a laceration requiring sutures.

Fall onto or against objects

Injured slipped on fresh snow while getting into his pick-up truck, striking his right shin on the pick-up causing a laceration requiring sutures.

Injured fractured the tip of his left index finger while he was removing the activation cell cylinder of the NOLA silica analyzer. While he slid the cell off of the NOLA, the chain of the small chain hoist wrapped around his finger, causing the injury.
