Over-exertion (Not Elsewhere Classified)

Using a form clamp spring loaded to remove a concrete from. When ee went to push the clamp on spring was too tight, sprung back, jammed his shoulder.

Struck against stationary object

Employee was stepping off a ladder of a drill when she stepped on a rock with her left foot and twisted her ankle.

Contact with hot objects or substances

Employee was about to cut a wear plate that was plugging a feeder. As he turned on the oxygen and acetylene to the torch system and lit the system, a fire ball came from the connection and struck his left palm.

Over-exertion (Not Elsewhere Classified)

Employee was standing on a concrete slab outside the truck shop near stall one. He was looking for his truck and his right foot slipped off the right side of the concrete slab. The condition of the slab was dry.

Struck by flying object

Employee was grinding with a face shield on and when done, lifted shield and dust fell into his right eye

Employee was pulling a 15"" wrench toward him at head level. The wrench slipped and hit him on the left side of the nose.

Employee was using an air lance to clean debris from a pump impeller. Debris went into his left eye.

Employee was steam cleaning the 34EX01 excavator bucket. He had raised his hands to spray down and as he brought up the wand, the connecting coupler came apart and the hose hit him in the chest with a spray of water. This pushed mud, grease and water up under his face shield and safety glasses into his right eye.
