Employee was driving a service truck down the Colony Bay access road. Just past CP-4, employee stated he lost his brakes and was the road clear. The truck apparently hit the bank at the padded pond and went along the pond road. The truck slid into the pond. Employee was under water about 2-3 minutes. He died 2/26/04.

EMPLOYEE FINISHED LOADING THE DRILL HOLES. HE WAS ATTEMPTING TO PUT UP THE UNUSED PRIMERS INTO THE PORTABLE DAY BOX. HE SAID AT THAT TIME THE WIND BLEW THE LID OF THE DAY BOX BACK ONTO HIS LEFT HAND, CATCHING HIS HAND BETWEEN THE LID AND THE DAY BOX BODY.
Employee had complained of low back pain for several weeks. Employee stated while preparing drill steel for drilling, he felt pain in his low back. This report has been delayed waiting on the diagnosis from the treating physician.
EE, an employee of Mystic Energy's Candice No. 2 mine, was driving his own S-10 pick-up truck on the Candice No. 2 Mine access road when he collided with the rear tires on the driver's side of a Kenworth Coal Truck driven by Witness Employee of Tyler Trucking Co. LLC. EE's truck went over an embankment.
