An employee was traveling the track way on 23 right section when the roof fell inby him. He did observe the roof fall. The roof fell on 23 right no. 3 entry at survey station n. 29683. All approaches to the fall have been timbered off. The fall did not interrupt the ventilation. The fall will be cleaned up later. The fall was approximately 22' wide, 70' long and app 10' thick

A non work related accident happened on mine property. The injured had not started any duties or reported to work. The injured was traveling the Home Mtn mine access road in his personal vehicle when he stated that he fell asleep and his vehicle hit a utility pole and went over an embankment causing a laceration to his forehead and pain in his left shoulder.
The injured was operating one of the two buses that helped push a broken down tram into a side track on 18 rt section. The injured stepped out of the operators compartment of the No. 3 bus when he stepped on the side of a water line that was beside the bus. The injured twisted his right ankle, causing a fracture to his ankle.
The injured was operating a tram on 8 vent bottom when the rock dust hose started to slip off of the tram. The injured raised up out of the operators deck to put the hose back on top of the tram as he did this he accidentally hit the dead man and the tram moved forward, causing him to strike his head against the mine roof. The injured suffered a strain/sprain of the neck.
Job-related hearing loss. Workers compensation compromise settlement.
Was found on 11/08/15 at 12:43 pm by a security officer. She was unconscious and unresponsive.
