Fall from machine

While exiting the CAT 980 loader, employee was using 3 points of contact and facing the machine as required. During the descent, they believed they were on the last step and attempted to place a foot on the ground. However, they were actually on the second to last step and missed the final step, resulting in a fall to the ground that rolled their ankle.

Struck by falling object

Employee was hosing the ground under the catwalk and a 3/4"" rock fell from a pile and struck them on their shoulder.

Over-exertion (Not Elsewhere Classified)

Employee was holding a piece of angle and flipped it upside down to hold it in place to tack weld. As EE flipped it over, EE's wrist felt a pop and sharp pain that ran up to EE's elbow.

Rubbed or abraded

Employee was taking a guard off of the screening plant conveyor that is a part of the sand system. When taking off the guard, right hand was between the framework. The guard went across right hand causing some skin to get scrapped off the top of ring and middle finger.
