Struck by falling object

INJURED WAS REMOVING POWER PANEL FROM SIDEWALL WHEN A CHUNK OF LOOSE ROCK FELL BETWEEN THE SIDEWALL & HANGING SCREEN PANEL, STRIKING HIM ABOVE HIS RT. EYE.

Over-exertion in pulling or pushing objects

Injured was using a 6' scaling bar to move a large rock that was hung up in the ore pass raise, when he experienced pain in his neck and lower back. Saw physician 8/11/05.
