Struck by flying object

An employee was attempting to clean out a previously blasted vertical drill hole with an air hose. A sudden release of pressure from an unknown source inside of the hole forced material to be ejected from the hole and struck the employee. The employee passed away from the incident.

Miner was drilling a slab shot when the roof collapsed in on him causing fatal injuries.

Employee was opening a box at the receiving desk with a utility knife. The knife slipped and they cut the top of their right thumb. Employee was taken to the emergency room where they received three stitches on the top of their thumb.

EE was walking out of the headframe heading towards the mill where EE slipped and fell on the ice from freezing rain. EE landed on EE's right arm trying to catch self and fractured EE's elbow from the fall.

Rubbed or abraded

Employee was uncoupling line inside storage room and a piece of metal on the pipe clamp cut their pinky finger while they were lifting the line. Employee was taken to the emergency room and given five(5) stitches.

Employee was removing old rails from the inside ring that belongs to the mill crusher. When needing to reach the top rails the employee rolled the ring and as it took off it caught their hand between the ring and a tote that was close by.

While installing a dag the chuck on the jackleg come loose bringing the jackleg back toward two miners. One miner was struck in the cheek with the jackleg

EE was cutting insulating tape for termination in oil switch at Mahler white dole stockpile transformer bay. While cutting EE cut right hand with hook knife when removing tape. EE went to the ER and was given 4 stitches in right palm.

Fall from machine

10:35am EE was jack legging to install pins and screen in the back of Upper Mahler Incline on LT3435 when the steel broke about 5in from the machine due to a flaw. The Leg fully extended. EE went over handrail at height of around 13ft 5in landing on EE's left side & hitting EE's head. Mine Rescue stabilized and transported from heading to surface on back board and cervical collar.

IE and co-worker were tying in a round in MWD 4163. Working their way right to left, IE was past the cut tying in the last 3 holes when a chunk of ore from between 2 slips fell striking IE and the handrail. Mine Rescue was dispatched and brought IE to surface for the Ambulance. IE suffered a mild concussion, 3 staples in their head and a bruised hip.

A Miner was underground for their shift when they started experiencing chest pain and numbness in their arms. Miner was brought to surface immediately and transported to the local hospital by ambulance.

Fall from machine

Miner was riding on man carrier of tractor; the tractor was traveling up a ramp when it stalled and started rolling backwards. The miner then jumped off the moving equipment spraining left ankle.

Miners were removing tank inspection door which is steel plate 28""X40"". Top nuts were loosened and left on. The remaining nuts were removed. Miners hit door to relieve pressure. Once removing top nuts the door was still hung on studs. They tugged on door. Material shifted causing door to push employee back against the belt guard causing them to turn, twisting the right ankle.

Was walking looking up at mining area, tripped over rock, put hand out to catch fall and landed on a rock that cut hand next to thumb. Transported to surface.

Inhalation of radiations, caustics, toxic and noxious substances

Miner was operating LR1033 in New Fold escape drift when turbo caught on fire. Miner deployed fire suppression system, it did not put the fire out. Suppression system possibly malfunctioned or did not operate due to wet streams being too concentrated. Miner then used 10lb extinguisher to suppress flames. Miner was taken to local hospital for evaluation and released the same day.

An Exterminator with Orkin was walking through a storage area looking for a trap when EE tripped over an unseen object and fell directly onto left shoulder/upper arm causing a fracture.

After the end of shift an employee slipped and fell in parking lot while walking to EE's vehicle. The fall resulted in a sore shoulder that was a bruise or sprain.

Fall from machine

A miner was leaning out over a fall protection railing and in a downward direction and was standing with one foot upon a round scaling bar. The scaling bar rolled causing the miner to lose balance and fall approximately 7 feet to the ground below. As a result of this fall injury was caused to miner's shoulder.

The miner was standing on a ladder and pushing up on a roof hatch door on the new shaft cage in the shop. The hatch door weighs approximately 40-50 lbs. The miner felt the ladder move and EE's hand slipped on the door EE was holding open causing it to come down on EE's left pinky finger tip.

Miner was drilling when the striker bar broke inside the drifter. The miner used a hammer to try and free it, the coupler was struck with the hammer causing a sliver of metal to come off and embedded in miner's left forearm just above the wrist.

Miner got brake cleaner spray in left eye, treated by a doctor and prescription provided.

Miner felt a pop from the right knee as EE was stepping up to another leveled surface. Miner woke up the next day to a swollen knee and pain that required medical treatment.

Miner was helping install long support in the ladder raise, the miner was installing a dag. The Jackleg kicked out and the dag slapped down on the miners right side of the hard hat and then the right side of the face by the right eye.

Miner was dismounting tractor, as miner stepped off the tractor miner stepped on a rock and rolled ankle.

The operator of a haul truck lost control on the main travel way. The haul truck struck the rib with a full load causing the operator to strike their head and ribs off the inside of the cab causing bruised ribs and a concussion.

Mechanics were working on a loader in the shop that was having a hydraulic issue. They removed 2 cartridges and started to remove the 3rd cartridge for the stab cylinder. When the mechanic removed the 3rd cartridge it blew out under pressure causing EE to stumble back and strike chin on the metal table.

Worker was operating the controls of the power wedge with right hand while in a kneeling position in front of the axle and brake pack when the pack suddenly loosened and moved off of the axle. The pack, which weighs approximately 300 pounds, fell approxmately 20 inches before contacting workers left hand, crushing it between the brake pack and a plastic oil pan.

Worker was clearing muck away from collar pipe. EE grabbed a rock and when EE did another rock slide down and pinched EE's left thumb causing a laceration that required surgical glue.

While manually adding a 10 foot HQ drill rod to the drill string the miner felt a strain in EE's right forearm.

Employee was inspecting trailer jack for installation. Jack was already extended, employee retracted jack by removing pin, not realizing jack was spring loaded, jack quickly retracted and caught EE's right thumb between foot and jack housing.

Miner was stepping off the jumbo drill with EE's right foot on the sill, when EE put EE's weight on EE's foot it slipped off the sill rolling the ankle. Miner was not maintaining 3 point contact when EE slipped.

Using a jackleg drill when the retainer collar came off causing the jackleg to twist pulling employee's right arm causing a pain in the right elbow. Was reported as first aid but due to on going pain the miner was treated by a doctor on 10/7/2019.

As the miner grabbed the whip-check to pull the jackleg drill down, a small piece of metal went through the glove into the right pointer finger. The next day the puncture wound was red and sore and appeared to be infected prompting the worker to seek medical attention.

Worker was using a jackleg to bolt the back in 2700-400 Mud Pond. A piece of rock about the size of a softball dislodged from the screen on the back that fell approx. 15' striking EE's hard hat and then the bridge of EE's nose. The rock caused a laceration to the bridge of EE's nose. The miner received 5 stitches and was released to work with no restrictions.

Worker was performing a pre-operational inspection of LH410 at the time of the incident. The worker was checking the filters for the air conditioner which required the hood to be raised on the loader. The worker had EE's left hand on the frame of the housing when the hood came down and struck EE's left thumb.

Miner was stripping off the sheathing from a high voltage cable at the transformer, the hook knife that the miner was using to strip the sheathing slipped causing a laceration to the little finger on the left hand requiring 3 stitches.

Contact with heat

EE was walking out of B20 drill station after returning tool when ee fainted. EE reports became weak. Transported to surface then to hospital as precaution. EE assessed and treated for dehydration and sent home. See attached reports Ambient air temp 60 deg, Fahrenheit, in fresh air ventilation drift, 5 gal water coolers at set up.

After mucking employee began setting area up to drill and became light headed. As the area had been blasted on previous shift exposure to blast gases were thought to be a potential issue. However, gas sampling preformed in area and blood gas testing revealed only a moderate level of carbon monoxide was present. 7000-1 is being submitted as a precaution.

Employee was drilling hole to install ground support, when hole intercepted previously installed rock bolt. Upon contact, the machine stalled, causing it to move upwards suddenly, striking employee on left upper lip. 5 stitches received.

Employee pinched middle finger on left hand while adjusting forks on a forklift resulting in a broken finger tip.

Employee was trouble shooting headlamp circuit on Kubota tractor. EE was standing in front of right rear tire when ee rotated ignition key. The engine started, causing the tractor to move forward, allowing the rear tire to contact left lower leg and ankle. Resulted in sprain/bruise of left lower leg.

Employee was pulling wire from the back of a tractor. Employee rolled a foot on the railroad track, stumbled and caught themselves on the back of the tractor. Employee was taken to the Emergency Room and diagnosed with a broken tibia in the left leg. Employee had surgery to repair the break.

While exiting the change house at the end of the shift, the injured employee was bitten by a bat. Employee was evaluated at the local hospital and the health department was notified. The bat was tested for rabies- test was negative.

Struck by falling object

Two miners on the rehab crew were in the process of installing 5 foot split set rock bolts with a pneumatic drill from the deck of a scissor lift truck. When the crew bottomed out the hole, the drill steel became bound inside the hole. As the operator collapsed the leg, a large slab fell from the back striking the scissor truck causing injuries to the rehab crew.

Miner was scaling sidewalls, when barring toward the wall, the bar slipped. His momentum carried him into the wall, with his left hand between the bar and wall. Pinching his finger, causing a puncture wound, that became infected.

The employee was being transported on the rear of a transport/utility tractor. He was standing on the platform with his foot in the coil of a 1"" hose the end of the hose fell free and became entangled in the rail pulling his leg into the ""safety"" rack.

Employee was scaling and a small piece of loose rock fell cutting his left wrist. The injury required three stitches.

Operator of a Getman scaler started the unit and noticed dense smoke immediately. There were no visible flames but the smoke was intense so he evacuated the area and notified supervision who in turn evacuated the mine.

After dumping a haul truck load of muck, the operator thought the box was completely down, however it wasn't and it struck the main 6"" compressed air line. The airline blew something into the cab which struck him on the left bicep causing a small laceration.

Driller was helping a mechanic lift a componant of his drill when he felt something give in his back. Resulted in a strained lower back.

While roof bolting and screening, a small piece fell through the screen and struck the injured employee on the back of his right hand, causing a laceration that required 7 stitches.

Employee was transferring fuel into a transport tank when a small amount splashed into his face and eyes. Six days later he had strong irritation to his left eye and was seen by an eye Dr. who removed a small piece of imbedded steel.

The employee stepped off a utility vehicle, placing his foot on something that caused him to twist his knee. At that time he only reported the accident to management and did not seek medical attention. After further discomfort, he visited his Doctor on 8/5/2008 and was placed on restricted duties.

EE parked his mucker, walked around it and slipped between bucket and rib, bumped elbow. This was evaluated as first aid and now being brought up to Medical Reportable. Minor out patient surgury on 4-08-08 during days off. He will return to work 4-21-08 with full release.

Miner was attempting to position roof bolt through screen when the bolt fell from his grip striking the left side of his face below his eye. The blunt force caused a laceration requiring 3 stitches.

While attempting to connect a 2"" airline with victaulic fittings the pipe slipped, striking the employee on the right hand. The blunt force impact broke the ring finger knuckle of his right hand.

The injured slipped on the operator's platform of a jumbo drill striking his knee. The injury was initially reported as a first aid to his supervisor and two days later he went to the emergency room. Diagnosed with a blood clot on his knee.

Employee was scaling his worksite when a piece of loose fell from the back. The piece of loose struck him in the left calf and ankle while he was attempting to flee.

Due to a power surge the MOV (metal oxide varister) of the Avtron electronic drive burned out shutting down the #4 Mine Hoist. The mine was safely evacuated via the #2 hoist (second way out). Night shift has been canceled.

Injured was on the ground and stepped to the edge of a scissors lift, at the same time the operator was extending the outriggers, the outrigger shoe contacted the top of his foot. Reulting in pressure, which caused bruises and a small cut.

Injured stepped from the back platform of a personnel transport vehicle and lost his balance when the operator inadvertently started to move forward. He fell to the ground landing on his battery pack causing injuries to his ribs.

Employee was installing split sets and hog panels for ground support. Driving the bolt to back small piece of rock fell out from between hog panel and back.

While bolting with a jackleg drill a small piece of loose fell from the back striking the machine and bouncing off striking the employee in the mouth chipping his front tooth.

Employee was on a scissor deck when he slipped and hit his left elbow.

Employee was dismounting the loader when he slipped and twisted his ankle. Further medical evaluation revealed a broken bone in his right heel.

Employee was walking on a muck pile slipped and fell on his left arm and bruised his left elbow.

Injured was bolting with jackleg, when he went to move it he felt a pull in his groin. The injury was reported, but he said he was fine and kept working. On 12/17/2007 he saw his family physician and surgery was performed.

Crew was in the scaling and bolting cycle of mining. Employee had finished pinning up 1st ring of bolts and wire with help of his partner. Continuing to the next ring to install of bolts, started drilling hole when loose fell out of back causing the steel and machine to be pushed down. He let go of the drill, turning away when the drill hit him in the R-leg.

The shaft employees noticed a slight deformity in the conveyance cable. Further investigations revealed this to be a minor kink, but repairs would be necessary The mine was subsequently evacuated using the secondary shaft.

Electrical power was interrupted to the 3100 vent doors; the crew attempted to open them manually. After proping the doors open, they unexpectedly shut and employee's feet were struck.

Employee was stepping back and didn't see a 6"" pipe. He placed his foot on the pipe and tripped, landing on his lower back. As a precautionary measure he was taken out on a backboard and sent to the local hospital.

EE was in work area 1972-MP bolting the back. Started pinning screen up, when the screen hinged back and hit him in the mouth. Loss of 1 tooth and loosened others in the bottom jaw. EE was with a work partner.

Employee was operating a haul truck; as he was approaching a corner he lost the steering due to a blown hose. The truck hit the sidewall and he was forced into the side of the operator's compartment striking his left arm and shoulder.

The employee was scaling the sidewall, stepped ahead under a freshly scaled section and a piece of ground, unknown size, fell striking him on the back. He was diagnosed with a pinched nerve.

Miner had left index finger and middle finger pinched between the back of the Jumbo electrical plug and the opening for the cable reel. This resulted in loss of the tip of his index finger and 3 stitches to his middle finger.

There were""shock waves"" felt underground and in some areas on the surface. There were no injuries or property damage, however the mine was evacuated and MSHA was immediately notified. An inspector was sent and mine rescue investigated the mine and the determination was made to allow work to proceed.

Mechanic was attempting to free a bushing from an LHD boom using a hammer. He missed and hit his thumb.

The #4 hoist malfunctioned and came to a controlled stop, the problem was not immediately known and the mine was evacuated using the #2 hoist (2nd way out).

The employee was removing discarded roof bolts and sreen from a waste rock dump in preparation to muck the waste rock for fill in a shop floor before concrete is poured. When he pulled a bolt free, a rock of unknown size rolled down the pile striking his right hand, causing a laceration to his middle finger.

Employee was bolting with a jackleg drill and had leaned a 10'drill steel against the rib. He hit the steel with a panel of screen knocking it over onto his left hand, causing bruising and swelling.

The miner was driving the DT1518 Mine master tractor down the roadway between MAMPVI and the 3790 face when he twisted his back. The miner stated that he did not know of a specific location in the roadway that had caused the injury, but that he had been jarred and twisted in the seat causing his back pain.

The miner was servicing underground mine equipment with 5 gallon safety cans when he picked up a full can of diesel (approx 35#). He turned to load the can on the service tractor and felt a pop in his left wrist. He continued to work the rest of his shift, but had discomfort and swelling during the night so he was taken to the Dr. the next morning.

The miner was holding open a steel ventilation door with his hand and foot to allow the airblast from the shot to pass through the opening. When the shot went off, the door was pulled from the miner's hand and struck him in the left lower leg and rib area.

Miner was standing at the MAMPVI air doors holding the door open with his hand and foot, to allow for the air blast from blasting to travel through the opening without damaging the doors. The round was initiated and the resulting concussion was enough to cause the doors to swing and strike the miner.

Miner was in the process of loading the face with a 3 bag ANFO pot, when the pot goes dry the ANFO loading hose gets an instant burst of high pressure air, which caused the end of the plastic loading hose to whip out of the hole and strike the miner above the right eye, lacerating the right eye brow and the bridge of the nose.

The operator of a 6 yd. LHD was mucking an ore pass (bore Hole). It became hung up and he was in too far when it opened up. The LHD became stuck and muck spilled into the operators compartment, some struck his left hand, causing an avulsion of the index finger and bruising to his left leg. Amendment: x-ray revealed fracture in leg.

The injured was roof bolting with a jackleg in MP2673 heading. He had collared a new hole and was approximately 3-4 inches in when two small pieces of loose fell from approximately 8 feet. One struck him on the left thumb (no injury) the second struck his right forearm causing a 2.5 inch laceration.

The injured was roof bolting with a jackleg. He started to collar a hole and a piece fell 8 feet from the roof. He had looked down at the water valve and the piece hit the steel, breaking up. One of the pieces hit the brim of his hard hat, driving it into his glasses.

Employee was cleaning lime tank #2. Despite being directed by co-workers to stay out of the tank, the injured employee entered the tank wearing work shoes of poor quality. The injured employee developed skin irritation to his feet and lower legs from the lime.

Operator of remote loader was mucking out a longhole shot when the loader ran up the right rib and tipped over onto the driver's side. The loader caught fire. Pulled scaler into heading; started scaling. A large slab came down scaler, bounced as weight came off boom, throwing operator upward. His hard hat came off and wedged between his head and cab causing a 2 inch cut on top of his head.

Three employees rolled a rock about 300 pounds off a de-energized cable, stood rock on edge. Two of them moved the cable. One EE lost balance of rock; rock rolled towards him onto his left leg. He over-extended the left knee. At hospital xrays showed nothing broken; doctor gave him tylanol and light duty release for 5/6/06.
