Struck by falling object

Employee stated that while loading a shot at the face, a hole above the employee ignited causing sand to fall on employee's back.

Struck by flying object

Employee stated that while loading a shot at the face, a shot tube hooked on the lift basket, stretched, broke, and initiated the hole.

Employee stated was standing along a conveyor while it was being started when a rock came off the conveyor and struck them on the hardhat.

Over-exertion in pulling or pushing objects

Technician was installing a suspension cylinder on a 773 Off Road truck. While attempting to push the rigged cylinder in to place technician felt a pinch and pain in lower back.

The employee stepped off the back of a truck and rolled EE's left ankle.

Fall onto or against objects

Employee stated was scaling the highwall, pulled down a piece of rock, the rock struck the bottom of the man-lift basket, but the bar was still stuck in behind the rock which caused EE to strike ribs on the man-lift railing.

Caught in, under or between a moving and a stationary object

Employee stated ee was helping splicing a belt and pinched finger in the belt clamp

Struck against stationary object

The employee stated that when stacking bolts on the roof bolter the employee cut arm on a band.
