Struck by flying object

Employee was attempting to remove a carrier roller from an Excavator undercarriage. While doing so they used a hammer to the knock the roller free from its housing. In doing so a small piece of steel splintered away from the roller and struck the employee in the sternum and embedded below the skin.

Struck against a moving object

The employee was loading out dirt from a ramp in the pit, when a large chunk of dirt fell into the bucket bouncing the loader.

The truck operator was being loaded when a large boulder, hit the bed truck it caused the truck and operator to be jolted left and right, causing pain to the operator's neck and upper back.

Over-exertion in lifting objects

The employee experienced pain in EE's right wrist after shoveling rock. The doctor prescribed physical therapy, which started on 7-12-24, making EE's injury MSHA Reportable.

The employee tried to pull on the handrail to make a large step onto the truck stairs. When doing so EE caused discomfort in their right shoulder.

The employee lost footing while cleaning up rock spill, fell on rock pile landing on chest. Employee was put on restrictions 7-12-2022 after requesting further medical observation.

The employee was loosening motor mount bolts to change drive belts, when EE slipped and fell.

Employee stated that while changing liners, a rock fell from a ledge, bounced off something, and struck EE in the mouth, breaking a tooth.

Employee stated was scaling the highwall, pulled down a piece of rock, the rock struck the bottom of the man-lift basket, but the bar was still stuck in behind the rock which caused EE to strike ribs on the man-lift railing.

Employee stated was lifting steel plate into place with chain hoist, tied the plate off to re-hook, released the chain hoist, and the chain slipped causing the chain hook to strike the employee on the cheek.

Driving a scraper from the landfill , the back of the scraper started to swing around, the employee hit a dry spot and it turned over on its side. Laceration on right hand requiring 6 stitches, possible metacarpal fracture.

EE alleges pain entire right side of spine from driving truck in quarry. EE sought treatment on his own 5 weeks ago (waiting on results to see if job related).

EE was using a 2lb sledge hammer attempting to remove a come-along safety latch that was outside the hook. He swung hammer and elbow hit guard rail causing hammer to miss latch and hit left hand causing laceration of left little finger.

EE does not know when or how accident occured. EE wanting to go to Dr 3/17/06 3:30 p.m. His eye was painfull and swollen. EE thought it could have been from 3/8/06 when he had been Welding. EE went to ER to have a piece of metal debris in his eye removed.

EE alleges he was injured on 2/9/06 but at the time he declined medical treatment.Then in 2/23/06 he was taken to SSM Health Works where an x-ray revealed him to have a low back strain. At which time EE was put on restricted activity.

EE was cleaning in the bin house and allegedly twisted his knee. He declined treatment at the time, Feb 6. On March 13th he was sent to doctor. On March 22 testing was completed. He is working full duty. No restriction. Awaiting medical opinion.

While descending the ladder of the 637 caterpillar scraper, when the last step was reached foot slipped, while falling backwards caught himself with right hand fracturing right thumb on frozen ground.

EE told others that he was coming out of the crusher where he had been welding. The others did not hear him. When he came out of the crusher he was hit in the head by a piece of steel requiring 4 staples in his head. When hit he had on a welding helmet.

A ROCK BECAME LODGED IN THE CONVEYOR BELT OF THE HEAD PULLEY OF THE 4""CONVEYOR. WHILE TRYING TO DISLODGE THE ROCK THE PRYBAR SLIPPED HITTING THE EMPLOYEE IN THE CHEEK, BELOW THE RT. EYE CAUSING A 1"" LACERATION.

Employee alleges boulders were dropped in his truck which bounced him around inside the cab. However, the ee told his superintendent that he was injured at home on April 27. The company could and would accomodate restricted duty. However, due to ee's suspected violation of company safety and health policy, the action required immediate suspension pending further investigation.

EE WAS SHOVELING WITH A HAND SHOVEL WHEN THE SHOVEL HIT THE FLOOR UNDER THE MATERIAL AND JARRED HIS RIGHT ELBOW, CAUSING SOME PAIN.

EE was greasing tail pulley on the Cedar Rapids screen when he felt rock falling on his head and shoulders and back. He got the boulder breaker operator's attention.

Employee states he was taking down a piece of belting to weld grizzly shaker, when he hit his right elbow on the drive guard. Doctor states he has a swelling and tenderness on the medial side of the right of the right elbow. Doctor gave the employee a 30 day prescription for Ibuprofen 800 mg., and released to full duty.
