Employee was attempting to remove a carrier roller from an Excavator undercarriage. While doing so they used a hammer to the knock the roller free from its housing. In doing so a small piece of steel splintered away from the roller and struck the employee in the sternum and embedded below the skin.

The employee was loading out dirt from a ramp in the pit, when a large chunk of dirt fell into the bucket bouncing the loader.

The truck operator was being loaded when a large boulder, hit the bed truck it caused the truck and operator to be jolted left and right, causing pain to the operator's neck and upper back.

The employee experienced pain in EE's right wrist after shoveling rock. The doctor prescribed physical therapy, which started on 7-12-24, making EE's injury MSHA Reportable.

The employee was while performing Maintenace, stepped down and EE's knee gave out.

The employee tried to pull on the handrail to make a large step onto the truck stairs. When doing so EE caused discomfort in their right shoulder.

The employee lost footing while cleaning up rock spill, fell on rock pile landing on chest. Employee was put on restrictions 7-12-2022 after requesting further medical observation.

The employee was loosening motor mount bolts to change drive belts, when EE slipped and fell.

Employee stated that while changing liners, a rock fell from a ledge, bounced off something, and struck EE in the mouth, breaking a tooth.

Employee stated that EE was dismounting the machine, missed the step and fell.

The employee stepped off the back of a truck and rolled EE's left ankle.

Employee stated was scaling the highwall, pulled down a piece of rock, the rock struck the bottom of the man-lift basket, but the bar was still stuck in behind the rock which caused EE to strike ribs on the man-lift railing.

Employee stated was lifting steel plate into place with chain hoist, tied the plate off to re-hook, released the chain hoist, and the chain slipped causing the chain hook to strike the employee on the cheek.

The employee was working in the hopper when a welding lead and stinger fell from above, striking him on the head, neck and shoulders.

Employee walked onto catwalk and hit his knee on mid rail.

The employee was adjusting the seat forward and struck her left knee on the dash.

Employee was using a hand held hammer to drive a 3/4"" bolt, missed the mark and struck his left thumb with the hammer. He refused medical treatment until 3/4/11 at which time x-rays revealed a fracture of the IA joint.

Employee was lying on his stomach checking the oil on a 8x20 shaker and when completed he went to sit up and felt a ""charlie horse"" in his low back.

Employee was using a power washer to clean the scale, when debris went underneath the safety glasses and into his left eye.

Driving a scraper from the landfill , the back of the scraper started to swing around, the employee hit a dry spot and it turned over on its side. Laceration on right hand requiring 6 stitches, possible metacarpal fracture.

EE was aligning holes for bolts in a slide bed on a seperator bypass conveyor. While fitting steel plate EE struck left thumb with sledge hammer.

While climbing ladder, EE slipped and fell to ground from about 7', hurting his left hand.

EE states dust blew in eye when he went outside to give driver paperwork.

EE states he was kneeling all day working on equipment when he noticed his left knee was bothering him.

EE alleges to have injured left ankle & right knee while performing maintenance on slide chute.

EE ALLEGES TO HAVE CUT HIS LEFT LONG FINGER, WHILE USING A PRY BAR.

EE alleges a piece of hot slag fell on his foot inside his work boot while welding.

EE alleges he cut his right ring finger on a bolt while removing a metal screen holding plate.

EE alleges he slipped off machine while helping remove a radiator and cut his left shin. Received stitches. No time lost.

EE states that he tripped over stacked steel on the floor and fell into a pallet of steel causing a laceration to his left wrist

EE alleges pain entire right side of spine from driving truck in quarry. EE sought treatment on his own 5 weeks ago (waiting on results to see if job related).

EE was using a 2lb sledge hammer attempting to remove a come-along safety latch that was outside the hook. He swung hammer and elbow hit guard rail causing hammer to miss latch and hit left hand causing laceration of left little finger.

EE does not know when or how accident occured. EE wanting to go to Dr 3/17/06 3:30 p.m. His eye was painfull and swollen. EE thought it could have been from 3/8/06 when he had been Welding. EE went to ER to have a piece of metal debris in his eye removed.

EE alleges he was injured on 2/9/06 but at the time he declined medical treatment.Then in 2/23/06 he was taken to SSM Health Works where an x-ray revealed him to have a low back strain. At which time EE was put on restricted activity.

EE was cleaning in the bin house and allegedly twisted his knee. He declined treatment at the time, Feb 6. On March 13th he was sent to doctor. On March 22 testing was completed. He is working full duty. No restriction. Awaiting medical opinion.

While descending the ladder of the 637 caterpillar scraper, when the last step was reached foot slipped, while falling backwards caught himself with right hand fracturing right thumb on frozen ground.

EE told others that he was coming out of the crusher where he had been welding. The others did not hear him. When he came out of the crusher he was hit in the head by a piece of steel requiring 4 staples in his head. When hit he had on a welding helmet.

Employee states he was getting in his truck when he hit his right knee on the step. He did not reort the incident to the company until 8/15/05. ""No medication was prescribed, only x-rays.""

EE ALLEGES HE GOT SOMETHING IN HIS EYE WHEN HE ROLLED DOWN HIS WINDOW TO SECURE A LOAD AT THE PORTABLE PLANT. TRUCK # 282631.

EE WAS USING A 16 LB SLEDGE HAMMER TO TAKE HAMMERS OUT OF THE MILL WHEN HE STRAINED HIS BACK.

Employee alleges he used his right hand to reach over and close the wing vent on his truck, when he hit a bump or dip in the road twisting his middle finger. Employee didn't tell anyone until 3 weeks later.

A ROCK BECAME LODGED IN THE CONVEYOR BELT OF THE HEAD PULLEY OF THE 4""CONVEYOR. WHILE TRYING TO DISLODGE THE ROCK THE PRYBAR SLIPPED HITTING THE EMPLOYEE IN THE CHEEK, BELOW THE RT. EYE CAUSING A 1"" LACERATION.

Employee alleges boulders were dropped in his truck which bounced him around inside the cab. However, the ee told his superintendent that he was injured at home on April 27. The company could and would accomodate restricted duty. However, due to ee's suspected violation of company safety and health policy, the action required immediate suspension pending further investigation.

EE WAS SHOVELING WITH A HAND SHOVEL WHEN THE SHOVEL HIT THE FLOOR UNDER THE MATERIAL AND JARRED HIS RIGHT ELBOW, CAUSING SOME PAIN.

EE was greasing tail pulley on the Cedar Rapids screen when he felt rock falling on his head and shoulders and back. He got the boulder breaker operator's attention.

Employee stated while he was getting off loader there was mud on the step, he slipped pulling his back.

Employee states he was taking down a piece of belting to weld grizzly shaker, when he hit his right elbow on the drive guard. Doctor states he has a swelling and tenderness on the medial side of the right of the right elbow. Doctor gave the employee a 30 day prescription for Ibuprofen 800 mg., and released to full duty.

EMPLOYEE STATES HE WAS CHANGING LINER ON #2 IMPACT CRUSHER WHEN THE LINER FELL, HE JERKED THE AIR HAMMER HE WAS USING AND STRAINED HIS LEFT SHOULDER.

EMPLOYEE WAS WELDING ON THE CRUSHER WHEN A PIECE OF HOT SLAG DROPPED IN HIS WORK BOOT CAUSING A BURNED RT. ANKLE.

Ee was loading bags of anfo to be delivered to the blast site. On 4-21-04 he told his supervisor that his arm was hurting and thinks it is from loading the bags of anfo.

Employee was removing a liner off separator shaft, while using a porta power hydraulic jack. The jack slipped & hit him in the elbow causing a laceration which required 2 d/t injections and 2 stitches and medication.

EMPLOYEE WAS REMOVING ICE OFF RAILROAD TRACK AT NORTH STONE WHEN HE ALLEGES HE FELT STIFFNESS IN HIS BACK THE NEXT MORNING.

WORKING ON END DUMP TRACK FROM LADDER WHEN LADDER SLIPPED CAUSING EE TO FALL TO THE GROUND 3'.

ATTEMPTING TO EXIT FLATBED TRUCK #284016 WHEN HIS BOOT CAUGHT A LIP STICKING OUT ON STEP CAUSING HIM TO FALL, STRIKING HIS FACE ON THE GROUND.

EMPLOYEE WAS STRIKING THE EDGE OF A BROKEN COUPLING, WHEN A SMALL PIECE BROKE OFF AND ENTERED EMPLOYEE'S LEFT LEG ABOVE THE KNEE. EMPLOYEE WENT TO THE DOCTOR WHERE HE RECEIVED SUTURES.

EE ALLEGES THAT WHILE PERFORMING ROUTINE MAINTENANCE IN THE PLANT HE STEPPED ON A ROCK & SPRAINED HIS ANKLE.

EE ALLEGES THAT WHILE HE WAS STARTING UP THE END DUMPS HE TWISTED HIS PELVIS WHILE CLIMBING OFF OF THE BOTTOM RUNG OF THE END DUMP LADDER.

EMPLOYEE REPORTED PAIN IN HIS WRISTS FROM PERFORMING MECHANIC DUTIES. EE ALLEGES IT IS WORK RELATED CARPAL TUNNEL SYNDROME. TO BE DETERMINED IF ILLNESS IS WORK RELATED.

EE EXITED CAB OF TAMROCK HYDRAULIC DRILL AFTER DRILLING HOLES HOLES FOR EXPLOSIVES AND STEPPED ON ROCKS TWISTING LEFT ANKLE CAUSING SPRAIN. THE FIRST AID EXCALATED TO NEEDING PHYSICAL THERAPY 3X WK. NO LOST TIME, NO RESTRICTED DUTY. UNINTENTIONAL FAILURE TO REPORT TO MSHA.

EE ALLEGES HE WAS CLIMBING DOWN THE LADDER AND STEPPED INTO A HOLE TWISTING HIS LEFT KNEE AS HE WAS TURNING TO WALK.

EMPLOYEE ALLEGES HE WAS SEATED AND TURNED TO HIS LEFT, DUCKED HIS HEAD TO EXIT THE CAB AND FELT A SHARP PAIN IN HIS BACK WHICH RAN DOWN HIS RIGHT LEG.

DRIVING END DUMP FROM PIT TO JAW WHEN EXPERIENCED EYE IRRITATION AND RUBBED EYE RESULTING IN ABRASION-RX EYEDROPS. DID NOT REALIZE THIS WAS RECORDABLE.

RAISING MOTOR WITH PRY BAR WHICH SLIPPED LANDINGON RIGHT RING FINGER

DEVELOPED TENDONITIS TRANSPORTING BOXES OF APEX FROM EXPLOSIVE TRAILER TO EXPLOSIVE TRUCK. THIS WAS FIRST AID ONLY UNTIL EE PLACED IN PHYSICAL THERAPY 11/06/01.

DURING ROUTINE MAINTENANCE LOOSE GROUND CAUSED EE TO STRAIN KNEE WHILE WALKING

AFTER CHECKING TIRE AIR PRESSURE EMPLOYEE SLIPPED WHILE CLIMBING UP LADDER TO DRIVER'S CAB & FELL APPROXIMATELY 3' TO THE GROUND.

PULLING DUMP LEVER ON HAUL TRUCK TO DUMP LOAD CAUSED INJURY/ILLNESS

CARRYING 50LB BAGS OF AMMONIUM NITRATE FUEL OIL (ANFO) CAUSED INJURY/ILLNESS

WHILE PERFORMING MAINTENANCE, EMPLOYEE GRABBED DRIVE BELT AND PULLED, CATCHING SMALL FINGER BETWEEN PULLEY AND BELT.

EMPLOYEE WAS CLIMBING DOWN A LADDER AND TWISTED ANKLE WHEN STEPPING ONTO GROUND. EMPLOYEE SPRAINED LEFT ANKLE.

LOADING BAGS INTO POWDER TRUCK IN SQUATTING AND TWISTING POSITION AND LOADING HOLES.

DURING MAINTENANCE WORK PERFORMED ON GRIZZLY, EE WENT TO SHOP TO GATHER NEW BOLTS TO REPLACE THOSE HE HAD JUST CUT OFF FROM UNDER GRIZZLY UPON REACHING INTO BOLT BINE EE FELT BACK SPASM. SPASM S GREA CONTINUOUSLY WORSE REQUIRING TRIP TO EMERGENCY ROOM.
