Caught in, under or between a moving and a stationary object

Employee was dumping a hopper into a dumpster. Employee triggered the release with his hand while standing next to the hopper. The hopper tilted forward to dump and bounced back. When the hopper came back, the employee's finger was pinched between the hopper frame and tow motor. The employee's finger-tip was broken and lacerated. The finger required 7 stitches to close the wound.

Struck by rolling or sliding object

Scaler exited scaling machine to check face. While doing so, passed by 206 lb rock leaning against face which tipped over onto his leg pinning him to the ground for 45 minutes. Radio was in cab of machine. Driller found him and freed him. Scaler was taken to hospital for assessment of leg. X-rays were taken no bones were broken. Diagnosis was contusion of right lower leg.
