Employee sustained a lower back muscle strain while lifting a 50lbs. bag of product

Fall onto or against objects

Employee's knee gave out stepping up on packaging cart and fell back hitting shoulder on pallet.

Fall onto or against objects

Employee was reaching for stretch wrap material when he slipped and fell against the machine guard, resulting in bruising on right rib area. Employee was evaluated at doctor with x-ray and found no broken ribs. Returned to work with restricted duty.
