Fall from machine

EE was clearing some rocks off bed of EE's trailer that is used to haul block and fell off trailer resulting in injury to EE's shoulder

Fall from machine

The employee was positioned at the inch control tram controls of the Fletcher HDDR Roof bolter and fell, striking the right side of EE's head, resulting in sutures to EE's right ear.

Rubbed or abraded

Positioning bolt into 6"" wrench. Operator inadvertently bumped rotation and burrs from wrench caught glove and operators hand was wrapped around bolt causing laceration to palm of hand underneath ring finger extending to middle finger.
