Found an unintentional roof fall on the #9 belt. Fall was 40' x 20' x 5'H.

He was taking a rub rail off of the miner and caught his right middle finger between the rub rail and lolo structure. Finger was amputated to the 1st joint.

Got out of mantrip car, stepped up and twisted knee. Started missing work 11-16-2010.

EE caught his left ankle between the rib and a rail motor and fractured ankle. Started missing work 10/2/10.

EE stood up in rail motor and accidentally hit the tram switch which caused him to fall down against the top of the equipment. Started missing work 9-9-2010.

EE stepped off rail motor and twisted his right knee. Started missing work 8-13-2010.

Struck by powered moving object

EE was standing beside the mine track at the portal and the rockduster wrecked off the track and caught his left leg. Fractured leg. Started missing 5-5-2010 and RTW 8-23-10 (75 days lost). EE started missing again due to complications on 11-1-2010.

Lifting jack kicked out and put pressure on arm and jerked jack bar out of hand - hurt shoulder. Started missing work 11-29-2010.

Scoop was tramming through a break and pushed a piece of 4"" waterline into employee's ankle. Fracture ankle. Started missing work 1/22/2010.

He was loading belt structure and had a pain in his groin area. Diagnosed with a hernia. Had surgery on 1/15/2010.

Beating a race into wheel and pin slipped and cut finger. 2 sutures required.

EE was cutting belt, knife slipped and he cut his left hand. 4 sutures required.

EE stood up and something in his foot popped. Diagnosed with a simple hairline fracture.

EE was tramming scoop and hit his hand on a rock and fractured his left index finger.

EE was using a lifting jack and it fell over and hit his right middle finger. Fractured finger.

EE was setting timbers and cut his right thumb on a sharp rock.

Letting bolter cable down and cable hook caught his right hand between the thumb and index finger cutting his hand. Sutures were required - no lost time.

He dropped a lifting jack on his right thumb. Thumb was fractured. Co just found out 6/4/2010 by medical records.

EE was driving a buggy in the return and ran over a rock and caused the steering wheel to hit his arm.
