Continuous Miner Operator fatality at No. 68 (Leeco Incorporated), KY, June 24, 2010
On June 24, 2010, a continuous miner operator was fatally injured in a machinery accident at No. 68 (Leeco Incorporated), KY.
Employee got caught between the continuous miner and the coal rib suffering fatal injuries.

The operator did not ensure compliance with provisions of the Approved Roof Control Plan, requiring that all persons be in a safe location away from the continuous mining machine and away from pinch points created by either the continuous mining machine and/or haulage equipment when the continuous mining machine is in operation.
