THE AREA WAS MINED 6-00 WITH A CONTINUOUS MINER AND SUPPORTED WITH 7' POINT ANCHOR BOLTS. ADDITIONALLY, CRIBS HAD BEEN INSTALLED ON BOTH SIDES OF THE ENTRY AT RIBLINE.

ROOF FALL AT 1 MAIN B TRACK AT 146 X-CUT; IT MEASURES 20'W, 20'L, 8'H, DOMED. A CLAY VEIN RAN HORIZONTALLY ACROSS INTERSECTION. THE AREA HAD BEEN MINED 12/69 WITH A CONTINUOUS MINER AND SUPP ORTED WITH 6' CONVENTIONAL ROOF BOLTS. OVER TIME, 7' AND 10' POINT ANCHOR BOLTS HAD BEEN INSTALLED RANDOMLY TO REPLACE OLDER BOLTS WHICH BECAME DISLODGED.

THE SECTION CREW WAS ADVANCING THE SECTION POWERBOX AND CABLES. THE EMPLOYEE WAS PULLING 7200V HIGH VOLTAGE CABLE UP TO BOX. HE STATED HE FELT A SHARP BURNING PAIN IN THE GROIN AREA.
EE WAS SHOVELING ALONG A BELT CONVEYOR. HE COMPLAINED OF PAIN IN HIS LEFT SHOULDER.
EE WAS TRAMMING A JEEP ON THE TRACK HAULAGE ROAD. THE TROLLEY POLE JUMPED OFF THE TROLLEY WIRE. THE EE REACHED FOR THE POLE AND STRUCK HIS HEAD ON A BEAM.
EE WAS WORKING ON A CREW SETTING UP A NEW LONGWALL FACE USING A SCOOP WITH A WINCH TO ALIGN STAGELOADER PARTS. A CHAIN CONNECTING THE COAL CHAIN TO SCOOP WINCH BROKE ALLOWING HOOK TO SWING. TH E HOOK STRUCK EE ON BACK OF NECK.
EE WAS WALKING TO A DOPZER TO BEGIN HIS SHIFT. HE STEPPED IN A HOLE, LOST HIS BALANCE, AND TWSITED HIS BACK.
EE WAS WORKING WITH OTHERS UNLOADING CRIBBING MATERIAL FROM A SUPPLY CAR. HE STEPPED ON A CRIB AND SEVERAL OTHERS ROLLED DOWN AND FELL ON HIS LEFT FOOT.
EMPLOYEE ATTEMPTED TO LIFT A CYLINDER OF COMPRESSED OXYGEN. HE STATED HE FELT PAIN IN HIS LOWER BACK.
EE WAS WORKING ON A CREW INSTALLING SHIELDS FOR THE NEXT LONGWALL FACE. AS HE ATTEMPTED TO PRESSURIZE A SHIELD AGAINST THE MINE ROOF, A VALVE ASSEMBLY CAME APART. A PORTION OF THE VALVE ASSEMB LY STRUCK THE EE IN THE AREA OF THE LEFT EYE.
AFTER HELPING TO MOVE THE CONT MINER THE EE WAS THROWING THE C/M CABLE AGAINST THE RIB HE STATEDHE FELT PAIN IN HIS LOWER BACK NOTE: THE EE DID NOT SEEK MED ATTENTION UNTIL 11/30/00 WHICH WAS HIS 1ST DAY OFF WORK
EE WAS BEING TRAINED AS A ROOF BOLTER HELPER. THEY WERE TRAMMING THE MACHINE BACKWARDS LAYING THE TRAILING CABLE OVER THE MACHINE AS THEY WENT. THE OPERATOR BEGAN TURNING THE BOLTER AROUND THE CORNER. HE HIT AN IRREGULARITY IN THE BOTTOM WHICH CAUSED THE ATRS TO SWING SLIGHTLY. THE PAD OF THE ATRS STRUCK THE INJURED EE ON THE LOWER LEFT LEG.
EE HAD FINISHED TAKING A CUT AND HAD MOVED THE CONT. MINER OUT ALLOWING THE ROOF BOLTER INTO THE CUT. BOLTERS STARTED BOLTING. INJURED ERE WALKED TO BOLTER TO TELL OPERATORS TO BE CAUTIOUS OF ROOF. AS HE STARTED TO WALK AWAY SOME LOOSE PIECES OF ROCK(@4' X 2' X 4"" THICK IN ALL) FELL FROM BETWEEN ROOF BOLTS AND STRUCK HIM ON RT SHOULDER, SCAPULA AREA.
EE WAS ROOF BOLTING. AS HE STARTED A DRILL STEELIN THE ROOF A PIECE OF ROCK 6"" X 6"" X 4"" FELL & STRUCK HIM ON RIGHT HAND CATCHING HIS HAND BETWEEN ROCK & BOOM OF ROOF BOLTER.
THE EE WAS HANGING A 2IN WATER SUCTION LINE FROMTHE ROOF, A PIECE OF ROCK 3""X3""X3"" DISLODGED FROM THE ROOF & STRUCK EE IN THE MOUTH
EE WAS TROUBLE SHOOTING AN AC/DC RELTIFIER ALONG THE TRACK HAULAGE. WHEN THE EE FLIPPED A TOGGLE SWITCH IN THE RELTIFIER A CONTACTOR ARCED CAUSING BURNS TO THE BACK OF EE RT HAND.
EE WAS WALKING THRU AN AIRCOURSE. HE OPENED A MANDOOR, STEPPED THRU WITH ONE FOOT. THE DOOR AND FRAME FELL LOOSE FROM THE STOPPING, HITTING THE EE IN THE BACK AND CAUSING HIM TO FALL FORWARD.
EE WAS CARRYING TIMBERS TO BE SET IN THE LONGWALL TAILGATE ENTRY OF THE NEXT PANEL EE FELT PAIN IN HIS LOWER BACK. HOWEVER HE DID NOT REPORT ACCIDENT NOR SEEK MEDICAL ATTENTION UNTIL 10-5-00 W HICH WAS HIS FIRST DAY OFF WORK.
EE WAS WALKING OVER AN OVERCAST ALONG THE BELT ENTRY. AS HE STEPPED OFF THE LAST STEP HE STEPPED ON AND SLIPPED ON A ROCK. EE SAID HE FELT PAIN IN HIS LEFT HEEL.
EE WAS WORKING WITH 3 OTHERS RAISING AND HANGING CONTINUOUS MINER CABLE. THE EMPLOYEES STATED HE FELT PAIN IN HIS LEFT SHOULDER.
EMPLOYEE AND ANOTHER WERE ROLLING THE H GUARD FOR THE C/M SCRUBBER OUT OF THE WALKWAY. THE EMPLOYEE FELT PAIN IN HIS LOWER BACK.
EMPLOYEE WAS OPERATING A SHUTTLE CAR, BEING LOADED AT THE CONTINUOUS MINER. THE CIM BACKED UP RAISING THE INBY END OF THE S/C. WHEN THE C/M TRAMMED INBY THE S/C DROPPED, CAUSING THE EMPLOYEE TO HIT HIS HEAD ON THE S/C CANOPY.
EE WAS SHOVELING MUCK FROM UNDER BELT ROLLERS. HE STATED HE FELT PAIN IN HIS GROIN AREA.
EMPLOYEE WAS WALKING ALONG THE SECTION TRACK ANDSTEPPED IN A HOLE, TWISTING HIS LEFT ANKLE.
EE WAS DIRECTING A CREW TRYING TO ADVANCE A LONGWALL SHIELD WHICH HAD BECOME STUCK. AS THE SHIELD CAME FREE THE TIP DROPPED AND STRUCK THE EMPLOYEE ON THE HEAD. NOTE: DID NOT SEEK MEDICAL ATTE NTION UNTIL 9/2/00 FIRST - DAY OFF WORK.
EMPLOYEE WAS INSTALLING A RAM JACK PIN ON A SHIELD. THE EMPLOYEE SLIPPED ON A PIECE OF SLATE, LOST HIS BALANCE. THE RAMJACK FELL ON THE EMPLOYEE'S RIGHT INDEX FINGER.
LIFTING LUG BROKE FROM A PIECE OF CHUTE THAT WAS BEING RAISED WITH A CHAIN FALL AND WHEN THE CHUTE FELL IT HIT THE GROUND FIRST THEN ROLLED BACK AND THE CORNER OF THE CHUTE CUT THE MAN'S RIGHT LOWER LEG.
EE WAS ATTEMPTING TO LOOSEN A BOLT/NUT ON A DEISTER TABLE HANGER USING 2 PAIRS OF CHANNEL LOCKS. EE STATED HE FELT PAIN IN HIS LEFT ELBOW. NOTE: DID NOT SEEK MEDICAL ATTENTION UNTIL 8/28/00 - FIRST DAY OFF.
EE WALKED UP TO INSPECT A PILE OF ROCK WHICH HAD BEEN BLASTED ON THE L/W FACE. A PIECE OF ROCK APPROX. 2' X 2' X 6"" ROLLED DOWN AND INTO THE WALKWAY AND STRUCK THE EE ON THE LOWER RIGHT LEG.
THE EE WAS WORKING WITH 4 OTHERS, PULLING A SECTION OF CONVEYOR BELT OVER THE BELT TAIL ROLLWE. THE EE STATED HE FELT PAIN IN HIS LWOER BACK.
EMPLOYEE WAS WALKING TOWARD THE FACE. THE BOTTOMWAS WET AND HAD SOME IRREGULARITIES DUE TO SHUTTLE CAR ROADWAYS. THE EMPLOYEE SLIPPED ON THE WETSLIPPERY BOTTOM. HE STATED HE FELT PAIN IN HIS LEFT THIGHT.
EE WAS CLEANING WALKWAYS ALONG A BELTLINE. HE PICKED UP A ROCK AND ATTEMPTED TO THROW IT ON THE BELT. AS HE DID, HIS FEET SLIPPED ON WET BOTTOM CAUSING HIM TO TWIST HIS BACK.
EMPLOYEE WAS ON TOP OF A SCOOP TRACTOR TO PLUG BATTERIES INTO CHARGE. AS HE WAS GETTING OFF THE SCOOP, HE SLIPPED AND FELL ON HIS RIGHT SHOULDER.
WHILE ROOF BOLTING THE EE ATTEMPTED TO BEND A ROOF BOLT FOR INSTALLATION. HE STEPPED ON BOLT WITH LEFT FOOT AND PUSHED. HE FELT PAIN IN LT KNEE. NOTE: EE CONTINUED TO WORK UNTIL 9-26-00 SOUGHT MEDICAL ATTENTION 9-27-00 WHICH WAS HIS FIRST DAY OFF.
EMPLOYEE WAS LIFTING THE D RING ON A RUNNING LINE TO THROW IT OVER A TIMBERHEAD. THE ROPE WAS HEAVY AND THE EMPLOYEE JERKED IT TO TRY TO RAISE THE LINE. THE EMPLOYEE FELT PAIN IN HIS RIGHT SHO ULDER.
EMPLOYEE WAS ROCKDUSTING ALONG A CONVEYER BELT LINE, AS HE WAS PULLING THE ROCKDUST HOSE, HE TRIPPED OVER A LUMP OF COAL AND FELL ON HIS LEFT KNEE.
EMPLOYEE WAS ROOF BOLTING IN THE FACE. HE HAD DRILLED A HOLE FOR A BOLT. AS HE WAS WITHDRAWINGTHE DRILL STEELS A PIECE OF ROOF ROCK (32"" X 36""X 4"" THICK SOAPSTONE) BROKE LOOSE, FELL & STRUCK EMPLOYEE ON HEAD, RIGHT FOREARM AND BACK.
EE WAS SPLICING A CONVEYOR BELT USING A 2N HANDLE LACING MACHINE. THE HANDLES BECAME STUCK AND THE EE TRIED TO FREE THEM BY JERKING ON THE HANDLES. HE FELT PAIN IN HIS RIGHT SHOULDER.
EE WAS ON TOP OF THE CONTINUOUS MINER TRYING TO REMOVE A PIECE OF LINE CAVAS WHICH HAD FALLEN AND CAUGHT ON THE MINER. AS HE PULLED UP ON THE CANVAS, HE STRUCK HIS LEFT ELBOW ON A HEADER BOARD
EE WAS WORKING ON A CREW EXTRACTING A CONTINUOUS MINER WHICH HAD BEEN COVERED IN A ROOF FALL. THE CREW HAD CONNECTED THE MINER EXTRACTOR TO THE REAR OF THE MINER AND STARTED TO TAKE UP THE SLA CK IN THE CHAIN. AS THE CHAIN TIGHTENED A CLEVIS BROKE AND A PIECE OF THE CLEVIS FLEW APPROX 55' AND STRUCK THE EE ON THE LEFT ARM.
EE HAD SWITCHED THE TRACK CLEANER OUT OF A SWITCH TO GET A FLATCAR OF CALCIUM CHLORIDE WHICH WAS BEHIND IT OUT. HE AND ANOTHER EE MOVED THE FLATCAR INBY ON THE MAINLINE AND BEGAN SPREADING THE CALCIUM, WORKING OUTBY TOWARD THE PARKED TRACK CLEANER. THE EE TRAMMED HIS MOTOR OUTBY TOO FAR AND WAS HIT BY THE DISCHARGE BOOM OF THE TRACK CLEANER.
AFTER RIDING THE MANTRIP IN, THE EMPLOYEE EXITED THE MANTRIP AND BEGAN WALKING TOWARD THE SECTION. AFTER JUST A FEW STEPS, HE STEPPED IN OR ON SOME BOTTOM IRREGULARITY AND TWISTED HIS LEFT AN KLE.
EMPLOYEE WAS IN THE PROCESS OF SETTING UP A GRINDEX ELECTRICAL MUCK PUMP, THE EMPLOYEE TRIPPED OVER A CEMENT BLOCK, WHICH WAS SUBMEGERED, AND FELL.
EMPLOYEE WAS WORKING ON EMPTY BARGES IN THE RIVER HARBOR. A CABLE (WIRE ROPE) HE WAS MOVING STARTED TO FALL INTO THE WATER. THE EMPLOYEE ATTEMPTED TO LIFT THE CABLE BACK ONTO THE BARGE.
EMPLOYEE WAS WALKING IN THE 32 ENTRY OF THE SECTION WHEN HE STEPPEDON AND SLIPPED OFF A BOTTOM IRREGULARITY. HE TWISTED HIS RIGHT KNEE.
EMPLOYEE WAS WALKING TO MANTRIP. AS HE WAS GOING THRU A CHECK CURTAIN HE TRIPPED OVER A ROCK AND FELL. HE FELT PAIN IN HIS RIGHT SHOULDER.
EE WAS OPERATING A MOTOR & STRUCK HIS HEAD ON A ROOF PLANK IN A LOW CLEARANCE ARA
EMPLOYEE WAS RIDING IN A MANTRIP INBOUND TO HIS WORK LOCATION. THE MANTRIP DERAILED AT A BROKEN RAIL. THIS CAUSED EMPLOYEE TO BE JOSTLED AROUND INSIDE THE MANTRIP.

EMPLOYEE WAS OPERATING A REMOTE CONTROL CONT MINER MOVING FROM PLACE TO PLACE. HE STPPED TRAMMING TO MOVE MINER CABLE WHICH WAS ALONGSIDE MACHINE, BUT DID NOT SHUT THE C/M OFF. AS HE WAS WALKI NG BACK AFTER MOVING CABLE HE TRIPPED, STARTED TO FALL AND ACCIDENTLY HIT A TRAM LEVER FOR THE C/M. THIS CAUSED THE C/M TO PIVET AND PIN THE EMPLOYEE AGAINST THE COAL RIB.
EE WAS PLASTERING A STOPPING. HE WAS STANDING ON A BUCKET TO REACH HIGHER. THE BUCKET TIPPED AND EMPLOYEE FELL.
EE WAS SETTING UP THE ROOF BOLTING MACHINE TO REPLACE A ROOF BOLT. AS HE WAS LOWERING THE DRILL HEAD HE ACCIDENTALLY ACTIVATED THE AUGER CLAMP. THE CLAMP CLOSED ON HIS LEFT HAND (LT 5TH FINGER ).
EMPLOYEE WAS HELPING THE HAULAGE ROOF BOLTER, SPOTTING ROOF BOLTS ALONG THE TRACK HAULAGE, THE BOLTER WAS INSTALLING BOLTS IN A CLAY VEIN AREA. AS THE EMPLOYEE PUSHED A ROOF BOLT INTO THE HOLE A SMALL PIECE OF ROOF ROCK BROKE, FELL, AND STRUCK EMPLOYEE ON LEFT SHOULDER.
EMPLOYEE WAS CARRYING AND INSTALLING RIGID BELT STRUCTURE FOR A NEW INSTALLATION. AS THE SHIFT PROGRESSED HE FELT PAIN IN HIS LOWER BACK.
THE EE WAS UNLOADING 6"" SOLID BLOCK OFF A FLATCAR. A BLOCK ROLLED OFF THE SKID AND FELL ON HIS LEFT HAND CATCHING HIS HAND BETWEEN 2 CONCRETE BLOCKS.
EE WAS CROSSING A SHUTTLE CAR CABLE. THE EE STEPPED ON THE CABLE AS THE OPERATOR STARTED THE PUMP MOTOR. THIS ""FLIPPED"" THE EE CAUSING HIM TO FALL.
EE WAS ATTEMPTING TO LOOSEN A BOLT/NUT. THE NUT WAS SO TIGHT HE TRIED HITTING THE WRENCH WITH A HAMMER. HE MISSED THE WRENCH AND STRUCK HIS RIGHT INDEX FINGER.
EE WAS WALKING ALONG THE TRACK ENTRY, THRU A MUDHOLE. HE STEPPED ON A WOODEN CAP BLOCK WHICH TURNED CAUSING HIM TO TWIST HIS RIGHT ANKLE.
EMPLOYEE WAS OPERATING THE CONTINUOUS MINER. HE STOPPED TO MOVE A SECTION OF MINER CABLE, AS HE WA PULLING THE MINER CABLE HE FELT PAIN IN HIS LOWER BACK.
EMPLOYEE WAS WORKING ON A CREW UNLOADING BUNDLESOF CRIBS OFF A FLATCAR. WHILE ROLLING A BUNDLE OFF THE CAR, HIS FOOT SLIPPED, LOST CONTROL OF CRIBS, AND BUNDLE ROLLED DOWN ON HIS RIGHT HAND.
EE WAS TRAMMING A SHUTTLE CAR. A BOLT HOLDING THE SEAT TO THE FRAME BROKE, CAUSING THE SEAT TO FALL. THE EE JAMMED HIS NECK WHEN THE SEAT HIT THE BOTTOM OF COMPARTMENT.
EE WAS WITH A CREW REPAIRING BRIDGES ON THE SECTION TRAMROADS. EE ATTEMPTED TO LIFT ONE END OF A 5"" X 7"" X 14' XBAR. HE FELT PAIN IN HIS LOWER BACK.
WHILE HELPING THE C/M OPERATOR, EE NOTCIED A PIECE OF LOOSE ROOF NEAR THE OPERATOR. SHE WENT AROUND THE C/M TO GET THE SLATE BAR. THERE WAS LOOSE ROCK ON THE BOTTOM. AS SHE STEPPED OVER THE RO CK TO GET THE BAR SHE SLIPPED AND TWISTED HER RIGHT ANKLE.
EMPLOYEE WAS WALKING UP AN ENTRY BECAUSE A SHUTTLE CAR CABLE HAD PULLED APART. THE AREA WAS WET AND SLICK, HE STEPPED ON A ROCK, SLIPPED AND FELL.
EE WAS POSITIONING A 55 GAL. DRUM OF ANTIFREEZE SO IT COULD BE PICKED UP BY A FORKLIFT. EE FELT PAIN IN HIS GROIN AREA.
EMPLOYEE WAS WALKING ALONG AN ENTRY. HE SLIPPED IN A WET AREA AND TWISTED HIS RIGHT ANKLE.
EE WAS ATTEMPTING TO CONNECT A DRAW BAR FROM A DOLLY TO A LOCOMOTIVE FOR AN EQUIPMENT MOVE. AS HE SLID THE DRAW BAR FROM THE WIDE SIDE TO THE CENTER OF THE TRACK HE CAUGHT HIS LEFT MIDDLE FING ER BETWEEN THE DRAW BAR AND THE RAIL.
EMPLOYEE WAS ROOF BOLTING ON THE SECTION. HE HADPLACED A DRILL STEEL IN THE BOLTER CHUCK AND REACHED FOR THE FEED LEVER WITH HIS OTHER HAND. HE ACCIDENTLY ACTIVATED THE STEEL CLAMP LEVER INSTE AD, AND THE CLAMP ON HIS RIGHT HAND.
EE WAS WORKING ON THE LONGWALL SET UP CREW. HE WAS USING A RAZOR KNIFE TO CUT HEMP ROPE INTO SECTIONS FOR HANGING HOSES. THE KNIFE SLIPPED AND HE CUT HIMSELF ON THE LEFT WRIST.
DURING A LONGWALL POWER MOVE THE EE WAS ATTEMPTING TO PASS A POWER CABLE PLUG THRU A HOLE IN A STOPPING. THE BOTTOM WAS WET AND MUDDY. THE EE SLIPPED AND FELL. THE CABLE PLUG FELL AND STRUCK E E ON THE HEAD.
EE WAS USING A PRY BAR WOODEN TO DISLODGED A ROCK WHICH WAS WEDGED IN A BELT TRANSFER CHUTE. THE PRY BAR SLIPPED OUT, THE EE LOST HIS BALANCE FELL FORWARD, AND CAUGHT HIS RIGHT HAND BETWEEN TH E PRY BAR AND A SECTION OF THE METAL CHUTE.
EMPLOYEE WAS ROOF BOLTING; HE WAS DRILLING ROOF BOLT HOLE. AS DRILL HEAD REACHED LEVEL WITH THEEMPLOYEE'S FACE, A PIECE OF ROOF ROCK FELL AND STRUCK THE DRILL HEAD, THEN GLANCED OFF & STRUCK EMPLOYEE ON THE MOUTH.
EMPLOYEE WAS UNLOADING TOP BELT ROLLERS OFF A CAR. EMPLOYEE STATED HE FELT PAIN IN HIS RIGHT SHOULDER.
