Fall onto or against objects

Rolling a roll of belt onto wagon, slipped and hit knee on wagon, felt pain but continued working.

Over-exertion in pulling or pushing objects

Was wearing hip boots and was pulling 28 hp submissible pump into water and felt pain in lower back.

Fall to the walkway or working surface

Loading belt rollers. Left heel caught belt and started to fall, right foot slipped and fell backwards onto ground. Felt pain in rt. hip, lower back & across shoulder. And right heel was sore.

Over-exertion (Not Elsewhere Classified)

Stepped off 20 ton motor on ""A"" track twisting right ankle. EE stated he heard & felt pain with snap in ankle area.

Fall onto or against objects

EE WAS WALKING FROM 18 X-CUT OUTBY. HE SLIPPED ON MUD AND STRUCK HIS LEFT KNEE ON CEMENT BLOCKS USED FOR SUMP.

Caught in, under or between a moving and a stationary object

HOOKING DRAW BAR UP, EE PINCHED HIS LITTLE FINGER BETWEEN CRIB AND DRAW BAR.

Struck by falling object

Employee was walking along side of right ribs which was outby face. A piece of slate/coal fell from between bolts striking his back & tail bone. Rock 5 1/2"" thick x 6' long x 3 1/2' wide.

Struck by falling object

While installing a roof bolt, a piece of slate/roof coal 2' square x 1 1/2"" thick fell out from between bolts struck the back of his calf near the ankle.

Over-exertion in lifting objects

Employee was working on a conveyor belt move, installing belt structure. Top roller was being put into place when he felt pain in his back. He did not report the accident to management. On Saturday, 2/19 he called the clinic for treatment.

Removing 6"" pipe off a mine car. Stringing out pipe, injured sat pipe down, other employee pushed pipe. Injured's rt middle finger smashed between 2 6"" pipes.

Over-exertion in lifting objects

CM was changing places - employee was helping to move miner cable. Felt pain in groin area.

Struck by falling object

Employee was roof bolting at the face, as he swung the boom to install a bolt a piece of roof rock 4 in thick by 1 ft long fell and struck ee on top of left foot.

Employee was hanging continuous miner cable. He complained of pain in his mid to low back.

Fall to the walkway or working surface

EE was standing beside the continuous miner. As he went to move, he slipped on a piece of rock, falling backwards and to the right, landing on his right shoulder.

Struck by falling object

Employee was using a spad gun to hang a piece of curtain, a small piece of rock broke off roof, fell and struck ee on upper lip.

EE WAS FOUND LYING ON THE TRACK, NOT BREATHING, NO PULSE. CPR PERFORMED. PRONOUNCED DEAD. HEART ATTACK ACCORDING MEDS. FED INVESTIGATION WAS DONE
