Over-exertion (Not Elsewhere Classified)

Employee was getting into service truck and the step which is the battery box lid came loose and caused the employee to slip. Slip resulted in a sprained knee.

Employee was working to clear jam in primary crusher. Employee was standing with feet at different levels and lower foot slipped causing the left knee to strike a metal bar. Employee finished the shift but knee later swelled up and ee was unable to work on 6-13-17. Employee's PCP set up appointment with Orthopedic doctor on 6-20-17.

Fall to the walkway or working surface

Employee lost footing in snow and landed on right shoulder. Initial diagnosis is a partially torn Rotator cuff

The truck operator was checking why a hydraulic cooling fan wasn't running. As ee checked the winning behind the fan and plugging in the wires the fan started up. A foreign object broke through the protecting cover over the fan and cut left pointer finger tip of operator, cut was repaired by 6 stitches.
