Struck by flying object

Employee was assisting to remove a piece of rock wedged between 2 wheels on truck. Removal of the rock cause the tire to explode causing small cuts to the face

Struck by flying object

Employee was trying to remove a piece of rock wedged between two wheels on truck. Removal of the rock caused the tire to explode causing facial lacerations.

Caught in, under or between a moving and a stationary object

Employee was in the process of changing a head pulley on the return conveyor belt. Despite having a sling around the shaft, when the mounting bolts were removed the head pulley shifted and employee's left hand was pinched between the pulley and framework. Employee left index finger suffered a laceration requiring 4 stitches.
