Injured was carrying two boxes of resin when he felt pain in mid-section.
Injured was helping cm operator when a piece of roll fell striking injured on back and head.
Injured was bolting top when piece of rock fell striking injured on head behind ear.
Injured was hauling MRS cable when he appeared to be shocked causing him to fall straining back. (injured was supposed to return next day-off for over week before brining excuse)
Employee was cleaning slope belt when he slipped and fell, putting his hand out to catch himself.
A piece of rock fell (about 20"" long, 12"" wide, 1"" thick) and some coal hit him. He was building an intake stopping. NOTE: We are suspicious of this alleged accident. There were no witnesses and no visible marks on his body.
Injured was handling roof drill cable when he came in contact with lead causing burn to hand.
Injured was running shuttle car and hit hole in belt entry causing head to hit canopy. Was not officially reported until 9/2/08.
Injured was tightening turnbuckle when he strained his back.
(Note: information received 8/7/08). Rib rolled off on top of operator's canopy. He flagged the injured employee around and asked him to swing the canopy in so the rock will fall off. When he did this, the rock started breaking and rolled off and onto his left foot.
Injured was lifting timber when he felt pain in groin and back area.

While getting tools from the tool box, the lid of the tool box fell down onto his left hand.
The was pulling flexible waterline under the belt when outby corner of coal seam rolled off striking him on the left leg.
Setting tail piece, Jack was setting on tail piece, fell off hitting ee on middle finger of right hand. tip
Injured was operating scoop when he ran over rock causing injured to hit back and hip against SCSR.
Injured was shoveling when he hit knee on piece of metal.
Injured was removing rock out of box (rock) when he hit hand against side.
Injured was near miner when piece of rib fell striking leg and pushing it against miner.
Injured was lifting lid on tool box when he strained back.
Injured was operating shuttle car, when he hit hole in roadway and jammed back. Did not start missing work until 2/29/08 and was unsure it was related to injury.
Injured was changing scoop battery and took piece of drill steel to knock pin out. Pressure from pin releasing caused drill steel to hit injured in head.
Injured was closing machine doors and had closed half of door when the other half closed catching injured between doors.
Employee had a utlity knife, trimming on a roll of 48"" belt. Knife slipped, cutting right arm. Cut was about 2"" to 2 1/2"" long.
He was building a stopping on belt line. While stacking the block, the metal band that held the block together was already broken and folded back behind the block. He reached for a block and the band came back over the block. He swung his arm into the band, making a small cut on left arm.
While taking the scoop off the charger, employee pulled the ground wire and the battery lid fell down striking his left hand between his thumb and finger.
While roof bolting, 2 piece steel got stuck, tried to pull apart, glove got stuck on steel and pinched hand.
Injured was taking cable down, when cable hanger slipped and the S-hook caught thumb and left hand.
