Roof fall (20'L X 18W X 8'H). Roof fall in intersection adjacent to 1 south belt 1 brk outby spad 286.

Elevator down due to car brake hanging when car coming to landing - car repaired on 7/28/11 at approximately 1:00pm.

Hoist brake car would not operate safely due to coil/contactor being bad on braking system. Contactor replaced and car was operational at approximately 6:00 am on 7/27/11.

Elevator went down - would run on inspection mode. Repaired ground wire - fixed on shift.

Encoder went down on elevator causing elevator not to run on automatic but in would run inspection speed. Replaced encoder and machine operative on 7-21 around 1:00p.m.

Fall was discovered in the #7 entry @ brk. 94 in 3 South inby.Spad# 1475. The fall was approx. 50' L. X 19' W. X 5-6' H.

Hoist experienced problems with solenoid on actuator that controls drum brake, causing hoist to not operate at all times. Men were withdrawn from mine until hoist was repaired at approximately 11:55 PM.

Employee was opening gate, tripped on gravel, fell to ground, hit left ribs, hip, leg

Over-exertion in lifting objects

Injured strained back while lifting miner cable. Did not report until 12/14/11 - Started missing 12/13/11.

Employee stated that he was putting together 4 inch waterline, and as he was clamping a coupling together the clamp sprung back and hit the index finger knuckle and middle finger knuckle of his left hand, employee stated that he was wearing his gloves.

Injured was operating miner when piece of rock fell off shuttle car striking injured on Rt foot.

Injured was prying rock out of shuttle car conv. boom when slate bar caught left hand between shuttle car and slate bar. *Started missing work on 10/10/2011.

Injured was sitting beside bolter when roof straps slipped off bolter striking left hand.

Employee was carrying belt tools and strained back.

Employee was changing shut off value at tail piece as he tighten up value nipple blew off and hit employee in the right knee.

Injured tripped over cable and strained right wrist.

Injured was unloading mtl (angle iron) when angle iron slipped catching finger between angle iron and side board of supply car.

He was at oil station and opened a can of oil. The lid popped up and hit him. Cut left arm. (Note: EE and mine are on vac at this time).

Injured was helping hand mrs cable when he tripped over cable straining left foot.

A piece of tin fell from overcast and struck employee in the back.

Crossing through man door and tripped man door fell and hit leg.

Employee reported that he stepped off a mantrip into water and stepped on a piece of rock or coal and twisted his right ankle.

Injured was moving miner cable when he felt pain in back. Did not start missing until 1/17/2011

Employee using pitot tube to measure air in a mine fan. The pitot tube got too far into the fan, the employee lost his grip on it and the pitot tube whipped around and smacked the employee's hand.

Fall down stairs

Employee walking down steps felt dizzy, caused by low blood sugar. Tripped on steps and fell, twisted right ankle.
