Employee was cleaning inside of a Reed Gunite Pot and got left hand glove caught between feed wheel and rock shear. Severed tip of middle finger and index finger.

The employee was stemming holes and while walking EE stepped off in a hole and strained left knee.

Over-exertion in lifting objects

We had two shots today. Halfway through the 2nd shot, I was moving the hose over some drill cutting piles and I felt pain in my scrotum. I went to the ER to be examined by a Dr. After receiving an ultrasound and CT scan, I was told I have 2 Inguinal Hernias that will require surgery to fix.

Struck against stationary object

Employee alleges that the tire blew out on equipment EE was operating causing injury to EE's head.

Over-exertion in lifting objects

An employee went to take a scoop of stemming rock and pulled a muscle in upper right thigh.

EMPLOYEE WAS REMOVING A BELT C-CLAMP FROM CONVEYOR BELT, SMASHED PINKY FINGER ON RIGHT HAND.

Struck against stationary object

Employee was attempting to remove a nut from a screen deck bolt. Bar that employee was prying with slipped and employee cut left hand ring finger.
